[Old unreduced traumatic patellar dislocation].

The old unreduced traumatic patellar dislocation is very rare and the results of the treatment are not always satisfactory. We report a 11 year-old boy with an unreduced patellar dislocation and a healed ipsilateral femoral fracture. The dislocation was reduced by an operative procedure. A long leg cast was applied for 6 weeks then the knee exercises were started. One year after surgery, he still has satisfactory results. This case suggests that if the dislocation is not of long duration and degenerative changes are absent in the patella and the knee joint, open reduction may be helpful in the

High early failure rate of the Columbus posterior stabilized high-flexion knee prosthesis.

BACKGROUND: Most studies report high survivorship rates for TKAs, however, we observed higher than anticipated rates of dislocation and femoral component loosening after implanting a Columbus posterior stabilized prosthesis. QUESTIONS/PURPOSE: We therefore determined (1) the incidence of dislocation and aseptic loosening that occurred after implantation of posterior stabilized high-flexion prostheses in TKAs, (2) the causative factors of dislocation and aseptic femoral component loosening when comparing two designs of prostheses, and (3) the mechanisms of dislocation. METHODS: We retrospecti

Tibial tuberosity-trochlear groove distance in Chinese patients with or without recurrent patellar dislocation.

PURPOSE: To compare the tibial tuberosity-trochlear groove (TTTG) distance in Chinese patients with or without recurrent patellar dislocation. METHODS: The TTTG distance of 14 knees in 5 men and 9 women aged 16 to 38 years with recurrent patellar dislocation were compared with that of 73 control knees using magnetic resonance imaging. RESULTS: The mean TTTG distance was significantly greater in patients with recurrent patellar dislocation than in controls (16.4 mm vs. 10.1 mm, p<0.001). Six (43%) knees with recurrent patellar dislocation had the TTTG distance greater than the cut-off thresho

Comparative analysis of medial patellofemoral ligament length change pattern in patients with patellar dislocation using open-MRI.

PURPOSE: Medial patellofemoral ligament (MPFL) reconstruction has become a common form of treatment for recurrent patellar dislocation. This study was performed using open-MRI to compare the length change pattern of MPFL in patients with a history of patellar dislocation to that in healthy subjects. METHODS: The subjects comprised 10 knees of 8 males and 13 knees of 12 females with a history of one or more patellar dislocations. The length of the MPFL was measured using open-MRI in both the leg-extended position and knee-flexed positions to analyse the length change pattern. RESULTS: The ave

Clinical outcomes of medial patellofemoral ligament repair in recurrent (chronic) lateral patella dislocations.

PURPOSE: The specific aim of this project was to review a homogenous group of patients who had chosen to undergo medial patellofemoral ligament (MPFL) repair to stabilize their patella after repeat (chronic) patellofemoral dislocations and to record the success of this procedure in stabilizing the patella against further lateral dislocations. METHODS: This is a retrospective study of 55 knees in 48 patients who underwent a repair of the medial patellofemoral ligament by a single surgeon over a 4-year period. The endpoint of the study was a documented patellofemoral dislocation or a minimum 2-

The mechanism of primary patellar dislocation: trauma history of 126 patients.

BACKGROUND AND PURPOSE: Several mechanisms are responsible for patellar dislocation. We investigated how the primary pathomechanism relates to patient characteristics and the outcome. METHODS: 126 patients (81 females) with primary patellar dislocation reported the knee position before the episode, the movement during it, and whether the patella was locked in dislocation. The median age was 20 (9-47) years. The subjective outcome and Kujala, Hughston VAS, and Tegner scores were evaluated after an average of 7 years. RESULTS: 102 patients moved to flexion during the dislocation, 98 from a str

Patellar dislocation with genu valgum treated by DFO.

Congenital habitual patellar dislocation is a rare condition of the knee where the patella dislocates during flexion and relocates during extension. The congenital form is permanent, irreducible, and presents at birth. It is characterized by a short quadriceps and a major patellofemoral dysplasia and short height. This article presents a rare case of a 27-year-old woman with recurring bilateral habitual dislocation of the patella after a failed previous proximal and distal realignment procedure. Clinical examinations of both knees revealed genu valgus knees and lateral joint pain that recurred

Late results of transfer of the tibial tubercle for recurrent dislocation of the patella.

The authors wished to determine the late results of the Hauser operation, with special reference to the development of osteoarthritis. Predisposing factors associated with recurrent dislocation of the patella were also investigated. Thirty-five patients with forty-four surgically treated knees attended for review, ten to twenty-five (average sixteen) years after operation. Two patients had subsequently undergone excision of the patella. Ten patients gave a family history of recurrent dislocation of the patella and seven patients showed generalised joint laxity. Pain was present in eight knees

The cartilaginous femoral sulcus in children with patellar dislocation: an ultrasonographic study.

One hundred and sixteen knees of 33 patients with patellar dislocation and 25 normal children 12-18 years of age were examined by ultrasonography to measure the cartilaginous sulcus angle on the patellar surface of the femur. In knees with patellar dislocation, the cartilaginous sulcus angle measured between 154 and 195 degrees, exceeding the corresponding values of normal knees (134-153 degrees). In patient knees, the cartilaginous sulcus was also consistently wider than the underlying osseous sulcus. This suggests that in pediatric patients with patellar dislocation, the actual patella-stabi

The "sliver sign": a specific radiographic sign of acute lateral patellar dislocation.

OBJECTIVE: The objective of this work is to assess the prevalence of the sliver sign, defined as an intraarticular linear or curvilinear ossific density, in association with knee effusion in patients with acute knee trauma, as a predictor of recent lateral patellar dislocation (LPD). MATERIALS AND METHODS: A retrospective radiology database search for the term 'patellar dislocation' on MRI knee exams performed at our institution over a 7-year period identified 216 studies. Of these, 142 exams met true positive gold standard diagnostic criteria for LPD. Imaging findings of both the retrospecti

A Young Skier with Leg Pain.

BACKGROUND: Proximal tibiofibular joint (PTFJ) dislocation is a rare injury that can disrupt the proximal tibia-fibula joint. The abnormalities in knee x-ray imaging can be subtle and difficult to detect, requiring careful assessment. This rare cause of lateral knee pain requires a high level of suspicion for diagnosis. Treatment is closed reduction; unstable PTFJ dislocations often require surgical intervention. CASE REPORT: A 17-year-old young man presented to the emergency department (ED) with right lateral knee pain and difficulty walking after colliding with another skier 2 days prior. T

Early patellar dislocation can lead to tibial tubercle lateralization in rabbits.

PURPOSE: To investigate the effects of early patellar dislocation on the tibial tubercle location. METHODS: Sixty knees from 30 healthy 1-month-old New Zealand white rabbits were randomly divided into two groups of 30 knees each. Group A (control group) comprised the left knees, which underwent no surgical procedures. Group B comprised the right knees, which underwent patellar dislocation surgery. Computed tomography (flexion 0°) was performed preoperatively and 6 months post-operatively. Measurements included the tibial tuberosity-trochlear groove distance (TT-TG) and tibial tubercle lateral

A laterally positioned concave trochlear groove prevents patellar dislocation.

Patellofemoral instability is a disabling condition that occurs in adolescence. Recurrence after patellar dislocation has been reported in 2-50% of patients. We compared the distal femur in patients with patellofemoral instability with distal femura of a healthy cohort using mathematical quantification of two-dimensional shape variation at the same position in different knees. One hundred eight computed tomography scans from 54 patients with patellofemoral instability were compared with 197 computed tomography scans from 102 subjects with normal knees. We used principal components analysis to

[Patella dislocation with special reference to cartilage damage].

Dislocations of the patella are discussed in the light of their pathogenesis, their possible injury pattern and their consequences. An exemplary case is presented. Especially the injury to the retropatellar cartilage with following chondromalacia--sometimes initiating the development of patellofemoral osteoarthritis--renders the dislocation of the patella a severe injury of the knee region. In case of spontaneous reduction and uncertain history the diagnosis may be difficult. The dislocation of the patella can be suspected if a hemarthrosis is found without an instability of the knee ligaments

Tape versus cast for non-operative treatment of primary patellar dislocation: a randomized controlled trial.

PURPOSE: We hypothesized that taping results in better short-term functional outcome and comparable redislocation rates. METHODS: In a prospective randomised clinical trial, 18 patients with a primary patellar dislocation ≥18 years old without accompanying fractures or previous surgery to the knee were included. After 1 week of dorsal splinting, they were randomized into two groups: taping and cylinder cast immobilization. Physical examination and knee function according to the Lysholm Knee Scoring Scale were taken at 1-, 6- and 12-week and at 1- and 5-year follow-up. We also compared the red

Congenital hyper-extension with subluxation of the knees - report of 2 cases.

Congenital dislocation of the knee popularly known as genu recurvatum; is a very rare condition, about 0.45% of all congenital anomalies. Furthermore isolated genu recurvatum is extremely rare because 60% of this condition is associated with other congenital anomalies. Congenital dislocation of hip is the commonest associated anomaly. Two babies 1 male and 1 female were born with severe hyperextension deformity of the knees about 30° degrees. Clinically and radiologically they were diagnosed as isolated genu recurvatum due to congenital dislocation of the knees. Gentle manipulation in the form
