Spontaneous internal carotid artery dissection: an uncommon cause of recurrent postpartum headache.

Postpartum spontaneous cervicocephalic artery dissection is an uncommon, poorly understood condition following pregnancy. We report a case of a 32-year-old woman with no history of trauma, chiropractic manipulation, connective tissue disorder, or previous headache who developed a mild, unilateral headache 7 days after the uneventful delivery of her third child (no general or neuraxial anesthesia was delivered). Seven days latter she presented to our emergency department complaining of recurrent episodes of right-sided headache coupled with a transient episode of sensory deficits in her left lo

Use of a multimodal conservative management protocol for the treatment of a patient with cervical radiculopathy.

OBJECTIVE: The purpose of this study is to describe and discuss the treatment of a cervical disk herniation using a sequential multimodal conservative management approach. CLINICAL FEATURES: A 40-year-old man had complaints of headache and severe sharp neck pain radiating to his left shoulder down to his arm, forearm, and hand. Results of electromyography/nerve conduction studies were abnormal. Magnetic resonance imaging revealed a large disk protrusion at C5-C6 with indentation of the thecal sac and a spur at the posterior margin. Moderate left neural foraminal narrowing was present at C5-C6

Dissection of the posterior inferior cerebellar artery in a young adult with cerebellar infarct.

The posterior inferior cerebellar artery (PICA) is frequently involved in dissection of the vertebral artery (VA); however, isolated PICA dissection has rarely been reported. A 37-year-old man experienced acute and progressive drowsiness, vertigo, occipital headache, vomiting, and ataxia. There was no precedent trauma or chiropractic manipulation. Neurologically, he had dysmetria of the left extremities. His NIHSS score was 3. Brain magnetic resonance imaging showed an acute cerebellar infarct in the left PICA territory. Magnetic resonance angiography showed a faint signal adjacent to the junc

Return to work after two years of total disability: a case report.

BACKGROUND: Little is specifically written regarding the details of clinical interventions with workers with chronic spinal pain. Specifically, the individual thought process behind the management of an injured worker is not often discussed except in clinical conferences or discussions with other clinicians. It is well known that once an injured worker is disabled from work for 2 years, successful return is unlikely. We thought it would be informative to illustrate the clinical approach we took with this case because of the history of long term pain and disability. METHODS: This paper describ

Case Report: Oral Fluphenazine for Treatment of Chronic Sacral Pain.

Management of chronic pain syndromes represent significant challenges to clinicians. Fluphenazine was shown to attenuate mechanical allodynia in rat neuropathic pain models via antagonism of voltage gated sodium channels and has been identified as a candidate drug for chronic pain. We present a 66 year old man with treatment-refractory chronic pain syndrome. Right ischial pain developed over time as a result of his years practicing as a dentist sitting on a stool with a tilted seat pan. Imaging and neurologic investigations led to a presumed diagnosis of chronic microtears of the sacrotuberous

Lumbar Spinal Stenosis: Diagnosis and Management.

Lumbar spinal stenosis is a clinical syndrome that affects more than 200,000 people in the United States annually. It is a common cause of chronic insidious low back pain, especially in older patient populations (mean age = 64 years). Lumbar spinal stenosis is a degenerative condition of the spine leading to narrowing in the spaces around the neurovascular bundles and the classic symptom of low back pain that radiates to the buttocks and lower extremities bilaterally. It is typically a progressive waxing and waning process that may deteriorate over years. The pain is typically burning or cramp

A case of multiple Vertebrobasilar artery fenestration misdiagnosed as vertebral artery dissection.

BACKGROUND: Fenestration of vertebra-basilar artery is an uncommon congenital vascular anomaly, which is identified by incidental findings on imaging in patients presented without related symptoms or in patients with intracerebral hemorrhage secondary to concomitant artery aneurysm or arteriovenous malformations. Yet, cases of fenestration being misdiagnosed as cerebral artery dissection have never been reported. CASE PRESENTATION: We present a patient of 66-year-old female with acute onset of dizziness after chiropractic manipulation of the neck. Neck computed tomography angiography (CTA) sh

Vertebral artery dissection after iatrogenic cervical subcutaneous emphysema.

We report a case of spontaneous vertebral artery dissection (VAD) in a patient who developed extensive subcutaneous emphysema following the removal of a chest tube after a cardiac transplant. The pathophysiology and management of this uncommon complication are reviewed. Although vertebral and carotid artery dissections are unusual events occurring in 2.5 to 3 per 100,000 people, they are increasingly acknowledged to be important causes of stroke in the young and middle-aged adult population accounting for up to 25% of such cases. VADs are associated with a variety of minor traumatic mechanisms

Improvement of lower extremity electrodiagnostic findings following a trial of spinal manipulation and motion-based therapy.

BackgroundLumbar disc herniation is a problem frequently encountered in manual medicine. While manual therapy has shown reasonable success in symptomatic management of these cases, little information is known how manual therapy may affect the structure and function of the lumbar disc itself. In cases where lumbar disc herniation is accompanied by radicular symptoms, electrodiagnostic testing has been used to provide objective clinical information on nerve function. This report examines the treatment rendered for a patient with lower extremity neurological deficit, as diagnosed on electrodiagno

Association Between Spinal Manipulation, Butalbital Prescription, and Medication Overuse Headache in Adults With Tension-Type Headache: Retrospective Cohort Study.

BACKGROUND AND AIMS: Butalbital is an acute headache medication commonly prescribed for tension-type headache (TTH), although discouraged by guidelines due to a risk of medication overuse headache (MOH). Considering spinal manipulative therapy (SMT) may reduce TTH frequency and intensity, we hypothesized adults with TTH receiving chiropractic SMT would be less likely to receive a butalbital prescription over 2 years versus matched controls not receiving SMT. We secondarily compared likelihood of MOH between cohorts. METHODS: We searched a United States medical records database of patients att

The association between chiropractors' view of practice and patient encounter-level characteristics in Ontario, Canada: a cross-sectional study.

BACKGROUND: Chiropractors have diverse views of practice, but the impact on their patient profiles and treatment approaches remains unclear. We assessed the association between chiropractors' view of practice (unorthodox versus orthodox) and patient encounter-level characteristics among chiropractors who practice in Ontario, Canada. METHODS: We conducted a cross-sectional study using Ontario Chiropractic Observation and Analysis STudy (O-COAST) data. In O-COAST, Ontario chiropractors were randomly recruited from a list of registered chiropractors in 2015 and recorded up to 100 consecutive pat

Quantitative ultrasound evaluation of the thoracolumbar fascia after manual and acupuncture therapies: an exploratory mechanistic randomized controlled trial with a sequential within-subject phase.

OBJECTIVE: Thoracolumbar fascia (TLF) alterations may contribute to nonspecific low back pain (NSLBP). We evaluated the effects of acupuncture, chiropractic care, and massage on TLF biomechanics, microstructure, and clinical outcomes in NSLBP. MATERIALS AND METHODS: In this exploratory mechanistic hybrid trial, sixty participants (median age: 47 years [interquartile range 39-63]; 63% women) were randomized (1:1:1) to acupuncture, chiropractic, or a 3-week waitlist control, providing between-group comparisons. After the waitlist, control participants received massage therapy, enabling a sequen

Prognostic ability of the sTarT back screening tool for disability and pain intensity outcomes in older adults with low back pain seeking chiropractic care: a multi-national external validation study.

BACKGROUND: Low back pain (LBP) is common among older adults, and it is a frequent reason for seeking chiropractic care. The STarT Back Screening Tool (SBT) was developed to stratify patients with LBP into low, medium, and high-risk treatment pathways, so that the treatment can be matched to each participant's risk profile. But its prognostic performance varies across settings and populations. No studies have focused on the SBT's utility as a stratified-care tool in older adults with LBP in a chiropractic setting. Therefore, our aim was to evaluate the ability of the SBT to predict three-, six
