Altered sensory nerve excitability in fibromyalgia.

BACKGROUND/PURPOSE: To investigate nerve excitability changes in patients with fibromyalgia and the correlation with clinical severity. METHODS: We enrolled 20 subjects with fibromyalgia and 22 sex and age-matched healthy subjects to receive nerve excitability test and nerve conduction study to evaluate the peripheral axonal function. RESULTS: In the fibromyalgia cohort, the sensory axonal excitability test revealed increased superexcitability (%) (P = 0.029) compared to healthy control. Correlational study showed a negative correlation between increased subexcitability (%) (r = -0.534, P =

Exploring the Relationship Between Objective Pupillometry Metrics and Midline Shift.

BACKGROUND: Pupillary examinations provide early subtle signs of worsening intracranial pathology. Objective pupillomtery assessment, although not yet the standard of care, is considered best practice. However, inconsistent findings from objective pupillometry studies have caused a lack of consensus among clinicians; as such, no clinical guidelines are available to guide clinical use of objective pupillometer devices. To add to the body of evidence, the purpose of this project was to explore the relationship between objective pupillometry metrics and midline shift (MLS). METHODS: A retrospecti

[Diagnostics of polyneuropathies].

The diagnosis of polyneuropathy (PNP) is based on the anamnesis and description of complaints of the patient and clinical findings. The type of distribution as well as known diseases and drug toxic factors can provide indications. Electromyography and electroneurography can be used to differentiate between axonal and demyelinating PNP. The laboratory examinations are initially directed towards frequent and treatable causes. These are then expanded depending on the suspected diagnosis. Analysis of cerebrospinal fluid (CSF) is facultative and should be carried out when there is a suspicion of a

Absent sural responses in tethered cord syndrome.

Context: Tethered cord syndrome is a progressive condition usually diagnosed early in life, and leads to a diversity of manifestations including neurologic, urologic, and orthopedic dysfunction.Findings: We report two cases of tethered cord syndrome associated with a unilaterally absent sural sensory response on nerve conduction studies while other causes of this finding being excluded.Conclusion/clinical relevance: We believe that this finding is caused by a selective injury of sensory fibers at the level or distal to the dorsal root ganglia.

Accuracy of the Scratch Collapse Test for Carpal Tunnel Syndrome in Comparison With Electrodiagnostic Studies.

Background: The scratch collapse test (SCT) is a clinical examination maneuver that has been previously reported as a reliable and reproducible test to diagnose carpal tunnel syndrome (CTS). The initial study by Cheng et al in 2008 showed a simple test with high sensitivity. However, subsequent attempts to reproduce those findings have resulted in lower accuracy. Our goal was to evaluate the use of the SCT for patients presenting with symptoms of pain, numbness, or weakness in an upper extremity. Methods: Forty patients were referred to the electrodiagnostic (EDX) lab for evaluation of an uppe

Does This Patient Have Parkinson Disease or Essential Tremor?

In the elderly patient with tremor, the differential diagnosis is usually between essential tremor (ET) and Parkinson disease (PD). A careful history and examination are the keys to the diagnosis. Essential tremor is a bilateral action tremor of the upper limbs whereas PD begins unilaterally and is a rest tremor. A handwriting sample can usually distinguish PD from ET as the former is small (micrographic) but atremulous whereas writing in ET is tremulous but normal sized. In ET, there are no signs aside from tremor but in PD, the tremor is accompanied by bradykinesia and rigidity.

Important Details in Performing and Interpreting the Scratch Collapse Test.

UNLABELLED: The utility of the scratch collapse test has been demonstrated in examination of patients with carpal and cubital tunnel syndromes and long thoracic and peroneal nerve compressions. In the authors' clinic, this lesser known test plays a key role in peripheral nerve examination where localization of the nerve irritation or injury is not fully understood. Test utility and accuracy in patients with more challenging presentations likely correlate with tester understanding and experience. This article offers a clear outline of all stages of the test to improve interrater reliability. Th

Hemiplegia and headache: a review of hemiplegia in headache disorders.

The most common scenario wherein the practicing neurologist is likely to encounter a patient with headache and hemiplegia will vary depending on his/her specific type of practice. A neurologist providing consultative service to an emergency department is far more likely to see patients with "secondary" headache and hemiplegia in the setting of either ischemic or hemorrhagic stroke than hemiplegia as a transient feature of a primary headache disorder. Neurologists subspecializing in headache medicine who practice in a tertiary referral headache clinic are more likely to encounter hemiplegic mig

Diagnostic Accuracy of the Slump Test for Identifying Neuropathic Pain in the Lower Limb.

STUDY DESIGN: Diagnostic accuracy study with nonconsecutive enrollment. OBJECTIVES: To assess the diagnostic accuracy of the slump test for neuropathic pain (NeP) in those with low to moderate levels of chronic low back pain (LBP), and to determine whether accuracy of the slump test improves by adding anatomical or qualitative pain descriptors. BACKGROUND: Neuropathic pain has been linked with poor outcomes, likely due to inadequate diagnosis, which precludes treatment specific for NeP. Current diagnostic approaches are time consuming or lack accuracy. METHODS: A convenience sample of 21 in

Progressive supranuclear palsy.

Progressive supranuclear palsy is a disorder of tau protein aggregation. Its clinical spectrum is now known to be wider than originally described, with a phenotype resembling Parkinson disease accounting for a third of cases. However, at least half of the patients with PSP exhibit the classic bradykinesia with disproportionate postural instability, erect posture with nuchal rigidity, frontal behavioral and cognitive changes, vertical gaze palsy, and other disabling brainstem deficits. Nonmendelian genetic risk factors exist, but PSP is almost entirely sporadic, with a prevalence of five to six

Role of electrodiagnosis in patients being considered for epidural steroid injections.

Electrodiagnostic (EDX) evaluation, and, more specifically, electromyography (EMG), has a well-established role in the diagnostic confirmation of cervical and lumbar radiculopathy. The role of EMG in prognostication, however, is less established and remains a topic of debate. The purpose of this article is to evaluate the diagnostic and prognosticating role of EMG in cervical and lumbar radiculopathies, and to assess the utility of EMG in predicting outcomes after epidural steroid injections (ESI). A review of the existing literature strongly supports a prognosticating role for EDX in predicti

Primitive reflexes and postural reactions in the neurodevelopmental examination.

The primitive reflexes and the postural reactions comprise one of the earliest, simplest, and most frequently used tools among child neurolo … ral palsy or developmental retardation as reported in a number of studies. Although a comprehensive neurologic examination in the context of a motor assessment instrument is preferable to an informal list of items, the combined examination of primitive reflexes and postural reactions should be considered by the child neurologist, as a simple but predictive screening tes …

[Symptomatology of movement disorders in children].

Abnormal involuntary movements are common symptoms in children. The neuroanatomical basis for these movement disorders is the extrapyramidal system. Although present at rest they may be more intensive during the will - depending motor activity and increased under stress. They are almost absent during sleep. The underlying causal factors of involuntary movement disorders include metabolic disorders, progressive neurodegenarative diseases, central nervous system infection. The symptomatology of chorea, athetosis, dystonia, tremor, myoclonus and tics are discussed. Special attention is paid to di

Carpal tunnel syndrome: reappraisal of five clinical tests.

A combination of 5 clinical tests: Tinel (9, 10), Phalen (wrist flexion) (1, 2, 17), reverse Phalen (wrist extension) (4), carpal compression (11, 18), vibration (13, 14) was used in 74 hands with symptoms of carpal tunnel syndrome and proved by median sensory conduction velocity and distal latency across wrist and compared with 58 normal hands. Tinel test was the most specific and the least sensitive, vibration test was the most sensitive and the least specific. Carpal compression test was less sensitive and specific compared to Durkan's study. There was no correlation between Phalen time, re
