Incidental finding of upper lip Warthin tumor.

AIM: This report shows an incidental finding of Warthin tumor in upper lip mucosa during hospitalization for a biting lesion of cheek mucosa MATERIALS AND METHODS: A 32-year-old male affected by a biting lesion of cheek mucosa was presented at Maxillo- Facial Unit of Federico II University. Clinical examination showed as an incidental finding a solid mass in the superficial layer of upper lip mucosa. We performed mini-invasive surgical treatment to obtain a radical excision of the cheek lesion at the same time as excision of Warthin tumor. RESULTS: a follow up of 12 months was performed. The …

Cleft Lip Repair: The Hybrid Subunit Method.

The unilateral cleft lip repair is one of the most rewarding and challenging of plastic surgery procedures. Surgeons have introduced a variety of straight line, geometric, and rotation-advancement designs, while in practice the majority of North American surgeons have been using hybrids of the rotation-advancement techniques. The anatomic subunit approach was introduced in 2005 by Fisher and has gained popularity, with early adopters of the design touting its simplicity and effectiveness. The objectives of this article are to summarize the basic tenets of respecting the philtral subunit, accur…

Fat grafting in primary cleft lip repair.

UNLABELLED: The goal of primary cleft lip repair is to unify the lip elements and achieve a nearly normal appearance. Many techniques can confer satisfactory results; however, scarring and contour irregularities may persist. Lipofilling can modulate scar formation and enable soft-tissue augmentation. The authors hypothesize that fat grafting during immediate cleft lip repair may be of benefit. Patients who underwent primary cleft lip repair with and without immediate fat grafting were compared. Postoperative photographs were analyzed by three blinded reviewers. Cronbach statistics and a two-ta…

Maintenance of certification: unilateral cleft lip repair.

Cleft lip repair is arguably one of the defining procedures in the field of plastic surgery. The results may be dramatic both visually and emotionally. The variety of techniques of cleft lip repair is well described in the literature, in part because of its inherent difficulty to master. The following article reviews the history of cleft lip repair and attempts to make clear the techniques of the rotation-advancement-type repair.

Local flap reconstruction of acquired lip defects.

PURPOSE OF REVIEW: The goal of this article is to review contemporary local flap reconstructive techniques for acquired lip defects. Lip reconstruction is a complex undertaking that necessitates an understanding of underlying principles. New techniques focus on local flap reconstruction which maintain oral competence, avoid microstomia, and achieve aesthetically satisfactory results. RECENT FINDINGS: The contemporary literature employs an aesthetic subunit approach to achieve reconstruction of vermillion, perioral cutaneous, and full-thickness defects. Several new techniques are variations of…

Mandibular lip bumper for molar torque control.

Treatment effects of lip bumpers alone include flaring of the mandibular incisors, distalization and uprighting of the mandibular first molars, and buccal expansion of the canines, premolars, and molar. Lip forces are transmitted through this appliance onto the molars. Moreover the lip bumper is able to derotate, expand or constrict, upright and reinforce the anchorage whereas torque control is lacking. Aim of this paper is the presentation of a new type of lip bumper that allows the molar torque control.

Postoperative wound management after cleft lip surgery.

OBJECTIVE: Our aim was to describe the postoperative management and wound care protocol after primary cleft lip closure, as it has been used in the Bruges Cleft and Craniofacial Center at the supraregional teaching hospital AZ St. Jan, Bruges, between June 1, 1991, and July 1, 2009. MATERIALS: The postoperative management and wound care included the use of a Logan bow, long-acting local anesthetic, elbow restraints, antibiotic therapy, crust removal with normal saline solution, and a special local wound ointment that was prepared at our center. RESULTS: During the last 19 years, 199 unilater…

Minimal scar repair of unilateral cleft lip.

BACKGROUND: Most conventional repairs for unilateral cleft lip are based on the notion that the two joined edges of skin should be the same length. To this end, they incorporate designs that include darts, cutbacks, rotation flaps, and other strategies to match the length of the two segments. As unequal skin edges are routinely joined surgically, it should be unnecessary to incorporate maneuvers in unilateral cleft lip repair to match the segment lengths. A repair that places the scar along the philtral ridge was developed. This eliminates scars that deviate from the aesthetic subunits. METHO…

Surgical modifications for microform cleft lip repairs.

Microform cleft lips are very rare forms of cleft lips, and the timing and methods of surgical correction remain controversial. We reviewed our surgical methods used between 1991 and 2003. There were eight such cases, with patients ranging in age from 3 months to 9 years, who underwent primary repairs, and their outcomes were evaluated. Two different surgical procedures were used, either a conventional linear incision, lower small triangular flap insertion, and orbicularis oris muscle reconstruction, or the two small z-flap approach in which the central philtral muscles are repositioned over t…

Lip cancer in South Australia, 1977-1996.

Lip cancer (140 ICD-9, C00 ICD-10) is a form of oral cancer occurring at the junction between the oral cavity and the skin. Lip cancer has a distinctive global epidemiology that is notably different from cancer occurring at other intraoral sites. This study reviews and analyses the epidemiological data for lip cancer from the South Australian Central Cancer Registry between 1977 and 1996. During this 20-year period, 2716 cases of lip cancer (2095 male, 621 female) and 35 deaths from this disease (23 males, 12 females) were reported. The average age of diagnosis was 58.3 years in males and 66.0…

[Lip-jaw-palate clefts. Heterogenous disease pictures].

BACKGROUND: Orofacial clefts are congenital structural deficits involving the lip with or without the palate. The genetic origins of orofacial clefts have been considered as heterogeneous for a long time. Non-syndromatic clefts are based on complex traits. DISCUSSION: The different approaches towards the genetics of this multifactorial disease are discussed and the consequences for genetic counselling for patients, persons at risk and their relatives are reflected upon.

Upper lip asymmetry in adults during smiling.

PURPOSE: This study examined the incidence of upper lip asymmetry in adults while smiling. PATIENTS AND METHODS: Two-hundred ten students between the ages of 21 and 44 years were examined. Subjects determined to have an occlusal plane cant were eliminated from the study. Repeated smiles were then evaluated in the remaining subjects to check for upper lip asymmetry. RESULTS: Fifteen subjects were observed to have occlusal plane cants and were eliminated from the study. Of the remaining 195 subjects, 17 (8.7%) were found to have an asymmetrical smile secondary to canting of the upper lip. CON…

Maxillary double lip: report of two cases.

Two cases of double lip malformation, an uncommon oral anomaly, are presented, and the factors involved in the development, pathogenesis, diagnosis, and treatment of these uncommon lesions are reviewed. Double lip is usually associated with Ascher's syndrome, which is a rare disease with three more or less consistently associated abnormalities: double upper lip, blepharochalasis and enlargement of the thyroid. Two cases of double lip are reported; one of which was related with Ascher's syndrome, while the other had a traumatic origin.

Median cleft lip with associated midline sinuses.

Midline upper lip sinuses alone are rare entitities, with less than 25 cases reported previously. Midline upper lip sinuses with associated cleft lip are even more rare. A patient report of median cleft lip with associated upper lip sinuses in a 16-year-old boy is presented. Controversy exists as to whether these abnormalities are related. This report demonstrates the plausibility of a cause-and-effect relationship between midline clefts and sinuses.

Bilateral vermilion flaps for lower lip repair.

A more natural reconstructive procedure of the lower lip using bilateral vermilion flaps was applied in five patients with excellent results. The vermilion defects were about two-fifths to three-fifths. In three patients, the vermilion defect was repaired using bilateral vermilion flaps alone. In the remaining two patients, a narrow horizontal lip defect was repaired by bilateral vermilion flaps and a subcutaneous V-Y advancement flap of the lower lip. A single vermilion flap or bilateral vermilion flaps are considered to be of great value for vermilion reconstruction because of the inherent e…
