Residual neuromuscular block in the postanesthesia care unit: incidence, risk factors, and effect of neuromuscular monitoring and reversal agents.

BACKGROUND: The aim of this study was to determine the incidence of residual neuromuscular block (RNMB) in a tertiary care hospital. Seconda … . RNMB was observed in 47 patients (21.8%). Seventy-eight patients (36%) were followed up with NMM. Neostigmine and sugammadex were used in 174 (80.5%) and 42 (19.5%) patients, respectively, and they were both underdosed (21.2 ± 3.0 mcg/kg and 1.5 ± 0.7 mg/kg, respectively). Use of neostigmine and absence of NMM were risk factors for RNMB (p: 0.01 and 0.001, …

Both the clinical response to the administration of neostigmine and the repetitive nerve stimulation test were positive. The motor evoked potentials at lower limb recordings were normal. Pentobarbital decreased nitric oxide release in the rat striatum but ketamine increased the release independent of cholinergic regulation.

The rats were freely moving, and PB and Ket were administered intraperitoneally. Neostigmine (1 and 10 µM) and mecamylamine (100 µM) were added to the perfusate. Calcium and magnesium concentrations were modified for each anesthetic to influence ACh release. PB decreased NO products (NOx) while Ket increased them. While perfusion with neost …

Train-of-four recovery after pharmacologic antagonism of pancuronium-, pipecuronium-, and doxacurium-induced neuromuscular block in anaesthetized humans.

Previous studies have suggested that the increased duration of action of long-acting neuromuscular relaxants may make their pharmacologic an … rst supramaximal twitch (T1) in a train-of-four (TOF). At this time, the block was antagonized with neostigmine 0.07 mg/kg and glycopyrrolate 0.014 mg/kg i.v., and recovery of TOF was recorded. Spontaneous recovery to 25% of the baseline T1 response occurred at 52 +/- 14 min (mean +/- SD) following administration of either pancuronium and pipecuronium, and 85 …

Enhancement by KW-5092, a novel gastroprokinetic agent, of the gastrointestinal motor activity in dogs.

KW-5092 ([1-[2-[[[5-(piperidinomethyl)-2- furanyl]methyl]amino]ethyl]-2-imidazolidinylidene) propanedinitrile fumarate) is a novel gastroprokinetic agent with acetylcholinesterase (AChE) inhibitory activity and acetylcholine (ACh) release facilitatory activity. The present study examined the effects of KW-5092 on gastrointestinal (GI) motor activity in dogs. In anesthetized dogs, KW-5092 at 0.03 to 1 mg/kg, i.v. dose-dependently enhanced the gastric antral and the colonic motor activity. Neostigmine, an AChE inhibitor, enhanced the motor activity at 0.03 and 0.1 mg/kg, i.v. Ranitidine, a hista…

Characterization of acetylcholinesterase in rabbit intrapulmonary arteries.

Substance P-induced contraction of rabbit airways: mechanism of action.

Aminoglycoside antibiotics: interaction with trimethaphan at the neuromuscular junctions.

Trimethaphan, a ganglionic blocking agent which is administered by intravenous drip to produce controlled hypotension during surgery, produces a complete neuromuscular blockade at the isolated phrenic nerve-hemidiaphragm preparation of the rat at a concentration of 0.3 mmol X l-1. This blockade is not reversed by neostigmine, a cholinesterase inhibitor, nor by calcium chloride, and this action is attributed to the local anaesthetic activity of the drug. Trimethaphan (1.5 X 10(-2) mmol X l-1) interacts with the following aminoglycoside antibiotics: gentamicin (0.04), streptomycin (0.05), netilm…

Acetylcholinesterase inhibitors reduce brain and blood interleukin-1beta production.

Overproduction of interleukin-1 within the brain is associated with Alzheimer's disease and other neurological conditions. We report that peripheral administration of the acetylcholinesterase inhibitors tacrine, rivastigmine, neostigmine, or EN101 (an antisense oligonucleotide directed at acetylcholinesterase messenger RNA) to mice significantly attenuated the production of interleukin-1beta in the hippocampus and blood, concomitantly with the reduction in acetylcholinesterase activity. These findings demonstrate that cholinergic enhancement produces central and peripheral antiinflammatory eff…

Narcolepsy in seven dogs.

Narcolepsy in 7 dogs was tentatively diagnosed on the onset of cataplexy, before or during young adulthood. Confirmatory polygraphic sleep recordings were done in 3 of the dogs. In 2 dogs, treatment with neostigmine did not cause the signs to disappear, thus ruling out myasthenia gravis; trials with imipramine reduced catapletic attacks. Data from case histories, polygraphic recordings, drug trials, and clinical tests were used to compare and contrast the disease in man and in the dog.

One Bite, Two Severe Envenomations Despite Antivenom Therapy: A Case Report.

This case report describes a 12-year-old child who was bitten by a cobra and developed two distinct systemic neurotoxic episodes, 19 h apart, along with severe local envenoming, despite receiving appropriate treatment with antivenom and making a full recovery. It also highlights the use of neostigmine for severe symptoms and emphasizes the importance of prompt wound care to prevent long-term complications. A potentially life- and limb-threatening event was successfully avoided through an inter-departmental approach.

Pro: The Use of Sugammadex Does Not Preclude the Need for Objective Neuromuscular Monitoring.

Based on strong evidence, optimal management of neuromuscular block should incorporate objective (or quantitative) neuromuscular monitoring with appropriately dosed antagonists such as sugammadex. Sugammadex has the unique ability to antagonize aminosteroidal-induced neuromuscular block at any level; however, neostigmine is a reasonable alternative at minimal levels. Blind or excessive administration of sugammadex does not eliminate the risk of patients having postoperative residual neuromuscular block. Significant variability exists in how patients respond both to neuromuscular blocking agent…

PURPOSE OF REVIEW: This review describes recent prospective and retrospective work exploring the incidence and clinical consequence of sugam … agents. There are several case reports that describe marked bradycardia with cardiac arrest from reversal with sugammadex. The incidence of this type of reaction to sugammadex appears to be very rare.
