Treatment of ectopically erupting permanent molars.

Eruption of a first permanent molar ectopically is a common occurrence in any office in which a significant number of children are treated. Approximately 65 per cent of the ectopically erupting molars will self-correct. It is imperative that the dentist closely examine all preschool children to determine the possibility that the condition will occur and follow the child closely to determine the subsequent need for treatment. If clinical treatment is advisable the dentist can use a modified version of the Humphrey appliance to successfully and rapidly correct ectopically erupting molars with re…

Follow-up study of apicoectomized molars.

The present material consists of 46 respected molars, 20 of which were located in the maxilla and 26 in the mandible. Altogether, 93 different roots were resected and 109 root fillings made. The follow-up study was made 1-6 years after the apicoectomy. The patients were examined clinically, special attention being paid to the subjective and objective symptoms shown by each apicoetomized tooth. Then a stereoscopic picture of the resected tooth and an orthopantomogram were taken. Of these 46 apicoectomized teeth, 33 (71%) showed complete healing, five (11%) were uncertain, four (9%) showed unsat…

[Removal or retention of nonimpacted third molars].

Nonimpacted third molars (N-M3s) refer to the third molars (M3s) that completely erupt and reach the occlusal plane without rotation. Although the removal of unerupted M3s in young adults is generally approved by most dentists, the decision on removal or retention of N-M3s remains challenging as a result of unawareness of the potential harm of N-M3s. Recent studies suggested that N-M3s were associated with deteriorated effects to the health of adjacent teeth, including higher risks of caries and alveolar bone resorption. Therefore, regular monitoring and early clinical decision on N-M3s are es…

[Third molars in the orthodontic therapeutic decision].

INTRODUCTION: Despite a high prevalence of agenesis, third molars should frequently be considered in our orthodontic treatment plans. MATERIAL AND METHOD: The aim of this study was to describe, according to the literature, the therapeutic possibilities of third molar management. RESULTS: The avulsion isn't systematic. The French National Authority for Health has updated its guidance about it. Indeed, surgical procedures can present with potentially irreversible risks (nerve damage, bone necrosis). It is necessary to assess the benefit/risk balance and to inform the patient accordingly. Wheth…

[Asymptomatic third molars; To remove or not to remove?].

Three adult patients, ages 28 (female), 36 (male) and 53 (male), respectively, were referred by their dentists to an orofacial surgeon concerning problems with their third molars. With the woman, teeth 38 and 48 were removed. After the removal of tooth 38, a permanent periodontal attachment loss of the adjoining second molar occurred. With the second patient, retention of tooth 48 caused serious damage to the adjoining second molar, resulting in the loss of the tooth. At the time of removal, there was a question of increased risk of inflammatory complications and nerve damage. With the third p…

Infiltration anesthesia for extraction of the mandibular molars.

PURPOSE: Profound anesthesia for mandibular molars by buccal infiltration has been tried in recent years, with promising results. This prospective, randomized, single-blinded, crossover study investigated the clinical anesthetic efficacy obtained with 1:100,000 epinephrine plus 4% articaine (A100) 1.8 versus 3.6 mL as mandibular first molar buccal infiltration during removal of impacted lower third molars. MATERIALS AND METHODS: Thirty adult patients underwent removal of symmetrically positioned impacted lower third molars in 2 separate appointments. The patients randomly received mandibular …

Spontaneous eruption of impacted second molars.

OBJECTIVES: To propose a treatment protocol for the management of impacted second molars. MATERIALS AND METHODS: Patients were randomly divided into two equal groups (20 patients in each). The patients in the first group underwent a surgical removal of the mucosa covering retained teeth; the second group was the control group and patients were not treated but only observed. The total number of retained teeth considered were 66 (34 in the first group and 32 in the control group). A Chi squared test has been performed to compare the prevalence of eruption in the two groups. We compared the freq…

The impact of molar pregnancy on the male partner.

BACKGROUND: Molar pregnancy is a complication of 1 in 200-2000 pregnancies whereby abnormal placental tissue proliferates in the absence of a fetus and may lead to metastases. The disease origin lies in dispermy or dual fertilisation of the egg. The aim of this study was to explore the impact of molar pregnancy upon the male partner. METHODS: Institutional ethics committee approval and individual consent were obtained. All women listed on the state molar pregnancy database who were receiving active follow-up (n = 102) and a random sample of women who had been registered in the previous 30 yea…

The success of emergency pulpotomies in primary molars.

PURPOSE: The purpose of this retrospective study was to assess the success of pulpotomies performed on an emergency basis and restored with a temporary zinc-oxide eugenol restorative material. METHODS: Records were collected from 216 pulpotomies in primary molars performed on an emergency basis by pediatric dental residents at the University of Florida pediatric dental clinic between July 1999 and June 2001. The selection criteria include teeth with a positive history of pain but with absence of clinical and radiographic signs of pulp degeneration. Formocresol was the medicament used and the …
