Does sleep matter before surgery? The impact of preoperative sleep quality on pain and analgesic requirement after percutaneous nephrolithotomy.

PURPOSE: Postoperative pain is a significant clinical issue that delays recovery and increases the risk of complications. Sleep quality is a modifiable factor that can influence the perception of postoperative pain. This study aimed to evaluate the association between preoperative sleep quality and postoperative pain scores, analgesic consumption, and sedation demand in patients undergoing percutaneous nephrolithotomy (PCNL). METHODS: In this prospective observational study, 92 adult patients scheduled for elective PCNL were enrolled. In the preoperative period, the Pittsburgh Sleep Quality I

Continuous Erector Spinae Plane Blocks to Treat Pain after Percutaneous Nephrolithotomy: A Randomized, Triple-masked, Placebo-controlled Clinical Trial.

BACKGROUND: Single-injection erector spinae plane blocks (ESPB) provide analgesia after percutaneous nephrolithotomy (PCNL) but are limited in their duration. A continuous ESPB involving local anesthetic administered via a perineural catheter may extend analgesia duration. This study hypothesized that adding a continuous ESPB to a single-injection ESPB after outpatient PCNL would decrease pain severity and/or opioid consumption over the first 2 postoperative days (dual primary outcomes). METHODS: Preoperatively, adults undergoing PCNL had an ultrasound-guided perineural catheter inserted at t

B.T.C.H. nephrolithometry score: a novel scoring system to predict stone-free rate and complexity for ultrasound-guided percutaneous nephrolithotomy.

BACKGROUND: The existing scoring systems for percutaneous nephrolithotomy fail to adequately consider the influence of renal anatomy, leading to limited predictive accuracy. This study introduces and validates a novel B.T.C.H. nephrolithometry score, designed to better predict stone-free rates and complexity for ultrasound-guided percutaneous nephrolithotomy. METHODS: B.T.C.H. nephrolithometry score evaluates four variables including stone burden, type of renal pelvis, calyces involved, and hydronephrosis. 134 patients who underwent ultrasound-guided percutaneous nephrolithotomy at Beijing Ts

More energy, same results: high energy settings do not improve the efficiency of dual energy lithotripsy during percutaneous nephrolithotomy.

INTRODUCTION AND OBJECTIVE: No prior in vivo clinical trial has investigated the effect of varying energy settings on the performance of a dual-energy lithotripsy. We prospectively evaluated various impact and frequency settings during PCNL procedures to achieve optimal fragmentation and extraction of large stones of varying density. METHODS: Four impact and frequency settings were chosen based on a prior in vitro study and applied sequentially for 1 min each in a randomized manner during PCNL for large renal calculi greater than 3 cm in 24 patients: 4 Hz + 100% impact, 4 Hz + 30% impact, 12

Nomogram for predicting risk factors of postoperative residual stones in patients after pure ultrasonography-guided access mini-percutaneous nephrolithotomy.

BACKGROUND: Some urologists prefer to perform PCNL by using pure ultrasonography-guided access (USGA), but some potential factors that may cause the residual stones need to be excavated. This retrospective case-control study is designed to confirm the risk factors for residual stones after pure USGA mini-percutaneous nephrolithotomy (mPCNL) in patients and develop a nomogram for predicting postoperative residual stones based on the risk factors. METHODS: From October 2019 to August 2021, our department recorded a retrospective record of 227 patients with kidney stones treated with pure USGA m

An interpretable machine learning model integrating computed tomography radiomics and clinical features for predicting the urosepsis after percutaneous nephrolithotomy.

BACKGROUND: The urosepsis after percutaneous nephrolithotomy (PCNL) is a critical health risk necessitating prompt medical identification and intervention. Nevertheless, a deficiency exists in the availability of a tool for precise and timely predictive analysis. The purpose is to establish a machine learning (ML) model using radiomic features and clinical data to predict urosepsis following PCNL. METHOD: This study retrospectively included 401 patients with kidney stones from two centers who underwent PCNL. To enhance the dataset's equilibrium, the synthetic minority over-sampling technique
