Succinylcholine pretreatment with magnesium sulfate.

The effect of pretreatment with 60 mg/kg magnesium sulfate on the neuromuscular blockade and consequent potassium release produced by 1.5 mg/kg succinylcholine in ten normal patients was compared with ten saline pretreated control patients. Magnesium had no significant effect on the characteristics of the paralysis. In control patients, serum potassium increased by an average of 0.57 +/- 0.20 (SEM) mmol/L. No patient in the magnesium group had an increase in serum potassium (mean change -0.05 +/- 0.02 mmol/L). The difference between the groups was statistically significant (P less than 0.01).

Magnesium, potassium and hormonal regulation.

Clinical and experimental data demonstrate that hormonal factors are involved in magnesium regulation. It is possible that the parathyroid hormone could play an important role in the maintenance of normal calcium and magnesium concentrations. The action of other hormones in magnesium metabolism appears to be an indirect response to factors such as calcium concentration or changes in volume. Alterations of the magnesium concentrations are important for the intracellular potassium, sodium and calcium content. Magnesium deficiency appears to lower intracellular potassium and to increase intracell…

Magnesium deficiency in cystic fibrosis.

We have described the first case of symptomatic magnesium deficiency in a patient with cystic fibrosis and have discussed its causes. Such deficiency probably occurs with significant frequency in cystic fibrosis and should be considered in such patients so that important morbidity can be prevented.

[Clinical importance of magnesium metabolism].

Magnesium is an important cation for the organism. In the cell, magnesium is a cofactor of numerous enzymatic processes, while in the extracellular compartment it is involved in neuromuscular excitation. Various clinical conditions lead to magnesium deficiency, the symptoms of which are highly protean. However, hypermagnesemia results almost exclusively from increased magnesium intake in patients with reduced glomerular filtration. The consequences of such magnesium intoxication are essentially neuromuscular.

Downbeat nystagmus with magnesium depletion.

Two patients with neurological signs of magnesium depletion complained of oscillopsia and manifested downbeat nystagmus. Serum magnesium levels measured less than 1.0 mg/dL. Hypomagnesemia resulted from debilitating intestinal disease in both patients and apparently was aggravated in one case by failure to add magnesium sulfate to the patient's regimen for total parenteral nutrition. Neither patient had any of the neurological conditions reported with downbeat nystagmus. Thus, downbeat nystagmus in the primary position occurs with the metabolic derangement of severe magnesium depletion. Anothe…

Nutritional aspects of magnesium metabolism.

The absolute necessity for magnesium in plant and animal nutrition is easily appreciated when one realizes that magnesium is found in high concentration in cells. It is necessary for photosynthesis in plants and for all reactions involving adenosine triphosphate in plant and animal cells. Although it is abundant in nature in general, deficiencies occur in both plants and animals. Human beings need about 5 mg of magnesium per kg of body weight per day. Infants and young children need twice as much. Children and women during pregnancy or lactation require significantly greater amounts than norma…

Magnesium therapy in neonatal tetany.

104 infants with symptomatic hypocalcaemia were randomly allocated to treatment with calcium gluconate, phenobarbitone, or magnesium sulphate. Infants treated with magnesium sulphate had higher plasma-calcium concentrations after 48 hours' treatment and fewer convulsions during and after the treatment period. Magnesium sulphate is recommended as the treatment of choice in symptomatic neonatal tetany whether or not there is hypomagnesaemia.

Magnesium metabolism: a brief review.

The important role played by the magnesium ion in the body is not generally recognized. The action of numerous enzyme systems critical to cellular metabolism is regulated by it and it contributes importantly to macromolecular structure. Magnesium defiency occurs more often than is generally suspected; magnesium excess, though uncommon, is of special interest to the anaesthetist because it produces a curare-like effect on neuromuscular transmission. It is hoped that this brief review of magnesium metabolism will draw attention to its importance and relevance in everyday practice.

Magnesium deficiency and cardiac disorders.

Magnesium deficiency can occur in congestive heart failure, after diuresis with furoxemide, ethacrynic acid and mercurials, and with digitalis intoxication, diabetic acidosis, acute and chronic alcoholism, delerium tremens, cirrhosis, malabsorption syndromes, protracted postoperative cases, open heart surgery, the diuretic phase of acute tubular necrosis, and with hypoparathyroidism, primary aldosteronism, juxta-glomerular hyperplasia and pancreatitis. Two cases of serious ventricular arrhythmias associated with magnesium depletion are described. Clinical manifestations are vague but center ar…
