Managing incontinence-associated dermatitis in the community: an overview.

Incontinence-associated dermatitis (IAD) is often treated a hygienic challenge, rather than a serious condition with potentially life-threatening consequences. More appropriate education on the management strategies specific to IAD is required, in order for personalised and effective care that reflects the critical nature of this condition to be provided. Francesca Ramadan provides an overview of the key elements of best practice in IAD management and treatment.

Urinary catheter use in end of life care.

Patients nearing the end of life can become agitated by a full bladder and may need urgent urinary catheterisation. However, urethral catheterisation needs authorisation by a doctor. Nurses at a London trust developed a urinary catheterisation consent form permitting nurses to perform this procedure when it becomes necessary without having to gain medical authorisation. This article discusses how the consent form can help improve bladder management, ensuring terminally ill patients do not have to attend the emergency department to be catheterised.

Catheter blockage in community patients.

Research has shown that over 50 per cent of long-term catheterised patients experience recurrent blockage. For patients in the community this presents particular problems but also places increased demand on resources and nursing time. Kathryn Getliffe describes the findings of a project which examined a number of issues relating to catheter blockage in patients at home. The efficiency of nursing services can, she argues, be enhanced by effective individualised care planning.

An evaluation of reinforcement of genetic counselling on the consultand.

This project studied the effect of reinforcement of genetic counselling in the home, on consultand recall of information discussed in the clinic. Acceptable recall was observed in 84% of 227 patients scored for recall of rate of recurrence, understanding of the nature of the disease at special risk and of its mechanism of origin. Our results show no significant difference in the frequency of acceptable recall after reinforcement of genetic counselling compared with an absence of reinforcement. The consultands' understanding was substantially affected by the type of genetic mechanism involved.

Home vaccination for children behind in their immunisation schedule: a randomised controlled trial.

OBJECTIVE: To ascertain the effectiveness of a home vaccination service for children behind in their vaccination schedule. DESIGN: Randomised controlled trial of nurse-administered vaccination at home. Children were allocated at random to the intervention or the control group before any contact with the parents was made. SETTING: 10 council areas in north-west metropolitan Melbourne defined by 56 postcode zones. Six-week intervention period from November 1996. PARTICIPANTS: 405 children--all those in the study area (n = 2610) 90 days late (age 9 months) for their third diphtheria-tetanus-pe

Prevalence of Musculoskeletal Disorders for Nurses in Hospitals, Long-Term Care Facilities, and Home Health Care: A Comprehensive Review.

OBJECTIVE: The aim of this study was to determine the prevalence of musculoskeletal pain and reported injuries for nurses and nursing aides. BACKGROUND: Nurses and nursing aides suffer from work-related pain and musculoskeletal disorders (MSDs). Although there have been a plethora of studies on MSDs, an overall understanding of the prevalence of MSDs and pain can lead to better prioritization of research needs with respect to the health care industry. METHOD: A total of 132 articles on prevalence of MSD pain and injuries were included in the review. All articles were published in peer-review

Faecal incontinence in the elderly: issues and interventions in the home.

This paper discusses current evidence-based guidelines for the management of faecal incontinence in the elderly. The key aspects of maintaining dignity, quality of life and self-esteem are addressed within a presentation of good assessment, prevention, management and treatment principles. Particular focus is given to the added problems that faecal incontinence may bring to the elderly person suffering with this debilitating, and often embarrassing condition. The increased risk to skin integrity is discussed, with reference to available guidance to help nurses manage the risk of skin breakdown

The effect of a short-term Balint group on the communication ability and self-efficacy of pre-examination and triage nurses during COVID-19.

AIMS AND OBJECTIVES: To investigate the effect of a short-term Balint group on the improvement in communication ability and self-efficacy of pre-examination and triage nurses during coronavirus disease 2019 (COVID-19). BACKGROUND: Working on the front lines of COVID-19 has brought unprecedented psychological stress on pre-examination and triage nurses. Nurse-patient communication ability and the level of self-efficacy are both significant for nurses' psychosomatic health and work input. However, limited empirical evidence exists regarding nurse-patient communication and self-efficacy and spec

A community child and adolescent psychiatry fellowship for nurse practitioners.

Over the past three decades, there has been a well-documented shortage of child and adolescent psychiatric medical providers while demand continues to rise. Youth from minority racial and ethnic backgrounds, low-income families, and rural settings are disproportionately affected, increasing disparity in access and quality of services. While psychiatric mental health nurse practitioners (PMHNP) can deliver a full range of mental health care services across the lifespan, there are discrepancies across training programs in exposure to child and adolescent cases and high quality training. To addre

Operation Enduring Freedom (OEF) and Operation Iraqi Freedom (OIF) military mental health issues. Information on the wars' signature wounds: posttraumatic stress disorder and traumatic brain injury.

This topic can be related to the nursing profession and the need to be better educated on military mental health. Since mood disorder, suicide, alcohol abuse, PTSD and TBI are evident in actively serving and returning service members, it is imperative to educate nurses and healthcare providers about these conditions, the available evidence-based treatments and referrals to programs for these signature OEF/OIF wounds. The authors encourage nurse educators to consider ways to include military mental health and other service-related health issues into nursing curricula and to use Veterans Adminis

Scaling community-based care for conductive hearing loss in low- and middle-income settings.

PURPOSE OF REVIEW: This review examines recent advances in community-based care for conductive hearing loss in low- and middle-income countries (LMICs), focusing on developments that have materially shifted the feasibility of decentralized service delivery across prevencommution, screening and diagnosis, medical management, and rehabilitation. RECENT FINDINGS: World Health Organization guidance now explicitly endorses task-sharing for primary ear and hearing care, including community health worker-facilitated hearing aid provision. Long-term follow-up from pneumococcal vaccination trials demo

Commentary: Advancing Nursing in Canada: Toward the Elimination of Anti-Black Racism.

This commentary challenges historic and contemporary issues within nursing and provides direction toward a more inclusive future for nursing. This is a call-to-action for nurses, nursing students and nurse allies to advance effort toward the elimination of anti-Black racism in nursing in Canada. To achieve this, it is imperative to move beyond the performative and adopt practices that enable critical reflection and action. Addressing the manner in which exclusion is reinforced and perpetuated requires interrogation of four distinct yet interconnected processes of racial exclusion and discrimin

Moving sexual health into primary care.

The rise in sexually transmitted infections (STIs) and the high rate of teenage pregnancies mean that sexual health services need to be increasingly provided in primary care settings by primary care professionals. Guidance from the National Institute of Clinical Excellence (NICE) in 2007 recommends proactive intervention sessions with individuals whose sexual history identifies them as at risk. Midwives and health visitors should make regular visits to under-18s who are pregnant or already mothers to provide advice on sexual health and contraception. Primary care professionals should be proact

It introduces and evaluates a new preadmission assessment, identifies key factors that influence the process of health care, and compares the results of this study with the current evidence base. METHOD: Process evaluation with multiple stakeholder analysis using semi-structured interviews for data collection. A focus group was used in preparation for the
