Vascular problems of the lower extremity in athletes.

The vascular anatomy of the lower extremity predisposes the popliteal area to traumatic injury owing to tethering at the adductor hiatus and the soleus arch. Knee dislocations should be followed with arteriography or exploration of the vessels. Epiphyseal separations of the proximal tibia and distal femur, as well as fractures about the knee, should be observed closely for 24 to 48 hours to ensure that the vascular status is intact. Traumatic pseudoaneurysms may arise from seemingly minor trauma and are treated surgically. The etiology of nonspecific claudication in the athletic population req

Lower limb salvage using an extended free radial forearm flap.

A case is reported of an unusual double injury, in which a lower limb salvage procedure employed a free radial forearm flap from an amputated upper limb. The use of amputated parts for salvage procedures is well documented but none has previously involved a free forearm flap. This clinical case shows that the entire forearm and hand skin can be adequately perfused on the radial artery along with the superficial palmar arch.

Single Leg Balance Training: A Systematic Review.

Single leg balance training promotes significant increments in balance control, but previous reviews on balance control have not analyzed this form of balance training. Accordingly, we aimed to review the single leg balance training literature to better understand the effects of applying this training to healthy individuals. We searched five databases-PubMed, EMBASE, Scopus, Lilacs, and Scielo-with the following inclusion criteria: (a) peer-reviewed articles published in English; (b) analysis of adult participants who had no musculoskeletal injuries or diseases that might impair balance contro

Saving the Limb-Tissue Collectors, Growth Factors, and Antibiotics in a War Injury.

ABSTRACT: As a life-saving procedure, limb amputation is frequently indicated in severe war injuries, especially in suboptimal medical suppl … year until full rehabilitation was achieved. This case illustrates the special demands in war lower leg injuries but also demonstrates that a combination of standardized and innovative treatment protocols can save a limb, even when amputation might be indicated as a first-line therapy.

Running and Jumping After Muscle Fatigue in Subjects with a History of Knee Injury: What Are the Acute Effects of Wearing a Knee Brace on Biomechanics?

The knee is one of the most frequently injured joints, involving various structures. To prevent reinjury after rehabilitation, braces are commonly used. However, most studies on knee supports focus on subjects with anterior cruciate ligament (ACL) injuries and do not account for muscle fatigue, which typically occurs during prolonged intense training and can significantly increase the risk of injury. Hence, this study investigates the acute effects of wearing a knee brace on biomechanics in subjects with a history of various unilateral knee injuries or pain under muscle fatigue. In total, 50 s

Changing Body Movement Patterns in 9-Year-Old Baseball Pitchers: A Pilot Study.

BACKGROUND: Arm injuries in throwing athletes continue to increase. Injuries may be due to multiple variables, including inefficient body movement patterns, especially in young baseball throwers. It is unclear whether these patterns can be efficiently altered in this population. PURPOSE/HYPOTHESIS: To investigate the effect of a novel 21-day throwing program on body movement patterns in youth baseball players using common practical tools. Our hypothesis was that this program would change body movement patterns over a relatively short period. STUDY DESIGN: Descriptive laboratory study. METHO

Management of acute Achilles tendinopathy: effect of etoricoxib on pain control and leg stiffness.

Tendinopathies are a major cause of disability in the athletic population; the main purpose of the treatment of these injuries is to reduce pain and improve function promptly. The objective of this randomized, active comparator controlled, blinded study was to evaluate etoricoxib efficacy in pain control and leg stiffness in athletes suffering acute unilateral Achilles tendinopathy. Fifty-six eligible male athletes (mean age 37.5 ± 11.0 y) suffering acute Achilles tendinopathy were randomized to receive either etoricoxib 120 mg oral once daily (n=28) or diclofenac 100 mg oral once daily (n=28)

The Effect of Functional Training on Dynamic Knee Valgus of Youth Soccer Players: A Pilot Study.

Knee injuries are very common in soccer players. High loads on the knee during landing or changes of direction can lead to a valgus shift of the knee, known as Dynamic Knee Valgus (DKV). Many studies have shown that a large shift in DKV is a predisposing factor for knee injuries and disease, such as anterior cruciate ligament injury (ACL), patellofemoral pain syndrome or osteoarthritis. Functional training could be a great tool to reduce DKV. Thus, in this pilot study, nine male youth soccer players (mean age: 16.4 ± 0.2 years) took part in six weeks of functional training program. DKV was mea

Quality of Life After Lower Leg Reconstruction With the Latissimus Dorsi Free Flap in Pediatric Patients.

BACKGROUND: The latissimus dorsi free flap is a widely used reconstructive technique for complex lower leg defects in the pediatric population due to its reliability and anatomical features. However, the impact of this technique on the postoperative quality of life in children and adolescents, who require appropriate lower extremity function during their developmental period, remains to be analyzed. METHODS: Patients who underwent microsurgical lower leg reconstruction using the latissimus dorsi flap were analyzed retrospectively. The quality of life of these patients was assessed prospective

Analysis of Force Mitigation by Boots in Axial Impacts using a Lower Leg Finite Element Model.

Lower extremity injuries caused by floor plate impacts through the axis of the lower leg are a major source of injury and disability for civilian and military vehicle occupants. A collection of PMHS pendulum impacts was revisited to obtain data for paired booted/unbooted test on the same leg. Five sets of paired pendulum impacts (10 experiments in total) were found using four lower legs from two PMHS. The PMHS size and age was representative of an average young adult male. In these tests, a PMHS leg was impacted by a 3.4 or 5.8 kg pendulum with an initial velocity of 5, 7, or 10 m/s (42-288 J)

A genetic model of copulatory wounding in Drosophila melanogaster.

In the animal kingdom, copulatory wounds occur due to aggressive behavior or injury from the female or male appendages in contact. Copulatory wounds are a source of sexual conflict as the injury can lead to death or infection. In the laboratory, Drosophila melanogaster does not ordinarily exhibit signs of injury during copulation, but here we find can be induced to do so by overexpression of the serine protease, Hayan, in either the male or female fruit fly. Melanin deposits mark sites of injury along the female ventral abdomen and occur following a single copulation event. The male fly mid-le

Investigating Strength and Range of Motion of the Hip Complex in Ice Hockey Athletes.

CONTEXT: Ice hockey athletes frequently injure the hip complex via a noncontact mechanism. The authors investigated patterns of strength and range of motion (ROM) to establish major differences compared with soccer athletes. Soccer athletes were compared with ice hockey athletes due to similarities between the 2 sports with regard to the intermittent nature and high number of lower-limb injuries. OBJECTIVE: To compare the differences in ROM and strength of the hip for both the dominant (Dom) and nondominant (Ndom) limbs in ice hockey and soccer athletes. DESIGN: Case-control study. SETTING:

The acute compartment syndrome following fractures of the lower leg in children.

INTRODUCTION: The acute compartment syndrome (ACS) of the lower leg is a rare but serious complication following either fractures or soft tissue injuries. An acute intervention consisting of fasciotomy is indicated as ACS may cause muscle and nerve damage. The aim of the present study was to evaluate the cause, the incidence, the time to fasciotomy and the outcome of ACS of the lower leg following fractures in a paediatric population. PATIENTS AND METHODS: A retrospective analysis of all patients with ACS following a fracture of the lower leg treated from 1998 to 2010 was performed. The time

[Rehabilitation of war injuries associated with peripheral nerve lesions].

The objective of this study was to examine frequency, characteristics and results of hospital medical rehabilitation of war injured patients with peripheral nerve lesions. War injured patients with peripheral nerve lesions make 10.83% of all war injured hospitalized patients. The major etiologic factor in these injuries were firearms. Most injured aged 26 to 36 years of age, whereas men were injured more often than women. The injuries were primarily localized in the regions of axilla, upper leg and upper arm. In regard to upper extremities injuries most often occurred on the right side or bila

[Trendy inline skating sports. Pattern of injuries and groups at risk].

One hundred and eighty-two patients presented with 200 inline-skating injuries over a 30-month period. Of these, 14% were admitted to hospital, 10% required operative treatment. Fractures (49%), contusions/lacerations (27%) and capsular/ligamentuous injuries (16%) were the most prevalent types of injury. Children had an even higher risk of sustaining fractures (62%). Falling on the extended arm caused 44% of all injuries. Thirteen percent resulted from torque mechanisms of the leg, whereas direct trauma to elbow or knee (5% each) were uncommon injury mechanisms. Injuries of the elbow, forearm,
