BACKGROUND: Fibula and iliac crest flaps are widely used for mandibular reconstruction, but accurate restoration of the mandibular angle using iliac flaps remains challenging and may affect postoperative aesthetics.
PURPOSE: The purpose of this study was to compare mandibular angle contour, donor-site function, and quality-of-life outcomes between subjects who underwent digitally assisted mandibular angle reconstruction using pedicled iliac crest flaps alone and those treated with vascularized iliac crest flaps combined with nonvascularized iliac bone grafts.

BACKGROUND: Mandibular bone defects present significant challenges in reconstructive surgery due to their complex structure, and the body's limited ability to heal these areas naturally. Tissue engineering provides a promising solution, combined with stem cell therapy to overcome these obstacles. This study investigated the effects of adipose-derived mesenchymal stem cells (ADSCs) cultured on novel three-dimensional (3D)-printed polycaprolactone/xenograft scaffolds on the reconstruction and regeneration of segmental mandibular bone defects in canine models.
METHODS: 3D-printed personalized PC

The high success rate of current dental implant protocols makes studies of catastrophic failures a rarity; however, this article presents a case study in which faulty medical intervention was the direct cause of traumatic implant failure. The patient, who had been treated with implants that successfully osseointegrated, ultimately experienced a life-threatening medical incident due to an ill-fitting definitive prosthesis. This case report includes a detailed narrative of how this patient, who began his dental journey with compromised and missing teeth, became edentulous and then, following an

Magnesium/Ceria nanocomposites (Mg/xCeO2 NCs (x = 0.5 %, 1 % and 1.5 %)) prepared by using powder metallurgy and microwave sintering method are assessed for their corrosion rate for a period of 28 days. As per the immersion tests results, the addition of ceria nanoparticles to pure Mg, brought about a noteworthy improvement to corrosion resistance. A corrosion rate of approximately 0.84 mm/year for Mg/0.5CeO2 and 0.99 mm/year for Mg/1.0CeO2 nanocomposites were observed. Another aspect of the study involves employing the simulation method i.e. finite element analysis (FEA) to compare the stress

BACKGROUND: The reconstruction of large mandibular defects is a challenge, and free vascularized bone flaps are most commonly used. However, the precision and symmetry of this repair are deficient, and patients have a risk of vascular embolism, flap necrosis, and donor site complications. Therefore, to explore an ideal alternative in mandibular reconstruction with high surgical accuracy and low complications is indispensable.
METHODS: Seven patients with recurrent or large-scope ameloblastoma were enrolled in this study. All patients were provided with a fully digital treatment plan, includin…

Simultaneous or secondary inserted implants with double-barrel fibula to reconstruct the mandible have become a common method. However, difficulties in later restoration caused by placement errors of fibula or incipiently placed implants have also been reported in some studies. This note describes a novel technique of implant-oriented guide plates helpful for mandible ablation, fibula segmentation and positioning, and implant placement. We design a series of guide plates especially an implant-fibula placing guide plate, and record and fix the relative spatial positions of the remaining teeth,

In this study, the authors introduced a dual application of patient-specific occlusion-based positioning guide and fibular cutting guide to obtain ideal occlusal relationship and mandibular contour in patients undergoing mandibular reconstruction. A retrospective review was performed in 21 patients who underwent mandibular reconstruction with a fibular osteocutaneous free flap. Using computed tomography and intraoral scanning data, fibular cutting guide and occlusion-based positioning guide were simulated in a modeling software and 3-dimensionally printed. Both guides were applied in 9 patient…

The fibula free flap represents the gold standard for mandibular reconstruction. However, when harvested as a single barrel, this flap does not allow the native mandibular height to be restored, which is required for implant-supported dental rehabilitation of the patient. The aim of this study was to present a new design for a patient-specific three-dimensionally printed reconstructive plate (3DBO-PSI) that positions the fibula bone at the height of the resected mandibular alveolar bone while restoring the mandibular profile to ensure a correct morphological outcome. Twenty patients were enrol…

Langerhans cell histiocytosis (LCH) is a rare disease in which Langerhans cells, which are bone marrow-derived antigen-presenting cells, proliferate in single or multiple organs. We successfully treated a patient with unifocal LCH of the mandible with malocclusion due to a severe pathological fracture, using reconstruction with a vascularised free bone.

This study describes an unusual case of mandibular asymmetry after fibula free flap reconstruction in a young man following major facial trauma that was corrected using a custom-made polyetheretherketone prosthesis. There is little information in the literature on the use of alloplasts to correct mandibular asymmetry as interest in 'aesthetic re-modelling' has traditionally focused on nasal, zygomatic and chin regions. This report demonstrates that this technique can be used successfully to address selected cases of mandibular asymmetry.
