22 cases (26 eyes) of keratoconus were performed penetrating keratoplasty and followed up 0.5-5 years. The disease was effectively controlled. The rate of graft transparency was 100%, and the immune rejection rate was 7.7%. Corrected vision was 0.4 or better in 23 cases after the operation. The authors regard keratoplasty the best choice for treatment of keratoconus.

We studied Saturn II contact lenses in keratoconus patients who were intolerant to other available contact lenses. A total of 24 patients, who were followed for up to 15 months, participated in the study. Visual acuities improved significantly following fitting with the Saturn II contact lens; whereas only one eye in the series was correctable to 20/20 before fitting, eight eyes were corrected to 20/20 following fitting. We report the results obtained in two groups of 30 eyes surgically treated for keratoconus. The eyes in the first group underwent epikeratoplasty, and those in the second group, penetrating keratoplasty. All eyes had sutures removed and a follow-up period ranging from 3 to 6 years. The clinical management of 746 eyes in 417 patients referred for keratoconus from January 1984 through January 1988 was retrospectively analyzed. Five patients with Down's syndrome underwent penetrating keratoplasty for keratoconus. In three patients, the indication for surgery was acute corneal hydrops, which had not resolved in the three months before surgery. The other two patients had corneal scars. This study presents a new clinical sign, Thalasselis' syndrome, defined as: an association between keratoconus, magnesium deficiency, type-A behavior and allergy.
