Two types of AVN occur in childhood. The first is the idiopathic type best characterized by Legg-Calvé-Perthes' disease associated with repeated infarctions that slow the healing process. The second is the traumatic type caused by a direct insult to the blood supply of the developing femoral head. Both can result in profound growth disturbances of the proximal femur and early degenerative changes. They may have little effect on hip function or appearance. A case of haemolytic uraemic syndrome in a three year old boy is described. The clinical course was complicated by hyperglycaemia, and biochemical assay of plasma showed an almost total lack of pancreatic insulin. Extensive necrosis of the exocrine and endocrine pancreas was found at necropsy. The possible pathological physiology of this complication and its prognostic importance are discussed.

A 76-year-old woman with classical rheumatoid arthritis presented with a left groin mass associated with radiologic erosion of the adjacent bony public ramus. The hip joint was not involved. Computerized tomography showed an encapsulated cyst of the pubic ramus that was surgically excised and measured about 6 x 8 cm. Histologic examination of the cystic mass and adjacent bone showed marked avascular necrosis, surrounded by an acute necrotizing vasculitis with fibrinoid necrosis. We postulate that, in this case, rheumatoid vasculitis caused an avascular necrosis of the pubic ramus.

Sudden death in cattle, especially calves, is a common problem of farm practice. Some cases are associated with significant cardiac lesions; in others, although necropsy suggests cardiac failure to be the cause of death, histological examination of H & E sections fails to demonstrate conclusively the site and nature of the lesion. Two elderly women presented with ischaemic necrosis of the tongue. In both there were typical features of cranial arteritis. The importance of early recognition is emphasised so that appropriate therapy may be prescribed to minimise further complications.

The clinical course is reported in 17 patients in whom the histological picture of subacute hepatic necrosis ("bridging hepatitis") was found on needle liver biopsy or at autopsy. The patients' ages ranged from 10-71 years, 12 patients being less than 40 years old. Ten patients were males. Jaundice lasted 2-4 months in nine cases and over six months in two, one of the latter having developed cirrhosis. In five patients a relapse of jaundice occurred within three months. A 31-year-old male had a sudden onset of painful swelling in the penile root region. He then developed necrosis of the penis and further a progressive, chemotherapy-resistant ulcer which involved the perineal region also. A man aged 59 years developed a rapidly progressive muscular weakness which within a few weeks involved almost the entire musculature. A carcinoma of the stomach was discovered at necropsy. The skeletal muscles showed widespread necrosis with minimal inflammatory reaction. The findings are compared with those in a woman of 51 with Hashimoto's thyroiditis who developed a slowly progressive weakness of her trunk muscles which after some months began to involve the limbs.
