STATEMENT OF PROBLEM: It has been suggested that risk for severe resorption in the anterior maxilla is increased in persons wearing mandibular implant-retained overdentures. However, little information is available about the changes in the edentulous maxilla after mandibular implant treatment.
PURPOSE: This study determined the possible changes in the width of the maxillary residual ridge 6 years after receiving mandibular implant-supported or implant-mucosa-supported overdentures and evaluated the association between the anatomic changes and subjective complaints with maxillary complete dent…

INTRODUCTION: The reconstruction of combined vertical and horizontal bone defects in the posterior maxilla is a common challenge in implant dentistry. The maxillary tuberosity presents a source of autogenous bone that can be harvested with minimal morbidity. This study evaluates the clinical outcomes of using maxillary tuberosity grafts for such reconstructions.
METHODS: A retrospective analysis was conducted on 6 consecutive patients with posterior maxillary defects, treated between 2021 and 2023. All procedures were performed under local anesthesia. Bone grafts were harvested from the maxil…

Congenital fusion of the maxilla and mandible is an extremely rare craniofacial anomaly that can present at birth with trismus and feeding difficulties. We report a case of a neonate with right-sided osseous fusion between the maxilla and mandibular body, initially unrecognized at multiple centers. The patient developed severe hyperbilirubinemia and dehydration due to impaired feeding. Computed tomography with three-dimensional reconstruction confirmed the diagnosis and revealed no involvement of the temporomandibular joint or tooth buds. Ultrasonic osteotomy was used to resect the fusion, res…

Intraosseous lipomas are rare benign tumors of mesenchymal origin, particularly unusual in the maxilla. Spindle cell lipomas, a histological subtype, are even more uncommon, with limited cases reported in the literature. This report documents a rare case of intraosseous spindle cell lipoma in the anterior maxilla of a 66-year-old male and presents a review of 39 gnathic lipoma cases from the literature published between 1976 and 2024. The majority of the gnathic intraosseous lipomas (32/39, 82%) occurred in the mandible. The posterior region was more frequently affected than the anterior regio…

Ewing sarcoma (ES) is a small, round, blue cell malignant neoplasm that rarely occurs in the craniofacial skeleton, with presentation in the maxilla being exceedingly rare. We report the case of a 14-year-old girl with a two-year history of slowly progressive facial swelling that has recently become greatly enlarged. Imaging revealed an aggressive expansile left maxillary lesion with soft-tissue involvement and internal calcifications. Surgical resection was performed, and histopathological analysis showed sheets of homogenous round cells with minimal cytoplasm and round nuclei. Immunohistoche…

BACKGROUND: Fibrosarcoma is a rare, invasive soft tissue sarcoma that is more common in males aged 30-60 years. It accounts for 1% of all malignancies in the human body. Treatment is radical surgical removal, radiotherapy, and chemotherapy.
CASE PRESENTATION: A 58-year-old Khorasani Persian male patient with a complaint of expansion, pain, and paresthesia in the left maxillary quadrant was diagnosed with infiltrative high-grade sarcoma. Tumoral cells were strongly diffuse positive for vimentin and negative for S100 and desmin, indicating high-grade adult fibrosarcoma.
CONCLUSION: This report…

This case report presents a rare instance of a Calcifying Epithelial Odontogenic Tumor (CEOT) in the maxilla of a 52-year-old male, characterized by a painless, growing mass and facial asymmetry. Imaging revealed extensive involvement of the maxillary sinus, nasal cavity, pterygoid plate, and infraorbital margin. Histopathological analysis confirmed the diagnosis, showing polyhedral odontogenic epithelium with amyloid-like material. The patient underwent conservative surgical excision, preserving function and aesthetics, with no recurrence observed at 16 months postoperatively. This case under…

Maxillary ameloblastoma is one of the rarest odontogenic epithelial tumors encountered, as 80% of ameloblastomas are seen within the mandible. Ameloblastoma is usually incidentally detected in the third to fourth decades of life, as most patients remain asymptomatic; yet some patients may complain of a slowly growing, painless swelling. We present a case of maxillary ameloblastoma with pulmonary metastasis along with a brief literature review. A 17-year-old male initially presented with painless right facial swelling, which, on examination, was non-tender, immobile, irregular, pink in color, w…

Peripheral giant cell granuloma (PGCG) is described as an elevated lesion that is located mostly on the gingival mucosa and alveolar crest, consecutive to irritative factors and trauma. It predominantly occurs more in the mandible than the maxilla, and it is usually seen in the 4th to the 6th decades. The clinical appearance of this lesion is red-bluish in color, presenting a similar tissue to the one observed in the liver, usually measuring less than 2 cm. The treatment of the PGCG is the surgical excision. The recurrence of this lesion is rarely described in the literature. The present case …

STATEMENT OF PROBLEM: Rehabilitation of the partially or completely edentulous posterior maxilla using dental implants is a clinical challenge because of the presence of the maxillary sinus, as well as the low quality and quantity of bone in that region. In addition to bone augmentation procedures, posterior maxillary rehabilitation using implants includes their anchoring in bones such as the zygoma, pterygoid, and maxillary tuberosity, as well as in short implants. However, the performance of pterygoid and tuberosity implants in the atrophic posterior maxilla is unclear.
PURPOSE: The purpose…

Bone plasty of congenital and residual defects of the alveolar process of the upper jaw is a technically difficult surgical intervention which is necessary for full rehabilitation of a patient with congenital lip and palate clefts. This operation creates conditions for stabilization of maxilla fragments and normalization of the upper jaw development and also for successful teeth eruption. The aim of the study - to assess the state of oral hygiene before surgery in children with congenital residual defect of the alveolar process we used Fedorov-Volodkina Hygiene Index and index of the intensity…

BACKGROUND: Odontogenic keratocyst (OKC) is the third most common odontogenic cyst which arises from cell rests of dental lamina, and usually observed in the jaws. Because OKC is noted for its high rate of recurrence, there are various treatment strategies. Here, we present a rare case of OKC which occupied the entire maxillary sinus and pterygoid process of the sphenoid bone extending nearly to the skull base.
CASE PRESENTATION: The patient was a 21-year-old male and underwent surgical removal of the cyst using the Caldwell-Luc procedure which in this case extended the surgical approach to t…

Benign fibrous histiocytoma (FH) is a benign soft-tissue tumor that can present as a fibrous tissue mass anywhere in the body. The involvement of the paranasal sinuses is extremely rare, and very few cases have been reported in literature till date. We here report a case of benign FH localized in the maxillary sinus. The clinical and histological features of the lesion are discussed with a brief literature review of this pathology in the paranasal sinuses.

Osteopetrosis is a rare hereditary bone disorder that results in an increase in bone density due to gene mutations and osteoclastic dysfunction. This may lead to cranial nerve compression, bone fractures and osteomyelitis. Osteomyelitis of the maxilla is rare even in osteopetrosis patients. We report on a case of a 25-year-old male who presented with multiple episodes of osteomyelitis of the maxilla following dental extractions. The patient was initially managed with the incision and drainage of an acute infection, and intravenous amoxicillin-clavulanic acid. This was followed by the debrideme…

Composite tissue allotransplantation of the face has led to renewed interest in the vascularization of the maxilla. The maxillary artery, which is deep within the tissue and difficult to access, is considered the main artery of the maxilla. The objective of this study was to describe the distribution of the maxillary artery in the deep regions of the face and maxilla. Twenty-four maxillae were studied, of which 20 were injected with latex and four with India ink. The maxillary artery in the pterygopalatine fossa gave rise to the sphenopalatine artery, infraorbital artery, descending palatine a…
