Role of mannose-binding lectin in nosocomial sepsis in critically ill neonates.

We investigated the association of mannose-binding lectin (MBL) serum levels with nosocomial sepsis (NS), their changes overtime during infection, their relation with pathogens, with the MBL2 genotype and their relationship with mortality. In a prospective observational study, we included 365 critically ill neonates: 261 had no infection and 104 had at least 1 septic event. The median MBL serum concentration was significantly lower in infected than in noninfected neonates (p < 0.001). Low MBL levels on admission increased the risk of infection, independently from gestational age and invasive p

Amebic liver abscesses among neonates can mimic bacterial sepsis.

Amebic liver abscesses are unusual among neonates. Three (3.6%) of 83 children who presented to a university hospital with amebic liver abscesses in a 17-year period were neonates (<28 days of age). Patients presented with nonspecific clinical and laboratory findings mimicking fulminant neonatal sepsis. Diagnosis was delayed and was based on the presence of a hypoechoic lesion in the liver, antibodies against Entamoeba histolytica and microbiologic examination of pus. In addition to parenteral metronidazole treatment and early ultrasound-guided aspiration of the abscess, intensive care is vita…

Myeloid hematopoietic growth factors and their role in prevention and/or treatment of neonatal sepsis.

Sepsis continues to be an important cause of morbidity and mortality among both full-term and preterm infants, secondary to an immaturity in neonatal host defense. The incidence of neonatal sepsis ranges from 30% in very low birth weight infants to 0.4% in preterm neonates and 0.1% in term neonates. The dysregulation of the expression and production of hematopoietic cytokines in the neonate contributes to quantitative and qualitative deficiencies in neonatal myeloid progenitor activity and decreased availability and function of mature effector neutrophils. These abnormalities contribute in lar…

We conclude that selective muscle proteolysis contributes to nitrogen loss in neonatal sepsis. Although proteolysis abates by 48 hr, short-term injury results in significant muscle-mass deficit.

Postnatal penicillin prophylaxis and the incidence of group B streptococcal sepsis in neonates.

We conducted a retrospective chart review of infants, born over a 3-year period, who had positive urine latex agglutination and/or positive blood culture for group B streptococci (GBS). Infants routinely received intramuscular aqueous penicillin for the first half of the study period, and no penicillin was given for the subsequent 18 months. Overall, infants who received penicillin prophylaxis had a decreased incidence of clinical sepsis and positive blood culture for GBS (4.8/1,000 versus 8/1,000 and 1.3/1,000 versus 5.4/1,000, respectively). The incidence of GBS sepsis during the time of pen

Buffy coat transfusions in neonates with sepsis and neutrophil storage pool depletion.

A randomized study was initiated in neonates with neutropenia (absolute peripheral neutrophil count less than 1,500/microL) and suspected bacterial infection. Twenty infants with proven infection were enrolled, nine of whom had depletion of bone marrow stores of maturing neutrophils (less than or equal to 7% metamyelocyte, band and mature forms per 100 nucleated cells). These nine were randomized to receive 15 mL/kg of either buffy coat transfusions (group 2) or plasma and blood products (group 3). The remaining 11 (group 1) were observed. Peripheral neutrophil counts were monitored to determi

Intravenous immunoglobulin in the treatment of neonatal sepsis: therapeutic strategies and laboratory studies.

IGIV appears to have a promising future in treating and perhaps preventing neonatal bacterial infections. However, all IGIV lots do not contain equal amounts of pathogen-specific IgG with functional opsonic activity. To ensure effective therapy it will be important to inform physicians that the IGIV lots available for use contain functional antibacterial antibody to the responsible pathogens. To optimize therapy IGIV may need to be given early in the infection and doses may need to be repeated if pathogen-specific antibody is rapidly depleted. Further, clinical studies will be necessary to det

Analysis of eight cases of neonatal meningitis and sepsis due to Enterobacter sakazakii.

Eight cases of neonatal meningitis due to Enterobacter sakazakii (formerly known as yellow-pigmented Enterobacter cloacae) which occurred in The Netherlands during the last 6 years were investigated retrospectively. Two patients had necrotizing enterocolitis and meningitis simultaneously. Despite treatment (in most cases with ampicillin and gentamicin), the fatality rate was 75%. Strains were much more susceptible to some of the new beta-lactam antibiotics than to ampicillin. A mode of transmission other than passage through the birth canal was likely, at least for some patients. A cluster of

Neonatal sepsis. A survey of eight year's experience at the Louisville General Hospital.

Information from 50 infants with neonatal septicemia from the Louisville General Hospital during an eight-year period (1964-1972) is presented. Twenty-five infants had gram-positive and the other 25 had gram-negative organisms. E. coli (13 cases), staphylococcus (10 cases), and hemolytic Streptococcus non-Group A (7 cases) were the mustcommon causative microorganisms. Only one of the 25 infants with gram-positive sepsis died; three with gram-negative sepsis died. Listeria monocytogenes was demonstrated in three infants; all had meningitis with no mortality. Early diagnosis, prompt intensive an

Comparative Evaluation of Antimicrobial Use in Neonates at Risk for Early-onset Neonatal Sepsis Using Different Diagnostic Strategies in a Neonatal Care Unit.

BACKGROUND: Early-onset disease (EOD) remains one of the most common causes for initiating antibiotics. In response, diagnostic approaches such as the guidelines by the National Institute for Health and Care Excellence (NICE) and the American Academy of Pediatrics (AAP) have been proposed. The Kaiser-Permanente model by the AAP estimates the probability of EOD per 1000 births in neonates born at ≥34 weeks' gestation. This study aims to evaluate the antibiotic use in a neonatal department for suspected EOD cases and to compare the existing practice-a combination of NICE guidelines and individua

Hormonal, biochemical and hematological changes in neonatal calves with sepsis induced by enteritis.

BACKGROUND: Diarrhea is one of the most important problems seen in calves during the neonatal period. Sepsis caused by diarrhea is the main cause of death. In the case of sepsis, many hormonal, hematological and biochemical changes occur in calves. It is extremely important to evaluate these parameters together. AIMS: The present study aimed to investigate hormonal, biochemical, and hematological changes in neonatal calves affected by septic enteritis to obtain more information about the pathogenesis of sepsis. METHODS: The study included a sepsis group consisting of 40 calves aged 0-15 days

Upregulation of miR-223 by SIRT1 attenuates the inflammatory response in neonatal sepsis.

BACKGROUND: The diversity of clinical symptoms of neonatal sepsis leads to difficulties in diagnosis, and the treatment is limited. This study aims to investigate the role of the SIRT1/miR-223 signaling axis in neonatal sepsis. METHODS: 100 neonates with sepsis and 100 healthy infants were included in this study. The RT-qPCR was used to detect RNA and inflammatory factor levels in serum. Peripheral blood neutrophils were isolated by gradient centrifugation. LPS-stimulated neutrophils were detected for MPO activity and inflammatory factor expression. Neutrophils were transfected with miR-223 o…

Melatonin as a Supportive Biomarker in Early-Onset Neonatal Sepsis: A Prospective Study.

BACKGROUND: Early-onset neonatal sepsis (EOS) remains a diagnostic challenge because clinical signs are nonspecific and blood cultures, although the diagnostic gold standard may yield false-negative results due to low blood volumes or maternal antibiotic exposure. PURPOSE: To determine whether serum melatonin levels differ between term neonates with clinically suspected EOS and healthy controls, and to evaluate the diagnostic performance of melatonin relative to standard inflammatory markers. METHODS: This prospective case-control study included 84 term infants: 42 with clinically suspected …
