OBJECTIVE: Temporal bone encephalocele has become less common as the incidence of chronic mastoid infection and surgery for this condition has decreased. As a result, the diagnosis is often delayed, and the encephalocele is often an incidental finding. This situation can result in serious neurologic complications with patients presenting with cerebrospinal fluid leak and meningitis. We review the occurrence of, characteristics of, and repair experience with temporal encephaloceles from 2000-2012.
METHODS: We conducted a retrospective review of 32 patients undergoing combined mastoidectomy and…

BACKGROUND: The main risk factors for postoperative failure in tympanoplasties are large perforations that are difficult to repair, annular perforations, and a tympanic membrane (TM) with extensive granular myringitis that require middle ear exploration and mastoidectomy. The aim of this study was to investigate a novel technique of perichondrium/cartilage composite graft for repairing the large TM perforation in the patient of otitis media.
METHODS: Retrospective chart reviews were conducted for 102 patients with large tympanic membrane perforations, who had undergone tympanoplasty from Augu…

OBJECTIVE: The extended retrosigmoid approach is presented as a simple and safe modification of the traditional retrosigmoid approach, with increased exposure resulting from a limited mastoidectomy and skeletonization of the sigmoid sinus.
METHODS: Patients with posterior fossa vascular lesions treated with the extended retrosigmoid approach between 1997 and 2003 were reviewed. A detailed description of the surgical approach, as well as case illustrations, is provided. We present a video narrated by the senior author in which a description of the technique is offered.

OBJECTIVES/HYPOTHESIS: Multiple techniques of ossicular reconstruction have been advocated for hearing rehabilitation in the setting of chronic otitis media No single method can adequately address the clinical spectrum of disease severity. In the situation of a severely diseased ear requiring a canal wall down (CWD) mastoidectomy in the presence of an intact stapes superstructure, the authors have employed a double cartilage block (DCB) ossiculoplasty. The technique and short-term results are reviewed.

INTRODUCTION: Glomus jugulare tumors are rare, highly vascular paragangliomas arising within the jugular foramen. They are typically slow-growing but can cause significant morbidity due to involvement of surrounding neurovascular structures. Optimal management requires a tailored approach based on the extent of the tumor, symptoms, and patient-specific factors.
CASE PRESENTATION: A 60-year-old woman presented with a seven-year history of progressive left-sided hearing loss, pulsatile tinnitus, and a sensation of fullness. She had undergone a previous cortical mastoidectomy with no symptomatic…

Meniere's disease is a common inner ear disorder accompanied by vertigo attacks and fluctuating hearing loss that some believe is due to a stressful lifestyle. To elucidate the scientific relationship in neuro-endocrinology between Meniere's disease and stress, we examined the surgical results of endolymphatic sac drainage surgery and changes in stress-induced plasma arginine-vasopressin levels. We enrolled 100 intractable Meniere's patients and examined surgical results and plasma vasopressin levels. Fifty-four chronic otitis media patients who underwent tympano-mastoidectomy formed a control…

Through the year 1993 to 1996 a total of 132 cases of chronic otitis media were taken up for tympanoplasty with or without mastoidectomy, out of which 30 cases were found to have tympanosclerosis; 11 cases (37%) out of this were found to be significant and were subjected to tympanosclerotic plaque extraction. Clinically significant tympanosclorsis has been defined as that affecting the surgical procedure by requiring removal to effect a hearing improvement. Success in hearing restoration so achieved in such ears is approximately as good as in non-tympanosclerotic cars.

Chondrosarcoma of the temporal bone is a rare disease, with only 36 cases of this tumor having been reported in the English language literature. We report the case of a 52-year-old woman with myxoid chondrosarcoma of the temporal bone whose only symptom was progressive hearing loss. The tumor was removed using aspiration, mastoidectomy and tympanotomy, with retention of the jugular bulb. No recurrence has been noted 45 months after the operation. Diagnosis was established from histological examination of the surgical specimens. The clinical characteristics and management of this rare lesion ar…

Leukemic involvement of the temporal bone is not uncommon and may present in a variety of ways including auricular or external canal skin lesions, red or thick tympanic membrane, middle ear effusions, otitis media, hearing loss or mastoiditis. Symptomatic facial nerve involvement, on the other hand, is extremely unusual. We discuss a pediatric patient whose sudden onset facial nerve paresis was the presenting symptom that led to her diagnosis of leukemia. At the time of mastoidectomy, a granulocytic sarcoma or chloroma was noted to be overlying the VIIth nerve.

The author describes the histopathology of fenestration operation in five temporal bones of the Massachusetts Eye and Ear Infirmary collection. The four subjects underwent the intervention more than 18 years before their death. One of the patients was operated bilaterally by Lempert himself in 1950. Special attention is given to the effect on hearing results of the otosclerotic disease; the condition of the middle ear, mastoidectomy cavity, and fenestra; and the lateral and posterior membranous semicircular canals. Also the patency of the three fenestrae (the oval, the round, and the new fenes…

The diagnosis of carcinoma primary in the mastoid bone is usually made while performing a mastoidectomy in an effort to control presumed chronic mastoiditis. The association of chronic infection, serosanguineous otorrhea, and severe otalgia, common warning signs of carcinoma of the middle ear or external auditory canal, may or may not be present with a carcinoma primary in the mastoid bone. If the amount of bony mastoid destruction seen by roentgenography is out of proportion to the degree of clinical infection, malignancy should be strongly suspected. The authors present a case of primary car…

We describe the case of a 12-year-old girl with acute otitis media complicated by acute mastoiditis, epidural empyema, thrombosis of the sigmoid sinus and paralysis of the abducens nerve. The patient underwent a mastoidectomy on the left side combined with drainage of the epidural empyema through an extended burr holl and received intravenous antibiotics for 6 weeks and anticoagulation for 12 weeks. This report discusses the intracranial complications of acute otitis media, which were a common problem before the advent of adequate antibiotic drugs but have become rare since their introduction.

Gradenigo syndrome is a rare condition consisting of otitis media, trigeminal neuralgia and abducens palsy. We report here a 6-year-old girl with this syndrome. Cranial magnetic resonance imaging (MRI) demonstrated inflammatory lesions in the left petrous apex and cavernous sinus, as well as stenosis of the left carotid artery. She was conservatively treated with antibiotics, which successfully improved her clinical and MRI findings. Surgical treatment including mastoidectomy was avoided. This case illustrates the usefulness of MRI in the diagnosis and management of Gradenigo syndrome.
