We report a case of accommodative spasm in an 8-year-old girl discovered by handheld photoscreener. The patient was found to have a Chiari I malformation, and managed with atropine drops and reading glasses, ultimately with improvement in her symptoms. We believe this to be the first case of accommodative spasm diagnosis aided by the use of a handheld photoscreener.

Although cosmetic extended-wear contact lenses are generally safe, they can be associated with a number of complications. A review of 124 cases of corneal ulcers treated between Jan. 1, 1982, and June 1, 1983, disclosed six cases in otherwise healthy patients (four women and two men, ranging in age from 15 to 41 years) who wore extended-wear contact lenses for correction of myopia. Following a brief review of various interpretations of the disease and its treatment, the author's observations in 13 cases (18 eyes) treated during a period of ten years are reported. The data (Tables 2 and 3) are discussed with reference to the different forms of clinical manifestation of the disease (Table 4). Therapeutic aspects are outlined on the basis of the personal experience of the authors. If conservative measures (mydriasis, hyperosmotics, carboanhydrase inhibitors) fail to control ciliary block glaucoma, surgical procedures directed at the anterior hyaloid membrane are indicated.

A comparative study of the cycloplegic effect of 1% cyclopentolate and atropine or homatropine was done by means of Lindner's method of retinoscopy.
