Holmes Tremor Following Midbrain Abscess in an Immunocompromised Patient: A Case Report.

Holmes tremor (HT) is a rare occurrence due to insults located at the midbrain. We are reporting a case of HT, which occurred in a retroviral disease patient who presented with a 1-week history of fever associated with third and fourth cranial nerve palsy. Magnetic resonance imaging of the brain revealed a midbrain lesion favoring abscess with blood investigations revealed positive serology for toxoplasmosis. The patient was started on toxoplasmosis treatment coupled with a six-week course of antibiotics. Following one-month post-treatment, the patient complained of a tremor that characteristi

Diencephalic-mesencephalic junction dysplasia: case report and literature review.

Diencephalic-mesencephalic junction (DMJ) dysplasia is a rare congenital brain malformation characterized by a poorly defined junction between the diencephalon and mesencephalon, often associated with a butterfly-like contour of the midbrain on magnetic resonance imaging (MR). We report the case of a newborn female diagnosed prenatally with DMJ dysplasia who presented with severe ventriculomegaly, hydrocephalus, and oligohydramnios. Prenatal MRI at 32 weeks revealed a thickened interthalamic adhesion, an elongated midbrain with ventral cleft, aqueductal stenosis, and corpus callosum dysgenesis…

Single nucleus transcriptomics of ventral midbrain identifies glial activation associated with chronic opioid use disorder.

Dynamic interactions of neurons and glia in the ventral midbrain mediate reward and addiction behavior. We studied gene expression in 212,713 ventral midbrain single nuclei from 95 individuals with history of opioid misuse, and individuals without drug exposure. Chronic exposure to opioids was not associated with change in proportions of glial and neuronal subtypes, however glial transcriptomes were broadly altered, involving 9.5 - 6.2% of expressed genes within microglia, oligodendrocytes, and astrocytes. Genes associated with activation of the immune response including interferon, NFkB signa

Single-cell transcriptional and functional analysis of dopaminergic neurons in organoid-like cultures derived from human fetal midbrain.

Significant efforts are ongoing to develop refined differentiation protocols to generate midbrain dopamine (DA) neurons from pluripotent stem cells for application in disease modeling, diagnostics, drug screening and cell-based therapies for Parkinson's disease. An increased understanding of the timing and molecular mechanisms that promote the generation of distinct subtypes of human midbrain DA during development will be essential for guiding future efforts to generate molecularly defined and subtype-specific DA neurons from pluripotent stem cells. Here, we use droplet-based single-cell RNA s

β2-subunit alternative splicing stabilizes Cav2.3 Ca2+ channel activity during continuous midbrain dopamine neuron-like activity.

In dopaminergic (DA) Substantia nigra (SN) neurons Cav2.3 R-type Ca2+-currents contribute to somatodendritic Ca2+-oscillations. This activity may contribute to the selective degeneration of these neurons in Parkinson's disease (PD) since Cav2.3-knockout is neuroprotective in a PD mouse model. Here, we show that in tsA-201-cells the membrane-anchored β2-splice variants β2a and β2e are required to stabilize Cav2.3 gating properties allowing sustained Cav2.3 availability during simulated pacemaking and enhanced Ca2+-currents during bursts. We confirmed the expression of β2a- and β2e-subunit trans

Clinical Reasoning: A 48-Year-Old Woman Presenting With Vertigo, Ptosis, and Red Eyes.

We describe acute vestibular syndrome in a 48-year-old woman with breast cancer who was finally found to have anti-Ma2-associated encephalitis. Although the initial diagnosis was vestibular neuritis elsewhere, progression of symptoms and additional findings of bilateral ptosis and circumlimbal injections, vertical saccadic slowing, and impaired convergence led to a suspicion of a rostral midbrain lesion and final diagnosis. The patient's symptoms and ocular motor signs improved markedly after administration of IV methylprednisolone and oral tacrolimus. Our patient again stresses the importance

Neuroprotective Effects of 7, 8-dihydroxyflavone on Midbrain Dopaminergic Neurons in MPP+-treated Monkeys.

Parkinson's disease (PD) is one common neurodegenerative disease caused by a significant loss of midbrain dopaminergic neurons. Previous reports showed that 7, 8- dihydroxyflavone (7, 8-DHF) as a potent TrkB agonist can mimic BDNF and play neuroprotective roles for mouse dopaminergic neurons. Nonetheless, the safety and neuroprotective effects are unclear in monkey models of PD. Here, we find that 7, 8-DHF could be absorbed and metabolized into 7-hydroxy-8-methoxyflavone through oral administration in monkeys. The half-life time of 7, 8-DHF in monkey plasma is about 4-8 hrs. Furthermore, these

Subdivisions of the auditory midbrain (n. mesencephalicus lateralis, pars dorsalis) in zebra finches using calcium-binding protein immunocytochemistry.

The midbrain nucleus mesencephalicus lateralis pars dorsalis (MLd) is thought to be the avian homologue of the central nucleus of the mammalian inferior colliculus. As such, it is a major relay in the ascending auditory pathway of all birds and in songbirds mediates the auditory feedback necessary for the learning and maintenance of song. To clarify the organization of MLd, we applied three calcium binding protein antibodies to tissue sections from the brains of adult male and female zebra finches. The staining patterns resulting from the application of parvalbumin, calbindin and calretinin an

Transcranial sonography in restless legs syndrome.

Restless legs syndrome (RLS) is one of the most common neurological disorders. The diagnosis is mainly based on the patient's report of the characteristic symptoms and further on the response to dopaminergic treatment. In the past years, transcranial B-Mode sonography (TCS) of the mesencephalic brainstem has been introduced as a promising new method to assist the diagnostic approach to RLS. This review of literature describes the typical TCS findings in RLS including hypoechogenicity of the substantia nigra and raphe as well as hyperechogenicity of the red nucleus. The diagnostic parameters fo

Bilateral wallerian degeneration of the middle cerebellar peduncles due to unilateral pontine infarction.

We report the case of a patient with bilateral and symmetrical T2 hyperintensities of the middle cerebellar peduncles. She had a history of left pontine infarction 8 months before. This was attributed to bilateral Wallerian degeneration. MR Spectroscopy showed decreased N-acetyl aspartate/Creatine (NAA/Cr) ratio in the cerebellar peduncles as well as in the whole cerebellum. We hypothesize that this could reflect neuronal degeneration following a stroke.

Dorsal mesencephalic lipoma with inferior collicular hypoplasia: a case report and review of literature.

Intracranial lipomas are rare lesions the pathogenesis of which is still a matter of debate. We report the case of a patient presenting an asymptomatic lipoma located in the quadrigeminal and supra-cerebellar cisterns. The lipoma was associated with hypoplasia of the right inferior colliculus and of the rostral part of the vermis. Analysis of the relationships between neural tube and meninges prompts us to propose that the primary event of this malformation is a defect of the neural anlage which gives rise to both nervous dysplasia and lack of meningeal induction responsible for lipoma develop

[Isolated oculomotor nerve palsy caused by mesencephalic hemorrhage].

A 62-year-old man with a history of diabetes mellitus, hypertension and liver damage has sudden diplopia on Nov. 4, 1992. Ocular movements of this right eye were normal, but his left eyelid was completely ptotic, and left pupil was dilated and nonreactive to light. In primary position, his left eye deviated outward, and could not move to any direction. He was diagnosed as having total oculomotor nerve palsy of the left side without any other neurological signs or symptoms; his consciousness level was alert and mental state was normal. The present case showed normal facial sensation, no facial

Accommodation and the nerve pathway to the ciliary muscle: a review.

Evidence has accumulated recently which challenges the conventional description of the parasympathetic division of the oculomotor nerve pathway to intraocular smooth muscles. It is claimed that the pathway from the midbrain to the ciliary muscle is direct and uninterrupted by a synapse in the ciliary ganglion. A similar, separate claim has been made regarding the innervation of the sphincter muscle of the iris. The weaknesses in the arguments supporting the revised versions are discussed in this review and they are rejected in favour of the conventional pathway.

Distribution of mu, delta, and kappa opiate receptor types in the forebrain and midbrain of pigeons.

Ligands that are highly specific for the mu, delta, and kappa opiate receptor binding sites in mammalian brains have been identified and used to map the distribution of these receptor types in the brains of various mammalian species. In the present study, the selectivity and binding characteristics in the pigeon brain of three such ligands were examined by in vitro receptor binding techniques and found to be similar to those reported in previous studies on mammalian species. These ligands were then used in conjunction with autoradiographic receptor binding techniques to study the distribution
