Round window membrane rupture following blunt force trauma.

Round window rupture following barotrauma such as diving and air travel is well documented. However, we describe a case of round window rupture following a slap to the ear, which has not been described previously. A 12-year-old boy was slapped by his teacher in rural Nepal over his left ear and immediately noted hearing loss. There was no dizziness or tinnitus. He presented to our unit 21 days after the trauma and examination showed a perforation of the tympanic membrane in the anterior inferior quadrant with an otherwise normal ear examination and no nystagmus seen. Audiogram showed a left pr

Intra-tympanic membrane bleeding after grommet insertion and tympanosclerosis.

The aetiology of tympanosclerosis following grommet insertion remains controversial. In this study tympanosclerotic changes of the tympanic membrane in 92 ears were shown to be related to the presence of intra-epithelial haemorrhage shortly after myringotomy with grommet insertion (P less than 0.01) and to the grommet being in situ on follow-up 6 months later (P less than 0.025). Ears in which haemorrhage was absent and where the grommet had extruded did not develop tympanosclerosis. These findings would indicate that bleeding, if causally implicated is not the sole aetiological factor, but ca

Audiological Alterations in Patients With Cleft Palate.

INTRODUCTION: Chronic otitis media with effusion (OME) is a recurrent complication, usually found in cleft palate patients.Conductive hearing loss is the result of the Eustachian tube dysfunction caused by the absence of fusion and the altered insertion of the muscles of the secondary palate. It is also the consequence of an ineffective muscular reconstruction after primary cleft palate repair. METHODS: This is a cohort study to compare 4 groups of patients born with isolated cleft lip (ICL), unilateral cleft lip/palate (UCLP), bilateral cleft lip/palate (BCLP), and isolated cleft palate (ICP

Association between symptomatic submucous cleft palate and otologic disease: A retrospective review.

INTRODUCTION: Submucous cleft palate (SMCP) is the most common congenital malformation of the posterior palate and characterized in part by palatal muscle diastasis. Patients with SMCP are considered symptomatic when suffering from velopharyngeal insufficiency which leads to hypernasality of speech and excess nasal air emission. SMCP results from defects in the muscles thought to be responsible for regulating the patency of the eustachian tube which can lead to a number of middle ear pathologies and ultimately hearing loss. Hearing loss, especially at a young age, can make speech acquisition a

Management of retraction pockets of the pars tensa in children by excision and ventilation tube insertion.

Retraction pockets of the pars tensa in children can result in erosion of the ossicles leading to hearing loss and eventually cholesteatoma formation. Several different types of treatment for the more severe grades of retraction pocket have been described. The aim of this pilot study was to assess the outcome following simple excision and ventilation tube insertion of grade II, III and IV retraction pockets of the pars tensa. The eardrums were graded according to Sadé's classification (1979). There were seven grade II and four grade III retractions. Ten eardrums healed completely in a mean tim

The effect of balloon dilatation eustachian tuboplasty combined with grommet insertion on the structure and function of the eustachian tube in patients with refractory otitis media with effusion.

BACKGROUND: The treatments for refractory secretory otitis media with effusion usually choose long-term grommet insertion. This study evaluated the effect of balloon eustachian tuboplasty combined with grommet insertion on the function and the opening length of the eustachian tube in patients with refractory otitis media with effusion. METHODS: Fifty-seven patients with refractory otitis media with effusion were enrolled. A three-dimensional reconstruction of an iohexol-enhanced computed tomography image was applied to evaluate the structural and length changes of the eustachian tube at both

Positive pressure therapy for Meniere's syndrome/disease with a Meniett device: a systematic review of randomised controlled trials.

BACKGROUND: Incidence rates of Meniere's syndrome/disease vary considerably from 157 per 100 000 in the United Kingdom to 15 per 100 000 in the United States. A wide range of treatments are used for the treatment of the condition with no consensus on the most effective intervention. OBJECTIVES: To assess the effectiveness of the Meniett device in reducing the frequency and severity of vertigo in Meniere's syndrome/disease. SEARCH METHODS: The Cochrane Ear, Nose and Throat Disorders Group Trials Register; the Cochrane Central Register of Controlled Trials (CENTRAL); PubMed; EMBASE; CINAHL; We

[Clinical comparative study on the treatment characteristics of secretory otitis media between cleft and non-cleft palate patients].

OBJECTIVE: To discuss the treatment characteristics of secretory otitis media (SOM) in cleft palate children. METHODS: A total of 319 patients (524 ears) with SOM and cleft palate (3-14 years old) who accepted treatment were divided into experiment group A, group B, and group C according to effusion characteristics in the middle ear and tympanic pressure. Group A included 112 patients with serous effusion (198 ears). Group B included 162 patients with mucinous effusion (248 ears). Group C included 45 patients (78 ears) with negative pressure in the middle ear without effusion and an acoustic

Accuracy of Internet guidance on pediatric otolaryngology procedures.

OBJECTIVES: To evaluate the reliability and accuracy of the most visited Internet websites for the frequently performed surgical pediatric otorhinolaryngology procedures. STUDY: A google.com search was used to determine the top 20 Internet sites for the terms 'adenoidectomy', 'tonsillectomy' and 'ventilation tube surgery'. The DISCERN plus questionnaire was used to score each website. Furthermore, a 'hit list' of reliable and accurate websites was created depending on the most clicked healthcare organizations with corporate identities and the personal websites of the otolaryngologists with ac

Ear drum perforations in children after ventilation tube treatment.

About 1% of ears treated with straight Teflon tubes get permanent drum perforations after extrusion of the tubes, according to the literature. This paper reports the results of treatment of 20 ears in 17 children who underwent myringoplasty to repair such perforations at a mean age of 9.6 years and with a mean follow-up of 2.4 years. The following history was found: 60% (12/20) perforated after 1 insertion, 47% (9/19) perforated within 12 months after insertion. Seventy percent (14/20) had recurrent infectious problems during tube-treatment and after the perforation was diagnosed. The mean pre

Secretory otitis media and high-frequency hearing loss.

31 children (mean age 13 years; range 8.5-17.2) who had been tympanostomy treated for SOM during the years 1977-1980 were invited for follow-up examination. 29 normal age peers were enrolled as controls. The test battery included clinical pneumotoscopical examination and conventional pure-tone audiometry as well as air-conduction and electric bone-conduction high-frequency audiometry. High-frequency hearing losses (6-18 kHz) were found as sequels of SOM. The median threshold difference between the SOM group and the controls varied from 0 to 10 dB depending on the frequency. The hearing losses

What is new in otitis media?

The "wait and see" approach in acute otitis media (AOM), consisting of postponing the antibiotic administration for a few days, has been advocated mainly to counteract the increased bacterial resistance in respiratory infections. This approach is not justified in children less than 2 years of age and this for several reasons. First, AOM is an acute inflammation of the middle ear caused in about 70% of cases by bacteria. Redness and bulging of the tympanic membrane are characteristic findings in bacterial AOM. Second, AOM is associated with long-term dysfunction of the inflamed eustachian tube
