A 31-year-old male patient with a biopsy-proven cutaneous malignant melanoma located in the interscapular area was referred to lymphoscintigraphy for preoperative sentinel lymph node mapping. Anterior and posterior planar images showed 3 hot spots suggesting left axillary sentinel nodes. Herein, we reported the contribution of preoperative SPECT/CT-guided sentinel lymph node excision on accurate staging, management, prognostic evaluation, and determination of the proper surgical positioning preoperatively.

The authors report a case of recurrent breast carcinoma in the right chest in a patient who earlier had a right breast mastectomy. Injection of Tc-99m sulfur colloid into the lesion site in the right midchest revealed drainage to a very faint node in the contralateral axilla on the left, an unexpected site, and none to the ipsilateral axilla, the expected site. Disease was found in the left axilla in the sentinel nodes. Lymphoscintigraphy added valuable information in the management of this patient.

Preoperative lymphoscintigraphy with Tc-99m antimony sulphide colloid was performed for a patient with a cutaneous melanoma on the left back. There was direct lymphatic drainage to retroperitoneal and paravertebral nodes but no drainage whatsoever to the axillary, inguinal, or triangular intermuscular space node fields, which would normally be expected to drain this area of the skin. Any attempt at sentinel lymph node biopsy in the axilla, groin, or triangular intermuscular space would have failed in this patient.

Gynecologic malignancies account for 15% of all cancer diagnosis in women. Primary lymphatic spread is well recognized in vulvar, cervical, uterine, and ovarian carcinomas. Vulvar carcinoma spreads locally to the inguinofemoral lymph nodes in a relatively predictable fashion similar to the local spread of breast carcinoma. Lymphatic mapping using radioactive colloid should provide adequate means to sample these nodal basins while attempting to reduce postoperative morbidity. Methods of vulvar lymphoscintigraphy are described.

Breast cancer usually metastasizes by haematogenous spread. This is a case report of a woman with unusual liver metastases from a recurrent invasive ductal carcinoma via a lymphatic route draining the outer part of the breast to the liver running parallelly with the right rectus abdominis muscle, depicted by preoperative sentinel node lymphoscintigraphy. Realizing this route of metastasis can impact survival, as it has a favourable prognosis compared with haematogenous metastasis, we want to draw attention to this.

We report the case of a 58-year-old man with erysipelas of both buttocks secondary to lymphoedema after pelvis surgery for prostatic cancer and multiple vascular interventions for iliofemoral thrombosis. The diagnosis was based on clinical examination and lymphoscintigraphy. This location of lymphoedema, limited to the buttocks, is uncommon; lower limbs are usually affected. This location needs a specific curative and preventive treatment, including antibiotic therapy, lymphatic drainage and contention to abate the condition and prevent a relapse.

We present the case of a 26-year-old man with congenital primary lymphedema. A lymphedema of the right upper limb appeared at the age of 2 years, followed by edemas of the right lower limb and of the right part of the head and neck. A lymphedema of the left lower limb began 5 years ago, and finally an edema of the external genitalia began 2 years ago. Lymphoscintigraphy of the lower limbs and pelvic/abdominal region (including SPECT/ldCT) revealed interesting abnormalities.

Spiradenocarcinoma is an exceedingly rare malignant neoplasm with ductal differentiation. Many consider it to be an eccrine neoplasm, and others favor apocrine differentiation. In nearly all cases, spiradenocarcinoma is associated with a benign spiradenoma, with most lesions appearing on the trunk and extremities. We describe a patient who presented with a long-standing and previously asymptomatic scalp nodule that became tender and enlarged. After diagnosis of spiradenocarcinoma, the patient was referred for lymphoscintigraphy, sentinal lymph node biopsy, and Mohs micrographic surgery.
