Juvenile Myasthenia Gravis: An Indian Perspective.

BACKGROUND: Myasthenia Gravis is an autoimmune disorder of the neuromuscular junction which is divided into three categories in children: neonatal or transient, congenital myasthenic syndromes, and autoimmune or juvenile myasthenia gravis (JMG). JMG differs from adult MG in many aspects. Characteristics of JMG also vary between children of various ethnic groups. OBJECTIVE: Although common, there is scant data about JMG in India. This study was performed to determine the characteristics of JMG in Indian children. MATERIALS AND METHODS: We did a retrospective chart analysis of all patients who…

Indirect Comparison of Nipocalimab Versus Efgartigimod and Rozanolixizumab in the Treatment of Generalized Myasthenia Gravis.

INTRODUCTION: Nipocalimab, efgartigimod, and rozanolixizumab (the last two cyclically dosed) are approved neonatal Fc receptor (FcRn) blockers for treating generalized myasthenia gravis (gMG). No trials have directly compared these therapies; hence, indirect treatment comparisons (ITCs) were conducted to evaluate their relative efficacy. METHODS: Matching-adjusted indirect comparisons (MAICs) and Bucher ITCs were used to compare nipocalimab vs. efgartigimod and rozanolixizumab for changes from baseline (CFB) in Myasthenia Gravis Activities of Daily Living (MG-ADL) total score observed in thei…

Myasthenic Crisis Unmasking Myocardial Infarction with Nonobstructive Coronary Arteries in a Patient with Undiagnosed Myasthenia Gravis.

We present a case of a patient with undiagnosed myasthenia gravis presenting in myasthenic crisis. In this case, the patient presented primarily with myocardial infarction with nonobstructive coronary arteries and respiratory failure and was later diagnosed to be in myasthenic crisis. The myasthenic crisis was treated with intravenous immunoglobulin (IVIg) and corticosteroids. Given the inadequate response to IVIg and glucocorticoids, the patient was subsequently administered an anti-CD20 monoclonal antibody in the form of rituximab. The patient responded well to rituximab, and her cardiac fun…

Estimating the minimal clinically important difference for the Myasthenia Gravis Quality of Life revised scale (MG-QOL15R).

PURPOSE: To estimate the minimal clinically important difference (MCID) for the MG-QOL15R. METHODS: Data from two multi-country myasthenia gravis studies were used: the ADAPT RCT (n = 157) and the MyRealWorld-MG (MRW-MG) survey (n = 92). MCIDs were estimated using four anchor-based methods: change difference (CD), receiver operating characteristic (ROC) curve with Area Under the Curve (AUC), linear regression, and equipercentile linking. MG-Activities of Daily Living (MG-ADL) was the anchor. MCIDs were applied to the ADAPT RCT to assess the impact of different thresholds. Odds ratios (ORs) and…

Minimal Symptom Expression in Generalized Myasthenia Gravis: A Valuable Patient-Centric Treatment Goal.

INTRODUCTION: Generalized myasthenia gravis (gMG) is a chronic autoimmune neuromuscular disorder characterized by fluctuating muscle weakness and exertional fatigue, often requiring long-term immunosuppressive or biologic treatment. Although international guidelines recommend achieving minimal manifestations as the primary treatment goal, the lack of a standardized measurement approach has limited its application in clinical settings and trials. Minimal Symptom Expression (MSE), defined as a Myasthenia Gravis Activities of Daily Living (MG-ADL) score of 0 or 1, has recently emerged as a practi…

Burden of glucocorticoid use in commercially insured adults with generalized myasthenia gravis in the United States: A retrospective claims-based analysis.

Generalized myasthenia gravis (gMG) is a chronic autoimmune condition characterized by muscle weakness. If frontline treatments inadequately manage symptoms, patients may require use of glucocorticoids (GC); however, long-term use of GCs is associated with toxicities. Few studies have examined the incidence of GC-related toxicities and economic burden among patients with gMG. This study aims to describe GC toxicities, healthcare resource utilization, and costs among US adults with gMG, stratified by level of GC use. Adults with gMG were identified using IQVIA PharMetrics® Plus data and classif…

Pediatric Myasthenia Gravis.

Pediatric myasthenia gravis encompasses a heterogenous group of disorders that cause impaired neuromuscular junction transmission. Patients exhibit a characteristic fatigable weakness that can be life-threatening if respiration is affected. In children, myasthenia gravis can be grouped into 3 subtypes: transient neonatal myasthenia (TNM) arising from transplacental antibody transfer, congenital myasthenic syndromes (CMS) related to genetic defects, and juvenile myasthenia gravis (JMG) caused by antibodies to synaptic components. Diagnostic evaluation combines clinical evaluation with serologic…
