Maternal Mortality from Induced Abortion in Malawi: What Does the Latest Evidence Suggest?

It is commonly claimed that thousands of women die every year from unsafe abortion in Malawi. This commentary critically assesses those claims, demonstrating that these estimates are not supported by the evidence. On the contrary, the latest evidence-itself from 15 to 20 years ago-suggests that 6-7% of maternal deaths in Malawi are attributable to induced and spontaneous abortion combined, totalling approximately 70-150 deaths per year. I then offer some evidence suggesting that a substantial proportion of these are attributable to spontaneous abortion. To reduce maternal mortality by large ma…

Maternal mortality at the Central Hospital, Benin City Nigeria: a ten year review.

Maternal mortality remains a major challenge in Nigeria. This retrospective study was conceptualized to document the number and pattern of obstetric deaths at the Central Hospital, Benin City, over a ten year period, to identify common causes of maternal deaths and proffer relevant interventions. The overall maternal mortality ratio (MMR) was 518/100,000. MMR was 30 times higher in unbooked as compared to the booked patients, while 60% of maternal deaths occurred within 24 hours of admission. The leading direct causes of maternal deaths were sepsis, hemorrhage, obstructed labor and preeclampsi…

Maternal mortality in seven districts of Uttar Pradesh--an ICMR task force study.

Maternal mortality is a major health and development concern. The available information on maternal mortality in rural India is inadequate and scanty. This study presented maternal mortality data from the demographically and developmentally (including for health) poor performing state of Uttar Pradesh. A descriptive, cross-sectional survey was conducted utilizing a stratified cluster sampling design between 1989-90 in eight districts of Uttar Pradesh. Four good performing districts namely, Agra, Farrukhabad, Ghaziabad and Badaun from the western region and four poor performing districts from t…

Trends in maternal mortality: a 13-year hospital-based study in rural Ghana.

OBJECTIVE: To measure the impact of the Safe Motherhood Initiative (SMI) on hospital-based maternal mortality since its start in 1987. STUDY DESIGN: Retrospective analysis of all 229 maternal deaths in a district hospital in rural Ghana, between 1 January 1987 and 1 January 2000. Main outcome measures were maternal mortality ratio and relative contribution of causes of maternal deaths to overall maternal mortality. Chi-square test was used to assess differences in proportions, and relative risks with confidence intervals were calculated. RESULTS: The overall maternal mortality ratio of 1077/…

Perinatal risk for mortality and mental retardation associated with maternal urinary-tract infections.

OBJECTIVE: The researchers analyzed the relationship between fetal exposure to maternal urinary tract infections (UTIs) and mental retardation or developmental delay and fetal death. STUDY DESIGN: A retrospective cohort design was used to explore the risk for fetal death and mental retardation or developmental delay associated with exposure to maternal UTI during pregnancy. POPULATION: Matched maternal-child pairs from the National Collaborative Perinatal Project (NCPP) from the decades of 1960 and 1970 were compared with a previous analysis of the South Carolina Medicaid Reimbursement Syste

Maternal mortality associated with HELLP (hemolysis, elevated liver enzymes, and low platelets) syndrome.

OBJECTIVE: The aim of this study was to determine factors contributing to deaths among women with HELLP (hemolysis, elevated liver enzymes, and low platelets) syndrome. STUDY DESIGN: Information from multiple sources was scrutinized to distinguish and profile maternal deaths associated with HELLP syndrome. RESULTS: Information was available regarding 54 maternal deaths. According to HELLP syndrome classification 60.0% had class 1 disease, 35.6% had class 2 disease, and 4.4% had class 3 disease. Events associated with maternal deaths included cerebral hemorrhage (45%), cardiopulmonary arrest

Fetal and maternal morbidity and mortality in menopausal women aged 45-63 years.

The pregnancies achieved up to April 1994 among 162 recipients who participated in our oocyte donation programme were studied. Of these, 113 were > 45 years old and 49 were < 45 years old (25-44); approximately 300 women were not admitted to our programme for various reasons. The 113 recipients considered in this study were divided into two age groups: group A, 45-49 years old (54 patient cycles); and group B, 50-60 years old (58 patient cycles); there was also one case of a 63 year old woman. Overall, 44 clinical pregnancies were established in 113 recipient cycles (38.9%) and 44 healthy babi

Postpartum Maternal mortality following live birth in Israel 1998-2017: Trends and causes.

OBJECTIVE: To describe temporal trends and causes of postpartum maternal mortality following livebirth in Israel during 1998-2017, including deaths up to one year postpartum. STUDY DESIGN: National retrospective population-based study using de-identified Israeli Central Bureau of Statistics mortality records linked to 3,076,207 livebirths (1998-2017). Maternal opioid use disorder and infant mortality in Wisconsin, United States, 2010-2018.

OBJECTIVE: The difference in infant health outcomes by maternal opioid use disorder (OUD) status is understudied. We measured the association between maternal OUD during pregnancy and infant mortality and investigated whether this association differs by infant neonatal opioid withdrawal syndrome (NOWS) or maternal receipt of medication for OUD (MOUD) during pregnancy. METHODS: We sampled 204,543 Medicaid-paid births from Wisconsin, United States (2010-2018). The primary exposure was any maternal OUD during pregnancy. We also stratified this exposure on NOWS diagnosis (no OUD; OUD without NOWS

METHODS: A case-control study was conducted, including 49 cases of maternal deaths and 98 controls, applying multivariate logistic regression to analyze data collected from hospital records. RESULTS: The absence of a partner (adjusted OR: 12.3; 95% CI: 2.6 to 58.8), history of anemia (adjusted OR: 5.8; 95% C…

METHODS: A nested case-control study embedded within the Ethiopian Obstetric Surveillance System (EthOSS) cohort. EthOSS was established in April 2021 to monitor women admitted with severe obstetric complications (e.g., obstetric hemorrhage, eclampsia, uterine rupture, sepsis, and severe anemia) during pregnancy, childbirth or within 42 days of termination of pregnancy. The Psychiatrist's Role in Treating Perinatal Opioid Use Disorder and Reducing Maternal Mortality.

Drug overdose is a leading cause of maternal mortality. Psychiatrists can play a critical role in reducing these deaths by delivering effective evidence-based treatments for perinatal opioid use disorder (POUD), including the use of buprenorphine. Medications for POUD (i.e., buprenorphine, methadone) are life-saving treatments, but only half of those who are diagnosed as having POUD will receive this treatment, which can result in an increased risk for return to opioid use, overdose, and death. Psychiatrists are well positioned to prescribe buprenorphine given the Drug Enforcement Administrati…
