Analysis on the application of a CT-guided medical adhesive-based localization method in sublobectomy to ensure surgical margins.

BACKGROUND: Sublobectomy is one of the standard surgical procedures for early stage lung cancer. The distance of resection margin is the basic demand of sublobectomy. Therefore, it is of great clinical significance to ensure the margin distance in sublobectomy. How to ensure the margin distance in sublobectomy is still a difficult problem for thoracic surgeons. Our center has recently adopted the CT-guided medical adhesive localization method to solve the problem of insufficient lung margins with excellent results. METHODS: The clinical data of 40 patients with pulmonary nodules, who received

Effects of tube voltage, radiation dose and adaptive statistical iterative reconstruction strength level on the detection and characterization of pulmonary nodules in ultra-low-dose chest CT.

OBJECTIVE: To explore the effects of tube voltage, radiation dose and adaptive statistical iterative reconstruction (ASiR-V) strength level on the detection and characterization of pulmonary nodules by an artificial intelligence (AI) software in ultra-low-dose chest CT (ULDCT). MATERIALS AND METHODS: An anthropomorphic thorax phantom containing 12 spherical simulated nodules (Diameter: 12 mm, 10 mm, 8 mm, 5 mm; CT value: -800HU, -630HU, 100HU) was scanned with three ULDCT protocols: Dose-1 (70kVp:0.11mSv, 100kVp:0.10mSv), Dose-2 (70kVp:0.34mSv, 100kVp:0.32mSv), Dose-3 (70kVp:0.53mSv, 100kVp:0

[An unusual cause of pulmonary cysts].

INTRODUCTION: Multiple pathologies may be associated with cystic appearances in the lungs. Lymphangioleiomyomatosis, Langerhans cell histiocytosis and lymphocytic interstitial pneumonia are the principal causes of thin walled pulmonary cysts. CASE REPORT: We report the case of a non-smoking woman of 71 years, a retired farmer, who was admitted on account of cough, increasing dyspnoea, fever and loss of weight that developed following exposure to hay. The thoracic CT scan revealed diffuse ground glass opacities and centrilobular nodules. Multiple thin walled cystic shadows were distributed hom

Multifocal micronodular pneumocyte hyperplasia associated with tuberous sclerosis.

A case of multifocal micronodular pneumocyte hyperplasia (MMPH) associated with tuberous sclerosis is reported. MMPH is a rare pulmonary disorder characterized by the nodular proliferation of type II pneumocytes. In the case presented here, MMPH appeared as multiple, well-defined small nodules with ground-glass opacity. It is necessary to consider MMPH when small nodules with ground-glass opacity are observed on HRCT in patients with tuberous sclerosis.

Delayed diagnosis of granulomatosis with polyangiitis presenting as irregularly shaped solid lung nodules with heterogeneous enhancement: A case report.

RATIONALE: Granulomatosis with polyangiitis (GPA) is a systemic autoimmune vasculitis. Thoracic radiographic findings in GPA can mimic various pulmonary diseases, potentially leading to misdiagnosis. We describe a case of delayed GPA diagnosis, initially presenting as irregularly shaped solid lung nodules with heterogeneous enhancement, such cases are rarely reported. PATIENT CONCERNS: A 69-year-old male was initially misdiagnosed with lung cancer based on the identification of solid lung nodules during routine physical examination. Pathological findings from a lung biopsy were inconclusive.

Pulmonary mucosa-associated lymphoid tissue lymphoma: computed tomography and ¹⁸F fluorodeoxyglucose-positron emission tomography/computed tomography imaging findings and follow-up.

PURPOSE: The aim of the study was to present the computed tomography (CT) and fluorine 18 (F) fluorodeoxyglucose-positron emission tomography (FDG-PET)/CT imaging findings of pulmonary mucosa-associated lymphoid tissue (MALT) lymphoma and evaluate their roles in the follow-up of this tumor. METHODS: Computed tomography and FDG-PET/CT imaging findings of 18 cases of pathologically proven pulmonary MALT lymphoma were reviewed retrospectively. RESULTS: Multiple and solitary lesions were detected in 15 and 3 patients, respectively. Of those patients with multiple pulmonary lesions, 12 were bilat

Pulmonary cryptococcosis mimicking metastatic progression despite systemic disease stability in advanced colon cancer: A case report and literature review.

RATIONALE: In patients with advanced cancer, newly detected pulmonary nodules are often interpreted as metastatic progression. The distinctive feature of this case was the appearance of multiple new lung nodules during otherwise controlled systemic disease: the primary colonic lesion and hepatic metastases remained stable while tumor marker levels declined. This radiologic-clinical discordance prompted reconsideration of metastatic progression and broadened the differential diagnosis. PATIENT CONCERNS: A patient with advanced colon cancer, who was undergoing systemic chemotherapy and had othe

Pulmonary Epithelial Myoepithelial Carcinoma on 18F-FDG PET/CT: A Case Report.

Pulmonary epithelial myoepithelial carcinoma is very rare. We reported the imaging finding of pulmonary epithelial myoepithelial carcinoma of bronchus in a 61-year-old man on 18F-FDG PET/CT. An irregular mass with an SUVmax of 6.36 growing along right upper lobe bronchus and multiple pulmonary nodules around the mass were found on 18F-FDG PET/CT. Postoperative pathology demonstrated the diagnosis of epithelial myoepithelial carcinoma.

Case 340: Pulmonary Necrobiotic Nodules as a Lung Manifestation of Crohn Disease.

A 36-year-old woman with an otherwise unremarkable medical history was diagnosed with moderate to severe Crohn disease and was started on infliximab. One year later, infliximab was discontinued due to intolerance, and the patient began azathioprine and mesalazine treatment, achieving stable disease. Eight months after discontinuing infliximab, during a Crohn disease flare-up, an urgent abdominal CT examination was performed, partially documenting the lung bases. The patient maintained her therapy, with remission of the flare-up. Sixteen months after discontinuing infliximab and 8 months after

Recent advances in nonsurgical treatment of pulmonary ground-glass nodules.

Since the 1990s, low-dose computed tomography technology has been used in lung cancer screening. With the increase of computed tomography screening, the detection rate of ground-glass nodules (GGN) has increased dramatically. At present, the main treatment strategy for GGN is surgical resection. However, for patients with poor cardiopulmonary functions, a history of lung resection, multiple pulmonary nodules, or the age of >75 years, surgical resection is very difficult and not medically encouraged. This article reviews the applications and outcomes evaluation of nonsurgical treatments, such a…

Large Thoracic Lymphadenopathy and Pulmonary Nodules in Young Man.

A 33-year-old man with obesity, systemic arterial hypertension, and psoriasis who had been treated previously with little success by a pulmonologist for chronic unproductive irritant cough came to the outpatient pulmonary department because of profuse cough and short syncope (probably cough-induced). Chest radiography revealed widened mediastinum with lobular, polycyclic contours that was suspected to be a large mediastinal lymphadenopathy or mediastinal mass.

Cavitary lung nodules as an extraintestinal manifestation of ulcerative colitis.

A man in his 30s with ulcerative colitis (UC) on immunosuppressive agents and extensive travel history presented with subacute dyspnoea and dry cough. CT of the chest demonstrated numerous cavitary pulmonary nodules. An extensive infectious, malignant and autoimmune evaluation was pursued, ultimately with histopathology most consistent with necrobiotic lung nodules as an extraintestinal manifestation of UC. Steroids and ustekinumab were initiated with improvement in symptoms and resolution of cavitary lesions on follow-up imaging. In a patient with inflammatory bowel disease and cavitary lung

Benign pulmonary metastasizing leiomyomatosis: case report.

The authors report a rare case of leiomyomatosis of the lung diagnosed in a 43-year-old woman, with uterine intravenous leiomyomatosis. Benign metastasizing leiomyoma (BML) is an extremely rare lesion characterized by usually multiple, benign-appearing smooth muscle tumors of the lung in females with coexisting uterine leiomyoma. On the basis of their histological and immunohistological features, a unified histogenetic view of leiomyomas with vascular invasion (LWVI) and BML of the uterus is proposed. LWVI and BML may be the same pathological entity and microscopic vascular invasion may repres

[A case of hypereosinophilia with diffuse pulmonary nodules].

We reported a case of a 36-year-old woman who presented with cough, dyspnea, hypereosinophilia, multiple pulmonary nodules and mediastinal lymphadenopathy. The percentage of eosinophils in bronchoalveolar lavage fluid (BALF) was as high as 65%. Pathogenic tests and cytologic examination of BALF were negative. Transbronchial lung biopsy and endobronchial ultrasound-guided transbronchial needle aspiration revealed only eosinophil infiltration. As the patient responded poorly to high-dose corticosteroids, a surgical lung biopsy was performed. The pathological diagnosis was angioimmunoblastic T-ce…

Nodular pulmonary amyloidosis diagnosed by ultrasound-guided percutaneous needle biopsy.

Pulmonary amyloidosis is characterized by extracellular deposition of fibrous protein called amyloid in the lungs and has three subtypes: nodular, diffuse, and tracheobronchial amyloidosis. Pulmonary nodular amyloidosis can mimic other lung diseases including infectious diseases, metastatic lung tumors, sarcoidosis, and pulmonary hyalinizing granuloma. A biopsy of the lesion is essential for a definitive diagnosis. Herein, we report the case of a 66-year-old man who presented for shortness of breath on exertion and was diagnosed with nodular pulmonary amyloidosis on ultrasound-guided percutane
