The effect of suppression of gonadotropin secretion was evaluated in 21 patients with the polycystic ovary syndrome. Medroxyprogesterone acetate (MPA) was administered intramuscularly in a dose of 400 mg every 15 days for 9 months. A significant decrease in luteinizing hormone (LH) and testosterone levels (70 and 40%, respectively) was apparent after 3 months. Three types of sexually transmitted infections were investigated among 150 family planning clients using different methods of contraception and 50 controls. For oral contraception, intrauterine contraceptive device, medroxyprogesterone acetate (depo-provera) and controls, the incidence of trichomonas was 0%, 6%, 8% and 4%; for gonorrhoea it was 0%, 8%, 2% and 6%; for syphilis it was 4%, 34%, 16% and 12%. Only the difference in prevalence of syphilis among intrauterine contraceptive users and control was statistically significant (P less than 0.3).

Loss of efficacy of hormonal contraceptives is of particular concern for women who are overweight or obese. Only recently have clinical trials started to include overweight and obese women in their study populations, limiting the data available on which to make the best prescribing decisions for hormonal contraceptives for these patients. Other methods, such as the copper or levonorgestrel-containing intrauterine device, the etonogestrel implant, and depot medroxyprogesterone acetate, are preferred over combined oral contraceptives, the transdermal patch, and the vaginal ring.

Chronic pelvic pain is a symptom of poorly understood aetiology. Pelvic congestion and psychological factors have been identified as important aetiological factors but surprisingly few studies of therapies directed at these aetiologies have been performed. A limited number of trials suggest that the progestagen medroxyprogesterone acetate may be useful in relieving symptoms, but benefit may be restricted to duration of treatment only. Further research is required into both the basis of chronic pelvic pain and its medical management.
