Ophthalmic complications are rare following maxillary osteotomies. Potential complications include a decrease in visual acuity, extraocular muscle dysfunction, neuroparalytic keratitis, and nasolacrimal problems involving both an increase or a decrease in tearing. Ophthalmic injuries appear to be primarily mediated through indirect injuries to neurovascular structures occurring from traction, compression, or contrecoup injuries from forces transmitted during the pterygomaxillary dysjunction using an osteotome or from fractures extending to the base of the skull or orbit associated with the pte

This study evaluated the biomechanical performance of 3 fixation systems-L-shaped miniplates, prebent plates, and a single-piece patient-specific plate (PSP)-for maxillary stabilization following Le Fort I osteotomy using finite element analysis. A three-dimensional finite element model of the maxilla was constructed from computed tomography data of a patient with maxillary deficiency. Simulated Le Fort I osteotomies included 5 mm maxillary advancement and 6 mm downgrafting, with iliac bone grafts placed between the segments. Fixation was achieved using 4 L-shaped miniplates, 2 prebent plates,

Midfacial hypoplasia is a common condition in clinical practice. It is characteristic of various craniofacial syndromes but can also occur as an isolated form of dento-skeletal dysmorphosis. Managing this condition in growing patients is challenging. Surgical approaches have high relapse rates, whereas orthodontic treatments yield variable results in midface advancement and stability. This study evaluates the effectiveness of the Meazzini-Mazzoleni Enhanced Sutural Protraction (MMESP) protocol for correcting severe midfacial hypoplasia in growing patients. Ten patients, with or without associa

BACKGROUND: Cleft lip and palate (CLP) frequently cause maxillary hypoplasia, requiring surgical advancement, and virtual surgical planning (VSP) is increasingly used to improve accuracy. This systematic review and meta-analysis synthesized the evidence on VSP-assisted maxillary advancement in patients with CLP, with surgical accuracy as the primary outcome and skeletal stability, intraoperative efficiency, complications, patient-reported outcomes, and software utility as the secondary outcomes.
METHODS: This review followed the PRISMA 2020 statement and was prospectively registered (PROSPERO

PURPOSE: Localization of the lacrimal sac is a critical step during endoscopic dacryocystorhinostomy (endo-DCR). A "light pipe" can be used to transilluminate the lacrimal sac endonasally. We hypothesized that this may misguide the surgeon learning endo-DCR to create an osteotomy mostly posterior to the maxillary line if only the bone overlying the transillumination was to be removed, as the thinner lacrimal bone will transmit light more readily than the thicker maxillary bone of the frontal process of the maxilla that forms the anterior lacrimal sac fossa.
METHODS: The charts of 32 patients

OBJECTIVE: The purpose of the present study was to examine the possibility that postoperative velopharyngeal function following maxillary advancement could be predicted using preoperative electromyography of the levator veli palatini.
DESIGN: Levator muscle electromyography was recorded preoperatively during speech and blowing. Levator activity was expressed as a percentage relative to the maximum value observed throughout the experiment. Postoperative velopharyngeal function was evaluated by means of perceptual judgment and nasoendoscopy.
PARTICIPANTS: The subjects were four patients with r

INTRODUCTION: Orthognathic surgery can lead to sinus alterations, including sinusitis, attributed to the exposure of maxillary sinuses during Le Fort I osteotomy. Furthermore, being a hospital-based procedure, there is potential risk of complications arising from bacteria prevalent in such environments. This study evaluated maxillary sinusitis occurrence and the presence of multidrug-resistant bacteria in the nasal cavity before and after orthognathic surgery.
METHODS: Ten patients with dentofacial deformities underwent Le Fort I osteotomy. Clinical evaluations using SNOT-22 questionnaire wer

BACKGROUND: Alar base widening is a common soft tissue change following Le Fort I osteotomy. Although alar cinch sutures are commonly used to limit this change, their long-term stability remains variable. Nasal sill resection may provide a direct resective option for managing horizontal excess at the alar base; however, its use concomitantly with maxillary orthognathic surgery has not been well described. This retrospective clinical series aimed to describe longitudinal nasal base width changes and patient-reported aesthetic outcomes following nasal sill resection performed concomitantly with

UNLABELLED: The lacrimal sac is the structure most vulnerable to injury when performing rhinoplastic osteotomies. When performed in a low lateral position or along the frontal process of the frontal-maxillary suture, osteotomies can potentially tear the medial canthal tendon and injure the underlying lacrimal sac, possibly resulting in dacryocystitis. In this case report, the authors discuss a case of dacryocystitis following primary rhinoplasty; this injury was repaired with endoscopic dacryocystorhinostomy (DCR) using a Sonopet ultrasonic bone aspirator (Stryker, Kalamazoo, Michigan) at a si

In 342 patients with dentofacial deformities cephalometric analyses were preoperatively performed. A study of occlusion on cast models as the simulated new position of the facial soft tissues was done on the photographs. Surgical treatment using bimaxillary osteotomies for correction of dentofacial deformities was done in 38 patients: 31 with prognathism and maxillary hypoplasia, 5 with facial asymmetry and laterognathism and in 2 with otomandibular dysostosis. The LeFort I type osteotomy was used for the upper jaw and Obwegeser technique for the lower jaw. Intermaxillary and circumzygomatic i

The presence of maxillary septa may render sinus augmentation more challenging particularly when encountered at the ideal implant position. This article demonstrated a novel technique for lateral access sinus augmentation using an assembled surgical guide to achieve proper lateral window outline, precise septum identification and osteotomy, and secure membrane detachment. This technique increases the predictability and efficiency of the procedure while reducing the risk of complications.

Epistaxis following maxillofacial trauma or maxillofacial surgery is uncommon. It usually occurs within 24 hours of the injury and can usually be controlled by packing. Rarely internal maxillary artery ligation is necessary and embolization has been used in some cases of severe trauma. We present a case of severe late-onset epistaxis following Le Fort I osteotomy. The diagnostic approach and treatment are discussed.
