Neurophysiological abnormalities in individuals with persistent post-concussion symptoms.

Concussion injury results in a rapid onset of transient neurological impairment that can resolve quickly, or sometimes evolve over time, but usually resolve within seven to 10 days. However, a small but noticeable cohort (~10%) of individuals continues to experience persistent lingering effects, particularly fatigue, recognized as post-concussion symptoms (PCS). This study explored neurophysiological mechanisms in people with persistent PCS. Studies involved using self-report post-concussion fatigue scale, transcranial magnetic stimulation (TMS) and somatosensory stimulation in those with diag

Pathophysiology of corticobasal degeneration: Insights from neurophysiological studies.

BACKGROUND: Several studies have applied electrophysiological techniques to physiologically characterize corticobasal degeneration (CBD). METHODS: We performed a systematic literature search of these studies and reviewed all 25 identified articles. RESULTS: Conventional electroencephalography (EEG) is usually normal even in the late stages of disease. Quantitative EEG (qEEG) with spectral analysis revealed mainly lateralized abnormalities, such as an increase of slow wave activity and occasionally the occurrence of sharp waves, and a significant increase of coherence between left parietal-ri

Neurophysiological correlates of abnormal somatosensory temporal discrimination in dystonia.

BACKGROUND: Somatosensory temporal discrimination threshold is often prolonged in patients with dystonia. Previous evidence suggested that this might be caused by impaired somatosensory processing in the time domain. Here, we tested if other markers of reduced inhibition in the somatosensory system might also contribute to abnormal somatosensory temporal discrimination in dystonia. METHODS: Somatosensory temporal discrimination threshold was measured in 19 patients with isolated cervical dystonia and 19 age-matched healthy controls. We evaluated temporal somatosensory inhibition using paired-

Concordance of clinical and neurophysiologic diagnoses of carpal tunnel syndrome.

INTRODUCTION/AIM: Clinical presentation and neurophysiological examination are crucial in diagnosing carpal tunnel syndrome (CTS). The aim of this study was to determine sensitivity and specificity of clinical examination for diagnosing of CTS in relation to neurophysiological evaluation. METHODS: The sample included 181 patients referred to the eurologist for further diagnosis of pain and parestesias in the arm (81 women and 100 men mean age 42 ± 14 years and 52 ± 16 years, respectively). All the patients were neurophysiologicly tested. RESULTS: Out of 181 patients, clinical findings were c

Mild Lafora disease: clinical, neurophysiologic, and genetic findings.

We report clinical, neurophysiologic, and genetic features of an Italian series of patients with Lafora disease (LD) to identify distinguishing features of those with a slowly progressive course. Twenty-three patients with LD (17 female; 6 male) were recruited. Mean age (± SD) at the disease onset was 14.5 ± 3.9 years and mean follow-up duration was 13.2 ± 8.0 years. NHLRC1 mutations were detected in 18 patients; EPM2A mutations were identified in 5. Patients who maintained >10 years gait autonomy were labeled as "mild" and were compared with the remaining LD patients with a typical course. Si

Intraoperative neurophysiological monitoring of microvascular decompression for glossopharyngeal neuralgia.

PURPOSE: To evaluate if adding cranial nerves (CNs) V and VI to standard intraoperative neurophysiological monitoring (IONM) of microvascular decompressions for glossopharyngeal neuralgia improve its efficacy. METHODS: We reviewed all patients who received a microvascular decompression for glossopharyngeal neuralgia at our institution between January 2008 and August 2012. All received upper extremity somatosensory evoked potentials, brainstem auditory evoked potentials, and free-running electromyography of muscles innervated by ipsilateral CNs VII, IX, and X. The sample was divided into 12 pa

Behavioral and neurophysiological effects of transdermal rotigotine in atypical parkinsonism.

Effective therapies for the so-called atypical parkinsonian syndrome (APS) such as multiple system atrophy (MSA), progressive supranuclear palsy (PSP), or corticobasal syndrome (CBS) are not available. Dopamine agonists (DA) are not often used in APS because of inefficacy and in a minority of case, their side effects, like dyskinesias, impairment of extrapyramidal symptoms or the appearance of psychosis, and REM sleep behavioral disorders (RBD). Transdermal rotigotine (RTG) is a non-ergot dopamine agonist indicated for use in early and advanced Parkinson's disease with a good tolerability and

Intraoperative neurophysiologic sensorimotor mapping and monitoring in supratentorial surgery.

It has been shown that aggressive removal of gliomas improves survival and the quality of life in both adults and children. Conversely, there is a strong correlation between incomplete resection of an epileptic focus and poor seizure control outcome in epilepsy surgery. Thus, it is no surprise that maximal resection of supratentorial lesions remains a priority in neurologic surgery. In many circumstances, this is difficult to achieve because of the close proximity of functionally eloquent regions. As a consequence, accurate identification of the latter is imperative to reliably identify safe b

Neurophysiological investigation of idiopathic acquired auditory-visual synesthesia.

We present a case of acquired auditory-visual synesthesia and its neurophysiological investigation in a healthy 42-year-old woman. She started experiencing persistent positive and intermittent negative visual phenomena at age 37 followed by auditory-visual synesthesia. Her neurophysiological investigation included video-EEG, fMRI, and MEG. Auditory stimuli (700 Hz, 50 ms duration, 0.5 s ISI) were presented binaurally at 60 db above the hearing threshold in a dark room. The patient had bilateral symmetrical auditory-evoked neuromagnetic responses followed by an occipital-evoked field 16.3 ms la

Neurophysiological and epigenetic effects of physical exercise on the aging process.

Aging is a gradual process during which molecular and cellular processes deteriorate progressively, often leading to such pathological conditions as vascular and metabolic disorders and cognitive decline. Although the mechanisms of aging are not yet fully understood, inflammation, oxidative damage, mitochondrial dysfunction, functional alterations in specific neuronal circuits and a restricted degree of apoptosis are involved. Physical exercise improves the efficiency of the capillary system and increases the oxygen supply to the brain, thus enhancing metabolic activity and oxygen intake in ne

Neurophysiological and cognitive effects of smoked marijuana in frequent users.

RATIONALE: Previously, we reported that acute marijuana intoxication minimally affected complex cognitive performance of daily marijuana smokers. It is possible that the cognitive tests used were insensitive to marijuana-related cognitive effects. OBJECTIVES: In the current study, electroencephalographic (EEG) signals were recorded as daily marijuana users performed additional tests of immediate working memory and delayed episodic memory, before and after smoking marijuana. METHODS: Research volunteers (N=24), who reported smoking approximately 24 marijuana cigarettes/week, completed this st

Neurophysiologic intraoperative monitoring during selective dorsal rhizotomy.

Selective dorsal rootlet rhizotomy (SDR) is a neurosurgical procedure designed to reduce spasticity in the legs, although preserving motor and sensory function, of appropriately selected children with spastic quadraparesis. This is accomplished by neurophysiologicially guided (e.g., selective) severing of specific dorsal rootlets in the cauda equina. This decreases facilitatory input to spinal anterior motor neurons, thereby reducing spasticity in the legs. This first portion of this article discusses the neurophysiologic intraoperative monitoring techniques during SDR, with the understanding

Propriospinal myoclonus revisited: Clinical, neurophysiologic, and neuroradiologic findings.

OBJECTIVE: The literature on propriospinal myoclonus (PSM) is poor and there are no systematic reviews of the subject. We sought to clarify the spectrum of PSM. METHODS: We first prospectively investigated all patients seen in our movement disorders clinic with a firm diagnosis of PSM between 2002 and 2007. All had a standardized interview, detailed clinical examination, laboratory investigations, comprehensive neurophysiologic examination, and spinal cord MRI, including diffusion tensor imaging with fiber tracking (DTI-FT). We also collected drug responses. Finally, we conducted a systematic

Effects of topiramate on neurophysiological and neuropsychological tests in migraine patients.

Topiramate (TPM) is a antiepileptic drug that has multiple mechanisms of action. It is effective as a monotherapy for migraine prophylaxis. Unfortunately, TPM can frequently cause adverse effects, such as cognitive dysfunction. The present study examines the neuropsychological and neurophysiological effects of TPM in 35 consecutive migraine patients above 18years of age. The TPM dose was started at 25mg/day and increased by 25mg/week, until reaching the maximum dose of 50mg twice daily in the fourth week. Patients were evaluated for development of side effects of the medication and for its eff
