How to help women at risk for acute stress disorder after childbirth.

For some women, childbirth is a traumatic experience that results in significant mental and emotional distress. Whether owing to birth complications, postpartum events such as hemorrhage or pre-existing risk factors such as past history of sexual abuse or rape, the emotional effects of childbirth trauma can lead to acute stress disorder (ASD). To provide the best care for women after childbirth, it's imperative that nurses be able to identify signs of ASD and intervene appropriately. There are many things nurses can do to help women in what could be the most vulnerable time of their lives.

Traumatic childhood exposures in the lives of male perpetrators of female intimate partner violence.

Despite efforts to use behavior modification interventions for male perpetrators, intimate partner violence (IPV) remains a significant problem in some male-female relationships. Childhood exposure to traumatic violent experiences, especially when untreated, can influence adult behaviors. Little is known about these possible factors in the lives of male perpetrators of IPV and if they influence their violent behavior against female intimate partners. This study's aim was to explore the life perspective of men who have been violent with their female intimate partners using Gadamer's hermeneutic

Effects of rumination on intrusive memories: does processing mode matter?

BACKGROUND AND OBJECTIVES: Excessive rumination following traumatic or highly distressing experiences has been proposed to be an important maintaining factor of posttraumatic stress symptoms. However, not all forms of repetitive thinking about a negative event appear to be dysfunctional. It has been suggested that the abstractness of thinking is critical for its symptom-maintaining effects. The present study tested this hypothesis using an experimental analogue design with participants who had experienced a recent negative life event. METHODS: After a short symptom provocation task, participa

Haematological, neurological and electrocardiographic findings in secondary hypothermia.

The association between secondary hypothermia and pancytopaenia is uncommon. A young woman presented with chronic hypothermia (28.8-34.6°C) secondary to surgical hypothalamic injury postcraniopharyingioma resection as a child. Associated findings included pancytopaenia (haemoglobin 8.1 g/dl, leucocytes 3500/mm(3), platelets 63,000/mm(3)), ataxia, upper motor neuron signs, decreased level of consciousness and new ECG changes. An extensive evaluation failed to reveal any cause of pancytopaenia other than chronic hypothermia. The haematological and neurological changes improved after active rewar

Psychiatry: life events and social support in late life depression.

OBJECTIVES: To examine the association of life events and social support in the broadly defined category of depression in late life. INTRODUCTION: Negative life events and lack of social support are associated with depression in the elderly. Currently, there are limited studies examining the association between life events, social support and late-life depression in Brazil. METHODS: We estimated the frequency of late-life depression within a household community sample of 367 subjects aged 60 years or greater with associated factors. ''Old age symptomatic depression'' was defined using the Co

Therapy discharge becomes part of the life story.

Despite great strides in the life-participation approach to aphasia therapy and the arduous work of several therapists to include patients in the decision-making and goal-setting process of therapy, the patient's direct perspective on the experience of treatment is seldom portrayed in academic journals. In the current article, the voices and different agendas of a stroke survivor with aphasia, her speech-language therapists, and an aspiring qualitative researcher are discussed by means of intertwined narratives, dialogues, and discourses. During an in-depth interview process, the researcher he

Sex distribution and psychiatric features of child and adolescent conversion disorder across 2 decades.

BACKGROUND: Earlier research suggested female predominance in adult conversion disorder, and the strong association between conversion disorder and experiences of being abused is well known. However, the data for child and adolescent populations are limited. In Taiwan, the dramatic increase in child abuse may have some impact on the features of child and adolescent conversion disorder. This study aimed to compare the demographic characteristics, psychiatric comorbidity, and experiences of being abused in Taiwanese children and adolescents diagnosed with conversion disorder in psychiatric consu

Suicidal ideation and suicide attempts in bipolar disorder type I: an update for the clinician.

OBJECTIVE: This article reviews the evidence for the major risk factors associated with suicidal behavior in bipolar disorder. METHOD: Review of the literature studies on bipolar disorder, suicidal behavior and suicidal ideation. RESULTS: Bipolar disorder is strongly associated with suicide ideation and suicide attempts. In clinical samples between 14-59% of the patients have suicide ideation and 25-56% present at least one suicide attempt during lifetime. Approximately 15% to 19% of patients with bipolar disorder die from suicide. The causes of suicidal behavior are multiple and complex. So

Social isolation and loneliness among older people: issues and future challenges in community and residential settings.

Although often associated with older age, loneliness and social isolation are not well understood in terms of their prevalence, risk and protective factors. Evidence suggests that only a minority of community-dwelling older people are "severely" lonely or isolated, however a number of factors need to be considered to fully understand the extent and significance of the problem. Community-based studies have identified a variety of risk factors for loneliness/isolation including widowhood, no (surviving) children, living alone, deteriorating health, and life events (eg, loss and bereavement). Higher risk of common mental disorders after experiencing physical violence in Rio de Janeiro, Brazil: the Pró-Saúde Study.

BACKGROUND: In Brazil, violence is a major public health problem. However, up until now, the extent to which violence acts as a risk factor for mental disorders has not been investigated prospectively. AIMS: We determined the risk of common mental disorders (CMD) associated with personal experience with physical violence (PV). METHODS: A cohort of 3253 public employees in Rio de Janeiro completed questionnaires that measured CMD (GHQ-12), experience with PV and stressful life events (SLE). RESULTS: After adjusting for age, income and SLE, those who experienced PV in either 1999 or 2001 and

Adverse life events and coping skills in panic disorder.

UNLABELLED: Research literature and clinical experience shows that panic patients are often able to identify stressors that preceded the onset of their first attacks. In this study we investigated the relation between life events, coping skills, and panic disorder. METHODS: Forty-tree panic patients were compared with 29 control subjects regarding the occurrence and the impact of stressful life events in a 1-year period preceding the onset of panic attacks using the Social Readjustment Rating Scale and London Life Event and Difficulty Schedule. Coping skills were measured using the Ways of Co

Management on tsunami causing posttraumatic stress disorder: a case report.

On December 26, 2004, tsunamis hit Southeast Asia and caused serious damage and loss of lives. In Thailand, six provinces (Ranong, Phang-Nga, Phuket, Krabi, Trang, and Satun) were impacted. The present study reports the psychiatric assessments such as Thai GHQ-60 and IES. It also reports management techniques of both cognitive behavior therapy and medication. Those were provided to a Thai female patient who was 54 years old. The patient responded to treatment quickly because of early management. The tsunami victim with Posttraumatic Stress Disorder (PTSD) is not an individual. A mass of people

Parents' experiences of living through their child's suffering from and surviving severe meningococcal disease.

The aim of this study was to describe the lived experience of parents whose child had suffered from and survived severe meningococcal disease. A Heideggerian phenomenological approach was used within the study and data were collected from long focused interviews. In total, eight parents took part in the study; one was a woman recruited as a pilot interview and the other seven were recruited for the main study. The data were analysed using Colaizzi's (1978) method. Eleven themes emerged: (1) complications/side effects; (2) emotional turmoil; (3) child's physical appearance; (4) family disruptio

Psychosocial factors are linked to functional gastrointestinal disorders: a population based nested case-control study.

BACKGROUND: It is widely accepted based on volunteer studies that levels of psychological distress are similar in those with functional gastrointestinal (GI) disorders and health in the community, while increased psychological distress in outpatients is largely explained by health-care seeking. AIMS: To determine if psychological distress, life event stress, or social support is associated with functional GI disorders in a population-based study. METHODS: A nested case-control study was performed by first mailing a self-report bowel disease questionnaire to a random sample of Olmsted County,

Changes in self-perception during treatment of social phobia.

Ninety individuals with social phobia were randomly assigned to a waitlist control group, a cognitive-behavioral therapy group, or an exposure therapy group without explicit cognitive intervention. Two independent raters classified more than 2,000 thoughts that were reported by participants while anticipating socially stressful situations at pretest and posttest. Each thought was classified on the basis of its valence (positive, negative, or neutral) and attentional focus (self or other). The 2 treatments demonstrated a greater reduction in the frequency of negative self-focused thoughts than

Experience effects on brain development: possible contributions to psychopathology.

Researchers and clinicians are increasingly recognizing that psychological and psychiatric disorders are often developmentally progressive, and that diagnosis often represents a point along that progression that is defined largely by our abilities to detect symptoms. As a result, strategies that guide our searches for the root causes and etiologies of these disorders are beginning to change. This review describes interactions between genetics and experience that influence the development of psychopathologies. Following a discussion of normal brain development that highlights how specific cellu
