Risk Factors and Cancer Screening in Myositis.

The idiopathic inflammatory myopathies, particularly dermatomyositis, are associated with an increased risk of cancer. Lung, ovarian, breast, colon, prostate, and cervical cancers, and hematologic malignancies, are among the most common associated cancers. Risk stratification for cancer in patients with myositis is based on clinical risk factors/red flags, myositis clinical subtypes, and myositis-specific autoantibodies. Clinical risk factors include older age at disease onset, male gender, dysphagia, acute onset/refractory myositis, cutaneous ulceration, necrosis/vasculitis, and elevated infl…

Inclusion body myositis in the rheumatology clinic.

Inclusion body myositis is a rare sporadic inflammatory-degenerative myopathy of the elderly. Despite being the commonest type of acquired myopathy after the age of 50, misdiagnosis is extremely common. The most frequent hurdle in identifying new cases is the wrong diagnosis of polymyositis or motor neuron disease. Novel insights into pathogenic mechanisms have heralded the quest for newer therapeutics as well as drug repurposing in this otherwise progressive disorder.

Up-to-date treatment and management of myositis.

PURPOSE OF REVIEW: Myositis, or idiopathic inflammatory myopathy, is an overarching concept that includes dermatomyositis, polymyositis, immune-mediated necrotizing myopathy and the antisynthetase syndrome. Glucocorticoids are still considered the mainstay of treatment of myositis but some patients require add-on immunosuppressive therapy because of insufficient response to glucocorticoids, relapses when glucocorticoids are tapered, or because they incur glucocorticoid-related side effects. RECENT FINDINGS: The goal of this article was to review (PubMed search from January 2019 through June 2…

[Myositis ossificans located at the chest wall].

We report the clinical case of a classic bone pathology with a surprising and original chest wall topography, so far not described. Myositis ossificans is a benign pseudo-tumoral pathology. Medical imaging therefore constitutes an essential tool for an accurate diagnosis.

Paratesticular myositis ossificans of the spermatic cord.

A 56-year-old man presented with a painless scrotal lump, enlarging over the preceding 1 month. The lump was roughly 1 cm in size, and located in his left hemiscrotum and separate from the testis. An ultrasound revealed an echogenic focus with dystrophic tissue calcification. Subsequent surgical excision and histopathological analysis revealed it to be late-stage myositis ossificans, a benign, extraosseous formation of the bone or cartilage. We report of only the second described case of myositis ossificans of the spermatic cord in the literature to date.

The clinico-serological spectrum of overlap myositis.

PURPOSE OF REVIEW: To provide the most recent evidence on the overlap myositis. RECENT FINDINGS: Several new evidences on the overlap myositides have recently emerged. Regarding the classical myositis associated antibodies, several contributions focused on a better definition of the clinical associations and the disease course associated with these autoantibodies. Moreover, in the last years, new autoantibodies in idiopathic inflammatory myositis or other connective tissue diseases have been identified [namely anti-RuvBL1/2, poly-U-binding factor 60 kDa protein (PUF-60) and cytosolic 5'-nucle…

Myositis as the initial presentation of panarteritis nodosa.

A 47-year-old man presented with weight loss, bilateral calf pain, fever, hypertension, orchitis and oligoarthritis. Lab tests: anemia and elevated muscle enzymes. Resonance magnetic imaging: hyperintensity in gastrocnemius muscles (myositis). Histologic exam of the muscles: inflammatory infiltrate with atrophy and perifascicular regeneration. Treatment: methylprednisone (bolus) and cyclophosphamide. Muscle pain and swelling and difficulty in walking are common in panarteritis nodosa (PAN), whereas histologically demonstrated myositis is not. Even more rare is myositis as the initial presentat…

Lagophthalmos and Ptosis in Inclusion Body Myositis.

Sporadic inclusion body myositis is the most commonly acquired type of idiopathic inflammatory myopathy in people aged 50 and above. There is early weakness and atrophy of forearms and quadriceps and a third of patients also have mild facial weakness. Extraocular muscles are not affected and ptosis is rarely seen. The authors describe a unique case in which inclusion body myositis presented with early mid face weakness and atrophy resulting in unilateral lagophthalmus, and ptosis, which have not been documented before. This case is not only unique in its presentation but also emphasizes the im…

Update on the pharmacological treatment of adult myositis.

The management of patients with idiopathic inflammatory myopathy (IIM) remains a challenge given the systemic features beyond active myositis. That is, recognizing the inflammatory arthropathy, varying dermatomyositis rashes, and overt and occult features of interstitial lung disease in addition to myositis adds to the complexity of diagnosis and treatment of IIM. However, clinicians now have available many more immunosuppressive drugs as well as biologic agents for use in patients with myositis and other autoimmune diseases. Here, the use of these agents is reviewed and support based on avail…

[Myositis and the skin: cutaneous manifestations of dermatomyositis].

Idiopathic inflammatory myopathies include dermatomyositis, polymyositis, and inclusion body myositis. Among them, cutaneous manifestations are observed most frequently in dermatomyositis. While dermatomyositis commonly affects the skin and muscles, it can also affect the lungs and other organs. Dermatomyositis presenting clinically and histopathologically with typical cutaneous lesions, but without myositis, is called amyopathic dermatomyositis. Given that the Bohan and Peter criteria cannot distinguish amyopathic dermatomyositis, understanding the characteristic skin manifestations may be es…

Myositis ossificans traumatica of the masticatory muscles.

Myositis ossificans traumatica (MOT) is a disease in which muscular ossification develops following trauma. Almost all cases of MOT are found in skeletal muscle. The authors report in a 39-year-old man MOT involving several muscles in the head and neck, namely, bilateral masseter muscles, the left temporal muscle, the left lateral pterygoid muscle, and the left frontal muscle. Involvement of the lateral pterygoid muscle is especially rare.

Myositis as an unusual presentation of secondary syphilis.

Myositis is a complication of many infectious and noninfectious processes. Secondary syphilis is an uncommon cause of myositis, and, therefore, the diagnosis may be delayed. We describe a man with human immunodeficiency virus infection presenting with diffuse muscle injury secondary to syphilis. He had complete resolution of all signs and symptoms after treatment with intravenous penicillin.

Focal myositis of the tongue presenting as macroglossia.

Focal myositis (FM) is a benign localized inflammatory process of unknown aetiology, which presents as a rapidly enlarging soft-tissue mass that evolves into a localized lesion. It is most often mistaken for a neoplasm. Histologically, it is characterized by marked myopathic changes of the muscle fibres, lymphocytic infiltration and interstitial fibrosis. We describe a case of focal myositis of the tongue and review this rare condition.

Myositis ossificans traumatica of the thenar region.

Myositis ossificans of the hand is extremely rare. We report an unusual case of myositis ossificans of the thenar muscles and discuss the diagnostic pitfalls whereby this condition can be mistaken for malignancy or infection.
