Noninvasive model of sciatic nerve conduction in healthy and septic mice: reliability and normative data.

Neuromuscular disorders frequently complicate sepsis and other critical illnesses in patients. Mice are the major species used as a model for sepsis. Nerve conduction studies (NCS), the primary tool for noninvasive assessment of nerve and muscle function, is challenging to perform in small animals. A reliable method for noninvasive, repeated NCS testing has not been reported in mice. We developed and validated a method for the repeated measurement of mouse sciatic nerve conduction in normal and septic mice. Our sedated and awake NCS system enabled minimally invasive long-term repeated measurem

[Intermediary syndrome in acute malathion poisoning].

BACKGROUND: Acute poisoning by an organophosphorus insecticide is frequently observed in Morocco. We report a case of malathion poisoning complicated by an intermediate syndrome. CASE REPORT: A 42-year-old woman was hospitalized 3 hours after ingestion of 50 g of malathion. Intubation and ventilatory assistance was required due to failing consciousness and bronchial plugging. On day 4, a neurological syndrome suggestive of the intermediary syndrome with diaphragmatic palsy made it necessary to continue ventilatory assistance to day 20. DISCUSSION: The intermediary syndrome occurs 24 to 96 ho

Selective muscle involvement quantification in late-onset Tay Sachs and Sandhoff disease using neuromuscular ultrasound imaging.

OBJECTIVE: Despite functional mobility losses being the primary complaint in late-onset Tay-Sachs and Sandhoff disease (LOTS-SD), a quantitative understanding of neuromuscular degeneration is lacking. Our aim is to quantitatively characterize muscle degeneration using ultrasound measures of muscle thickness, echogenicity, and stiffness. Additionally, we investigate if elbow and knee extensors are preferentially affected in LOTS-SD. METHODS: Twenty patients diagnosed with LOTS-SD and 20 matched controls (age, sex, BMI) participated. Muscle thickness, echogenicity, and stiffness were quantified

Development of respiratory care guidelines for Duchenne muscular dystrophy in the UK: key recommendations for clinical practice.

UNLABELLED: Significant inconsistencies in respiratory care provision for Duchenne muscular dystrophy (DMD) are reported across different specialist neuromuscular centres in the UK. The absence of robust clinical evidence and expert consensus is a barrier to the implementation of care recommendations in public healthcare systems as is the need to increase awareness of key aspects of care for those living with DMD. Here, we provide evidenced-based and/or consensus-based best practice for the respiratory care of children and adults living with DMD in the UK, both as part of routine care and in a

Impact of patient position on coronal Cobb angle measurement in non-ambulatory myelodysplastic patients.

OBJECTIVE: The purpose of this study was to assess the impact of patient position on the magnitude of the coronal Cobb angle measurements in relation to the change of position using plain radiograph on non-ambulatory children with myelodysplasia. Whole-spine radiographs with the patient sitting generally are preferred for the diagnosis and monitoring of progression of scoliosis in neuromuscular patients. Supine, supine traction, and sitting push-up positions have been used as substitutes, although there is no general consensus validating if these positions correlate with the sitting position.

The Benefit of Neuromuscular Blockade in Patients with Postanoxic Myoclonus Otherwise Obscuring Continuous Electroencephalography (CEEG).

Introduction. Myoclonus status epilepticus is independently associated with poor outcome in coma patients after cardiac arrest. Determining if myoclonus is of cortical origin on continuous electroencephalography (CEEG) can be difficult secondary to the muscle artifact obscuring the underlying CEEG. The use of a neuromuscular blocker can be useful in these cases. Methods. Retrospective review of CEEG in patients with postanoxic myoclonus who received cisatracurium while being monitored. Results. Twelve patients (mean age: 53.3 years; 58.3% male) met inclusion criteria of clinical postanoxic myo

Case report: Neuromuscular block induced by rocuronium following sugammadex administration.

We present a case in which rocuronium was applied for muscle relaxation following the administration of sugammadex. An emergency surgery under general anesthesia was planned for a 43-year-old male patient due to an L1 vertebral corpus and right tibia-fibula shaft fracture. Anesthesia was induced with fentanyl, propofol and lidocaine. After applying only 30mg of the total induction dose of rocuronium, it was learned that the neurological examination should be controlled again from the surgeon because of the controversial of the neurological deficit. As a result, patient awakened from anesthesia

Intubation conditions following rocuronium: influence of induction agent and priming.

A small priming dose of rocuronium can shorten the onset time of neuromuscular blockade. Induction agents with less cardiovascular depression also reduce the onset time. We hypothesized that ketamine, compared to thiopentone, would reduce onset time and improve intubating conditions following priming. Sixty patients ASA I to II, randomized by computer-generated sequence to four groups were investigated in a double-blind controlled trial. In the two groups with priming, 0.04 mg/kg of rocuronium was followed by three minutes of priming interval. Induction was followed by an intubation dose of 0.

The Easy Handgrip Test as a Tool for Assessing Motor Fatigability in Children With Charcot-Marie-Tooth Disease Type 1A.

BACKGROUND AND AIMS: Charcot-Marie-Tooth (CMT), the most common inherited neuromuscular disorder, causes progressive, symmetrical muscle weakness, often affecting distal extremities. Motor fatigability, characterized by a decline in maximal muscle force, is common in neuromuscular disorders but remains underexplored in children with CMT. This study aimed to evaluate the feasibility of a motor fatigability test using a bulb dynamometer in children and adolescents with CMT1A, assess test-retest reliability, and compare handgrip pressure, electromyographic parameters, perceived effort, and time t

Intraoperative feasibility of bulbocavernosus reflex monitoring during untethering surgery in infants and children.

Bulbocavernosus reflex (BCR) monitoring is used to assess the integrity of urinary and bowel function. In this study, we evaluated the feasibility of BCR monitoring during untethering surgery in infants and children to predict postoperative urinary and bowel dysfunction. The records of 22 patients ranging from 4 days to 10 years old (mean 2.7 ± 3.3 years) were reviewed. Anesthesia was maintained by propofol or sevoflurane/opioid without neuromuscular blockade. BCR waveforms induced by electrical stimulation (20-40 mA, train-of-four pulses with 500 Hz) to the penis or clitoris were recorded fro

A Quick Reference on Hypocalcemia.

This article discusses calcium homeostasis, emphasizing the importance of ionized calcium (iCa) as the active form regulated primarily by parathyroid hormone, vitamin D, and other factors. It highlights causes of hypocalcemia, including renal, gastrointestinal, and endocrine disorders, and notes that clinical signs-such as neuromuscular excitability and cardiac arrhythmias-often depend on severity and rapidity of onset. Diagnosis involves measuring serum and ionized calcium, with treatment focusing on correcting underlying causes and carefully restoring iCa levels, especially in animals with s

A Quick Reference on Hypercalcemia.

This article discusses hypercalcemia, emphasizing its causes, diagnosis, and management. It highlights that serum calcium levels reflect water and protein-bound calcium, with ionized calcium being the active form. Common causes include primary hyperparathyroidism, malignancy, kidney disease, vitamin D toxicity, and systemic illnesses. Clinical signs often relate to neuromuscular and cardiovascular disturbances, especially at severe levels. Diagnosis involves measuring serum and ionized calcium, assessing underlying conditions, and differentiating between parathyroid-dependent and independent c

Disease monitoring programs of rare genetic diseases: transparent data sharing between academic and commercial stakeholders.

It has recently been suggested that registries for rare neuromuscular diseases should be formed and governed exclusively by physicians and patients in an effort to limit conflicts of interest. Enacting such an approach would not only be challenging logistically and financially, but it would also exclude the involvement of sponsors, who are an integral component of drug development within the current compliance framework. Therefore, as an alternative to traditional registries, we propose the use of a better collaborative model for post-marketing follow-up that includes all stakeholders. We deve

Absence of significant clinical benefit for a systematic routine creatine phosphokinase measurement in asymptomatic patients treated with anti-programmed death protein (ligand) 1 immune checkpoint inhibitor to screen cardiac or neuromuscular immune-related toxicities.

AIM: Despite unprecedented results of anti-programmed death protein (ligand) 1 (PD-(L)1) immune checkpoint inhibitor in the oncology's armamentarium, immune-related adverse events (irAEs) represent a therapeutic hurdle. Currently, there is no consensual recommendation on a routinely monitored biomarker to early detect irAE. Biological markers such as serum creatine phosphokinase (CPK) are commonly used to measure muscular tissue injury. The potential of routine serum CPK monitoring to predict cardiac or neuromuscular irAE in patients treated with immunotherapy remains unknown. METHODS: In thi

Rocuronium-Induced Rhabdomyolysis: A Case Report and Literature Review.

This case report explores the rare association between rocuronium, a non-depolarizing neuromuscular blocker, and rhabdomyolysis, a serious condition characterized by muscle breakdown. We present a case of an elderly patient who developed rhabdomyolysis after receiving intravenous rocuronium for airway spasticity and analgesic sedation. The patient exhibited a significant increase in creatine kinase (CK) levels, consistent with the diagnostic criteria for rhabdomyolysis. Our analysis indicates that rocuronium may have contributed to the development of this condition, especially with prolonged u

Reflux during positive pressure ventilation through the laryngeal mask.

Patients undergoing anaesthesia for cataract surgery were anaesthetized and their lungs ventilated by intermittent positive pressure ventilation. In one group, ventilation was facilitated by tracheal intubation and in the other group by laryngeal mask airway (LMA). Reflux of stomach contents into the oesophagus was monitored continuously using an indwelling oesophageal pH electrode. The number of discrete episodes of reflux was higher in the LMA group (P = 0.0178), as was the incidence of reflux at antagonism of neuromuscular block (P = 0.0349).

Simple and cost effective clinical methods for measuring neuromuscular fade responses with emphasis on "train of four" fade.

OBJECTIVE: To evaluate different pressure transducers, available in the operating room for pressure measurements, interfaced with common monitoring equipment, for quantitation of the train of four (TOF) fade during clinical neuromuscular block (NMB). METHOD: We determined evoked pressure changes produced by the thumb in response to TOF stimuli. We studied the responses of: a) a membrane disc device, and b) modified pressure transducers which were placed directly under the distal phalanx of the thumb of the clenched hand. The responses were displayed/recorded on OR monitors. The optimal positi

Preclinical Ultrasonography in Rodent Models of Neuromuscular Disorders: The State of the Art for Diagnostic and Therapeutic Applications.

Ultrasonography is a safe, non-invasive imaging technique used in several fields of medicine, offering the possibility to longitudinally monitor disease progression and treatment efficacy over time. This is particularly useful when a close follow-up is required, or in patients with pacemakers (not suitable for magnetic resonance imaging). By virtue of these advantages, ultrasonography is commonly used to detect multiple skeletal muscle structural and functional parameters in sports medicine, as well as in neuromuscular disorders, e.g., myotonic dystrophy and Duchenne muscular dystrophy (DMD).

Dynamic magnetic resonance imaging of muscle contraction in facioscapulohumeral muscular dystrophy.

Quantitative muscle MRI (water-T2 and fat mapping) is being increasingly used to assess disease involvement in muscle disorders, while imaging techniques for assessment of the dynamic and elastic muscle properties have not yet been translated into clinics. In this exploratory study, we quantitatively characterized muscle deformation (strain) in patients affected by facioscapulohumeral muscular dystrophy (FSHD), a prevalent muscular dystrophy, by applying dynamic MRI synchronized with neuromuscular electrical stimulation (NMES). We evaluated the quadriceps muscles in 34 ambulatory patients and
