Seizure symptoms and ambulatory EEG findings: incidence of epileptiform discharges.

AIMS: Ambulatory video-EEG monitoring has been utilized as a cost-effective alternative to inpatient video-EEG monitoring for non-surgical diagnostic evaluation of symptoms suggestive of epileptic seizures. We aimed to assess incidence of epileptiform discharges in ambulatory video-EEG recordings according to seizure symptom history obtained during clinical evaluation. METHODS: This was a retrospective cohort study. We queried seizure symptoms from 9,221 consecutive ambulatory video-EEG studies in 35 states over one calendar year. We assessed incidence of epileptiform discharges for each symp

Anterior spinal artery syndrome after aortic surgery in a child.

Anterior spinal artery syndrome is rare in children. In adults, where it is observed most frequently after resection of thoracoabdominal aortic aneurysms, spinal magnetic resonance imaging is considered the first-line investigation to confirm the clinical diagnosis. A 3-year-old male who presented with this syndrome after palliative cardiac surgery for a complex cardiac malformation associated with aortic coarctation is presented. Clinical diagnosis of anterior horn cell impairment below the L2 level was confirmed by electromyography and F-wave studies. Sparing of dorsal sensory tracts was doc

Early neurophysiological biomarkers and spinal cord pathology in inherited prion disease.

A common presentation of inherited prion disease is Gerstmann-Sträussler-Scheinker syndrome, typically presenting with gait ataxia and painful dysaesthesiae in the legs evolving over 2-5 years. The most frequent molecular genetic diagnosis is a P102L mutation of the prion protein gene (PRNP). There is no explanation for why this clinical syndrome is so distinct from Creutzfeldt-Jakob disease, and biomarkers of the early stages of disease have not been developed. Here we aimed, first, at determining if quantitative neurophysiological assessments could predict clinical diagnosis or disability an

Anaesthesia for correction of scoliosis in children.

Surgical correction of spinal deformities in children presents a challenge to the anaesthetist because of the extensive nature of the surgery, the co-morbidities of the patients and the constraints on anaesthetic techniques of intraoperative neurophysiological monitoring of the spinal cord. Adolescent idiopathic scoliosis is the most common deformity. Patients with scoliosis secondary to neuromuscular conditions are at greatest risk of perioperative problems, particularly excessive blood loss and respiratory failure. The risk of spinal cord damage can be decreased by the use of intraoperative …

Technical note: pre-positioning lower limb SSEP during semi-sitting positioning in posterior fossa surgery- does it matter?

Intra-operative monitoring has been a crucial tool in modern neurosurgery as it allows to optimize surgical outcome whilst reducing neurological deficits. Somatosensory evoked potentials are routinely monitored in most spinal and brain surgeries due to providing invaluable information regarding the functional integrity of sensory pathways. The use of this neurophysiological technique is particular useful when positioning patients in semi-sitting position during posterior fossa surgery. However, there is general agreement within the intra-operative neuromonitoring community that either upper or

Spectral Differences of Anesthetic Agents: Addressing Fundamental Problems With New Methods.

BACKGROUND: Processed electroencephalography parameters are used to guide anesthesia to adequate levels for surgical procedures. Despite known spectral differences between anesthetics, studies often assume similar anesthetic states when titrating to the same target values, presupposing a reductive one-size-fits-all approach for all anesthetic agents. We hypothesize this may introduce bias and aim to characterize the differences using conventional and new approaches. METHODS: For this retrospective study, we included 108 patients undergoing surgery under general anesthesia with either fluranes

Which patients do we need to consider augmentation of muscle active potentials regarding transcranial electrical stimulation motor-evoked potentials monitoring before spine surgery?

BACKGROUND CONTEXT: Transcranial electrical stimulation motor-evoked potentials (Tc-MEPs) are the current trend and are important in preventing intraoperative neurological deficits. Posttetanic Tc-MEPs (p-MEP) can augment the amplitudes of compound muscle active potentials (CMAPs), especially in the case of insufficient conventional Tc-MEPs (c-MEP). PURPOSE: To retrospectively investigate pre- and intraoperative factors necessitating p-MEP monitoring and to examine changes in the success rates of baseline Tc-MEP monitoring before and after tetanic stimulation in patients with such factors. Overcoming age: Slow anesthesia induction may prevent geriatric patients from developing burst suppression and help developing intraoperative EEG signatures of a younger brain.

Elderly patients are prone to develop postoperative neurocognitive deficits potentially precipitated by inadequate anesthetic management. To investigate the potential of EEG-guided individualized anesthetic titration we evaluated the effect of the patient's age on the spectral composition of the EEG during slow propofol induction. Twenty-six young (<65 years) and 25 old (≥65 years) patients received propofol until loss of responsiveness (LOR). After LOR, we switched from a flow rate-based to a target-controlled infusion mode keeping the calculated effect-site concentration at LOR stable. We re

Intraoperative Lower Extremity SSEP Absence from Over-Tightened Safety Strap: Case Report.

Somatosensory evoked potentials (SSEPs) are used in spinal surgeries to monitor the functional integrity of dorsal column medial lemniscal pathways. Changes to waveform amplitudes can be due to surgical manipulation, in addition to many outside factors. We describe a posterior lumbar decompression/fusion case in which lower extremity (LE) SSEPs were performed and deteriorated bilaterally. These changes were found to be related to a safety strap traversing the posterior thighs that was excessively tight, causing neurological dysfunction. After loosening the strap, SSEP amplitudes and latencies

Intraoperative neuromonitoring in degenerative cervical myelopathy surgery: a systematic review and meta-analysis of diagnostic accuracy and postoperative neurological outcomes.

OBJECTIVES: Degenerative cervical myelopathy (DCM) often requires surgical decompression but carries a risk of intraoperative neural injury. Intraoperative neurophysiological monitoring (IONM) is widely used to detect impending spinal cord compromise during such surgery, yet its impact on postoperative neurological outcomes remains debated. This systematic review and meta-analysis evaluated (1) the diagnostic accuracy of IONM modalities - somatosensory evoked potentials (SSEP), motor evoked potentials (MEP), and multimodal monitoring - and (2) the effect of IONM versus no IONM on postoperative…

Multimodal neuromonitoring in the nordic countries: experiences and attitudes - a multi-institutional survey.

BACKGROUND: Multimodal neuromonitoring (MMM) aids early detection of secondary brain injury in neurointensive care and facilitates research in pathophysiologic mechanisms of the injured brain. Invasive ICP monitoring has been the gold standard for decades, however additional methods exist (aMMM). It was hypothesized that local practices regarding aMMM vary considerably and that inter-and intracenter consensus is low. The survey aimed to investigate this hypothesis including the knowledge, attitudes towards, and use of aMMM in the neurointensive care setting in the Nordic countries. METHOD: Th

Evolution of EEG Findings in Patients with Acute Brain Injury.

BACKGROUND: Increasing use of continuous electroencephalography (cEEG) provides the opportunity to observe temporal trends in EEG patterns during the acute phase of brain injury. These trends have not been extensively documented. METHODS: We conducted a retrospective chart review of patients undergoing cEEG between January 1st and June 30th, 2019, at two academic medical centers. Only patients with acute brain injury having electrographic or electroclinical seizures or epileptic EEG findings on day 1 of monitoring and ≥ 2 calendar days of cEEG were included. The temporal evolution of EEG patt

Quadripolar Stimulation as an Adjunct to Neuromonitoring in Scoliosis Surgery: A Case Report.

CASE: A healthy 13-year-old adolescent girl with a normal spine magnetic resonance imaging underwent posterior spinal fusion for adolescent idiopathic scoliosis (AIS). Before surgery, she had abnormal baseline motor-evoked potentials (MEPs). After technical issues were ruled out, quadripolar stimulation yielded normal baseline MEPs and somatosensory evoked potentials. The surgery was completed without complication, and the patient awoke from surgery without signs of neurological deficits. CONCLUSION: Quadripolar stimulation with the use of an additional paired anode and cathode is a useful ad

Practicalities of noninvasive neuromonitoring in intensive care.

BACKGROUND: Noninvasive neuromonitoring provides valuable insights into cerebral physiology and function, with the potential to support individualized care for patients who are critically ill. As precision medicine continues to advance within the intensive care unit, the integration of multiple noninvasive monitoring techniques may offer a more comprehensive, real-time understanding of dynamic cerebral pathophysiology while avoiding the risks of invasive procedures. This study aimed to identify the implementation challenges of combining multiple noninvasive neuromonitoring modalities. Six moda

A novel technique for monitoring somatosensory evoked potentials during brainstem surgery: an illustrative case series.

The surgical management of brainstem tumors and cavernous malformations represents one of the most demanding challenges in neurosurgery, given the critical concentration of eloquent neural structures in this region. These include nuclei, cranial nerve roots and their corticonuclear tracts, corticospinal and spinothalamic pathways. Intraoperative injury to these critical structures may result in permanent neurological deficits, including progressive sensory deterioration during the postoperative course. Therefore, continuous monitoring of the functional state of the spinothalamic tracts during

Intraoperative ECoG in bottom-of-the-sulcus syndrome using a novel flexible strip electrode.

The recording of epileptiform discharges from bottom-of-sulcus focal cortical dysplasia (BOSD) is often difficult during intraoperative electrocorticography (ECoG) due to the deep localization. We describe the use in this scenario of a new-generation electrode strip with high flexibility, easily adapted to cortical gyri and sulci. A right-handed 20-year-old male with drug-resistant focal epilepsy due to BOSD of the inferior frontal gyrus and daily focal aware seizures was evaluated for epilepsy surgery. Based on electroclinical and neuroimaging results, a focal cortectomy guided by ECoG was pr
