Extended-release naltrexone for alcohol and opioid dependence: a meta-analysis of healthcare utilization studies.

Through improved adherence, once-monthly injectable extended-release naltrexone (XR-NTX) may provide an advantage over other oral agents approved for alcohol and opioid dependence treatment. The objective of this study was to evaluate cost and utilization outcomes between XR-NTX and other pharmacotherapies for treatment of alcohol and opioid dependence. Published studies were identified through comprehensive search of two electronic databases. Studies were included if they compared XR-NTX to other approved medicines and reported economic and healthcare utilization outcomes in patients with opi…

Ocular surface abnormalities related to type 2 diabetes are reversed by the opioid antagonist naltrexone.

BACKGROUND: Ocular surface complications of type 2 diabetes are associated with reductions in tear production, increased corneal surface sensitivity, and delayed corneal re-epithelialization. This study examined the efficacy of topical application of the opioid antagonist naltrexone (NTX) in reversing these diabetic-related ocular surface complications in mice. METHODS: The genetic db/db mouse model of type 2 diabetes, along with C57Bl/6 wild-type mice were investigated. Tear production was assessed by phenol red impregnated threads, and ocular surface sensitivity was measured using Von Frey …

Naltrexone in the treatment of opioid-dependent pregnant women: the case for a considered and measured approach to research.

The present paper considers naltrexone to treat opioid dependence during pregnancy. The public health problem of opioid dependence and its treatment during pregnancy is reviewed first. Next, the naltrexone and opioid dependence treatment literature is summarized, with overviews of the pre-clinical and clinical research on prenatal naltrexone exposure. Finally, considerations and recommendations for future medication research for the treatment of opioid dependence in pregnant women are provided. The efficacy of long-acting injectable naltrexone relative to placebo, its blockade of opioid agonis…

Acceptance of naltrexone by pregnant women enrolled in comprehensive drug addiction treatment: an initial survey.

This paper reports the results of an initial survey regarding the potential interest in naltrexone treatment by pregnant women enrolled in comprehensive treatment for substance use disorders. Pregnant women (N = 112) were asked about their interest in taking either an oral or long-acting injectable medication that would stop heroin and/or alcohol use, leave them "clear-headed," and without neonatal withdrawal. Results indicate strong interest among pregnant women in antagonist treatment, in either form, with clear interest in learning more about naltrexone. Findings lend support for patients' …

Effects of amylin and bupropion/naltrexone on food intake and body weight are interactive in rodent models.

Antagonism of opioid systems (e.g., with naltrexone) has been explored as an anti-obesity strategy, and is particularly effective when co-administered with dual inhibitors of dopamine and norepinephrine reuptake (e.g., bupropion). Previously, we demonstrated that amylin enhances the food intake lowering and weight loss effects of neurohormonal (e.g., leptin, cholecystokinin, melanocortins) and small molecule (e.g., phentermine, sibutramine) agents. Here, we sought to characterize the interaction of amylin with naltrexone/bupropion on energy balance. Wild-type and amylin knockout mice were simi…

Early adoption of injectable naltrexone for alcohol-use disorders: findings in the private-treatment sector.

OBJECTIVE: The U.S. substance-abuse treatment system has been slow to adopt medications for the treatment of alcohol-use disorders (AUDs). The objectives of this study are to (a) determine how the inherent characteristics of injectable naltrexone (i.e., relative advantage, complexity, trialability, observability, compatibility) shape organizational-level decisions to adopt the medication and (b) identify key predictors of adoption and barriers that impede adoption. METHOD: This study uses data from a nationally representative sample of 345 privately funded U.S. substance-abuse treatment progr…

Improved parameters of metabolic glycaemic and immune function and arterial stiffness with naltrexone implant therapy.

Here the dramatic and rapid response of a 54-year-old obese hypertensive man with poorly controlled insulin-dependent diabetes with a 33 year history of high dose heroin use, a 1 year history of refractory ulceration of his hands, ankles and feet, treated coronary artery disease, and the metabolic syndrome, to implantation with long-acting naltrexone implants is presented. In particular his hyperlipidaemia, hyperglycaemia, proinflammatory state, evidence of hepatic and renal insufficiency, arterial stiffness, and extensive and chronic cutaneous ulceration all improved dramatically over just 13…

The effects of maternally administered methadone, buprenorphine and naltrexone on offspring: review of human and animal data.

Most women using heroin are of reproductive age with major risks for their infants. We review clinical and experimental data on fetal, neonatal and postnatal complications associated with methadone, the current "gold standard", and compare these with more recent, but limited, data on developmental effects of buprenorphine, and naltrexone. Methadone is a micro-opioid receptor agonist and is commonly recommended for treatment of opioid dependence during pregnancy. However, it has undesired outcomes including neonatal abstinence syndrome (NAS). Animal studies also indicate detrimental effects on …

High and low dosage oxcarbazepine versus naltrexone for the prevention of relapse in alcohol-dependent patients.

INTRODUCTION: Oxcarbazepine (OXC) reduces high-voltage-activated calcium currents, thus reducing glutamatergic transmission at corticostriatal synapses. This effect on NMDA glutamatergic transmission may play a role against the increased glutamatergic transmission determined by alcohol withdrawal. To investigate the efficacy and safety of OXC in relapse prevention we compared OXC at different dosages with Naltrexone (NAL) in a 90 days randomised open-label trial. Craving and psychiatric symptoms improvements were the secondary endpoints. METHODS: Eighty-four detoxified alcohol dependent subje…

Discriminative stimulus effects of acute morphine followed by naltrexone in the squirrel monkey: a further characterization.

The discriminative stimulus effects of acute morphine followed by naltrexone have been described previously in nonhuman primates. The purposes of this study were to 1) extend the pharmacological characterization of the discrimination by testing mu-opioid agonists other than morphine and opioid-like compounds other than naltrexone and 2) to examine further the relationship between agonist pretreatment time and manifestation of the cue produced by morphine followed by naltrexone. Subjects were trained to discriminate 1.7 mg/kg morphine --> 0.1 mg/kg naltrexone (MOR --> NTX) versus saline followe…

Cocaine and amphetamine attenuate the discriminative stimulus effects of naltrexone in opioid-dependent rhesus monkeys.

This study tested the hypothesis that stimulants (indirect dopamine agonists) attenuate the discriminative stimulus of naltrexone in monkeys chronically treated with L-alpha-acetylmethadol (LAAM). Four rhesus monkeys (Macaca mulatta) received LAAM (1.0 mg/kg s.c.) twice daily and discriminated a withdrawal-precipitating dose of naltrexone (0.0178 mg/kg s.c.) from saline. Cocaine (0.1-1.78 mg/kg), amphetamine (0.32-1.78 mg/kg), haloperidol (0.01-0.1 mg/kg), sulpiride (1.0-10.0 mg/kg), propranolol (0.32-3.2 mg/kg), clonidine (0.001-0.1 mg/kg), desipramine (0.32-3.2 mg/kg), and imipramine (1.0-10…

Naltrexone and lofexidine combination treatment compared with conventional lofexidine treatment for in-patient opiate detoxification.

This study compares a naltrexone/lofexidine combination treatment with a 7-day course of lofexidine alone in the treatment of opiate withdrawal in 22 opiate-dependent patients. Withdrawal symptoms were significantly less severe on days 4-7, and 9-13, in the naltrexone/lofexidine combination group. There were no significant differences in the percentage of patients completing detoxification or in the mean length of stay for the two groups. Both treatments had similar, minimal effects on blood pressure. The naltrexone/lofexidine combination was associated with a more rapid resolution of the opia…

Therapeutic effect of fluoxetine on naltrexone-resistant self-injurious behavior in an adolescent with mental retardation.

ABSTRACT A significant and sustained reduction in self-injurious behavior (SIB) was observed in a 17-year-old male with severe to profound mental retardation who was treated in an open clinical trial with fluoxetine. The 45-55% reduction in SIB was accompanied by improvement in affective stability, motor capacities, and social activity. No adverse effects or diminishing therapeutic effects were observed during 2 years of treatment at 40 mg daily. Twelve weeks before the start of the fluoxetine trial, this adolescent had been found to be unresponsive to the opiate receptor blocker naltrexone in…

Prophylactic oral naltrexone with intrathecal morphine for cesarean section: effects on adverse reactions and analgesia.

The influence of two different doses of oral naltrexone on the adverse effects and the analgesia associated with intrathecal morphine was compared in a double-blind, placebo-controlled study. Thirty-five patients undergoing cesarean section were provided postoperative analgesia by 0.25 mg intrathecal morphine. Sixty minutes later they were given 6 mg naltrexone, 3 mg naltrexone, or placebo as an oral solution. Pain relief was assessed by the Visual Analog Scale. Requirements for additional analgesics and side effects were recorded. Duration of analgesia was shorter in the 3- and 6-mg naltrexon…

Clonidine and naltrexone. A safe, effective, and rapid treatment of abrupt withdrawal from methadone therapy.

Clonidine hydrochloride and naltrexone hydrochloride, given in combination, were found to provide a safe, effective, and extremely rapid treatment of abrupt withdrawal from methadone hydrochloride therapy. Under controlled inpatient conditions established to assess dosage guidelines and to examine specific signs and symptoms of withdrawal, ten (91%) of 11 patients were able to withdraw completely from methadone therapy by the end of a six-day period. Six days of clonidine hydrochloride treatment, with a peak mean dose of 2.9 mg/day on treatment day 2, attenuated the withdrawal-inducing effects…

A comparative study of the oral, intravenous, and subcutaneous administration of 3H-naltrexone to normal male volunteers.

3H-naltrexone was administered orally, intravenously, and subcutaneously to groups of normal, male, paid volunteers. The doses given were: 50 mg orally (specific activity 4 microCi/mg), 1 mg intravenously (specific activity 200 microCi/mg), and 5 mg subcutaneously (specific activity 30 microCi/mg). At these doses, the subjects did not experience any noticeable effects. Following intravenous injection, plasma levels of radioactivity were immediately high and declined rapidly during the first 30 minutes and declined gradually thereafter. Following oral or subcutaneous administration, maximal pla…

Bridging the Gap: Empowering Pharmacists in Inpatient Injectable Naltrexone and Hospital-Dispensed Methadone Process Development.

Substance use disorders (SUD) affect 17% of the United States population, and over 25% of U.S. hospitalizations are related to drug and alcohol use. Despite strong evidence and guideline recommendations, medications for SUD remain underutilized due to social stigma and institutional, legislative, and financial barriers. To address underutilization in one academic hospital, pharmacy-led processes were developed for hospital-administered intramuscular naltrexone (IM-NTX) and hospital-dispensed methadone at discharge. Workflows relied on pharmacy leadership and clinical pharmacist champions. Key …

The opioid antagonist naltrexone decreases seizure-like activity in genetic and chemically induced epilepsy models.

OBJECTIVE: A significant number of epileptic patients fail to respond to available anticonvulsive medications. To find new anticonvulsive medications, we evaluated FDA-approved drugs not known to be anticonvulsants. Using zebrafish larvae as an initial model system, we found that the opioid antagonist naltrexone exhibited an anticonvulsant effect. We validated this effect in three other epilepsy models and present naltrexone as a promising anticonvulsive candidate. METHODS: Candidate anticonvulsant drugs, determined by our prior transcriptomics analysis of hippocampal tissue, were evaluated i…
