Neonatal intensive care: communication and attachment.

Birth has traditionally been recognized as a family-centred event where immediate exposure to the infant occurs and where the family begins, through direct contact, touch and voice, to form an attachment to the infant. In high-risk pregnancy and delivery, medical and technological advances distance the family from the birth event. This article describes some aspects of a research project with identified factors that facilitate and hinder family and mother attachment to the infant during periods of separation subsequent to premature delivery or other problems which require management in a neona

Perception of simulation-based education among nursing and midwifery students in Tanzania: a qualitative study.

BACKGROUND: While many nursing programs in developed countries have implemented simulation-based education as a pedagogic method of teaching, implementation of simulation in developing countries like Tanzania is rare. Traditional methods of auditorium lectures are widely conducted in low-income nursing and midwifery education institutions. Such pedagogy provides students with theoretical knowledge yet with limited hands-on exposure for clinical skills, which might affect the professional integration of students and quality care delivery. This study explored perceptions of simulation-based educ

Pain management for the pregnant, lactating, and neonatal to pediatric cat and dog.

Little information on the approach to analgesia in pregnant, nursing, or extremely young animals is available in the veterinary literature. Various analgesics and analgesic modalities are discussed, with emphasis placed on preference and caution for each group. Management of pain is extremely important in all animals but especially in the extremely young, in which a permanent hyperalgesic response to pain may exist with inadequate therapy. Inappropriate analgesic selection in pregnant and nursing mothers may result in congenital abnormalities of the fetus or neonate. Inadequate analgesia in nu

What's your literacy level?

I am writing this editorial after just returning from a conference highlighting an exciting evidence-based project headed by Dr. Shoo Lee from Toronto. Fondly known as EPIQ (Evidence-Based Practice for Improving Quality), this multiyear, multisite study funded by the Canadian Institutes of Health Research uses benchmarking and a process of continuous quality improvement to address five morbidities in low birth weight infants: retinopathy of prematurity, bronchopulmonary dysplasia, intraventricular hemorrhage, necrotizing enterocolitis, and infection. The three pillars of EPIQ include evaluatio

Effect of a dedicated percutaneously inserted central catheter team on neonatal catheter-related bloodstream infection.

PURPOSE: To evaluate whether the establishment of a dedicated percutaneously inserted central catheter (PICC) team is associated with reduced risk of catheter-related bloodstream infection (CRBSI) in the neonatal intensive care unit. SUBJECTS: Participants were extremely low-birth-weight infants admitted to a level IIIC neonatal intensive care unit. DESIGN: A before- versus after-intervention study design was implemented. Intervention group participants were admitted after April 2006 when the PICC team was established, dedicating line insertion and maintenance responsibilities to the team. H

Provider Perceptions of Bubble Continuous Positive Airway Pressure and Barriers to Implementation in a Level III Neonatal Unit in South India.

BACKGROUND: Bubble continuous positive airway pressure (bCPAP) is a simple, safe, and cost-effective strategy to provide respiratory support to newborns with respiratory distress syndrome in resource-limited settings. PURPOSE: To understand whether implementation of bCPAP, relative to other modes of respiratory support in the care of newborns with respiratory distress syndrome, increases positive attitudes about its potential for consistent and widespread use among providers in neonatal intensive care units (NICUs) of lower middle-income countries. METHODS: Semistructured qualitative intervi

Management of neonatal abstinence syndrome in the newborn nursery.

Maternal drug use and neonatal abstinence syndrome (NAS) are being seen across the United States. NAS occurs with withdrawal disturbances in response to the cessation of the pregnancy exposure. The clinical presentation of a newborn with NAS can include gastrointestinal, neurologic, vasomotor and respiratory symptoms. Assessment of newborns with NAS can often present as a challenge to maternal-child nurses. Treatment can include supportive care as well as pharmacologic therapies.

Prevention of the development of the typical dislocation of the hip.

Applying a diaper and baby clothing without changing the naturally flexed leg requires education of mothers and nurses. Retaining the flexed position which is present from the very beginning of postnatal life, markedly reduces the incidence of click sign. A program on neonatal diapering, clothing, and general care, reduced the incidence of congenital dislocation of the hip in 3 to 7 months old infants in a sample population of 200,000.

Clinical application of a new glucose analyzer in the neonatal intensive care unit: comparison with other methods.

We evaluated the operation of the Yellow Springs Instrument Co. (YSI) glucose analyzer (model 23A) by clinical nurses for the measurement of blood glucose in the intensive care nursery. In vitro performance was determined with the use of aqueous standards; with a 2-point calibration of 0.0 and 200 mg/dl, a precision of better than 1.0% of each standard (25, 50, 100, 200 mg/dl) was achieved, and the linearity was excellent (Y = 0.99X - 0.49, r = 0.99). The YSI correlated well with a manual spectrophotometric glucose oxidase method (r = 0.99) and the Kodak Ektachem analyzer (r = 0.98) using huma

Multimodal neonatal procedural and postoperative pain assessment dataset.

This paper presents the first multimodal neonatal pain dataset that contains visual, vocal, and physiological responses following clinically required procedural and postoperative painful procedures. It was collected from 58 neonates (27-41 gestational age) during their hospitalization in the neonatal intensive care unit. The visual and vocal data were recorded using an inexpensive RGB camera while the physiological responses (vital signs and cortical activity) were recorded using portable bedside monitors. The recorded behavioral and physiological responses were scored by expert nurses using t

Maternal and Neonatal Outcomes of Healthy Pregnant Women With COVID-19 Versus High-risk Pregnant Women: A Multi-Center Case-Control Comparison Study.

The purpose of this retrospective, matched case-control study (two controls [healthy control and high- risk control] vs. COVID-19 cases) was to compare the maternal and neonatal outcomes of pregnant women with and without COVID-19. A total of 261 pregnant women from three different countries with and without COVID-19 were included in this study. Several pregnancy complications were more common in high-risk pregnant women compared to COVID-19 cases and healthy pregnant women. These include preeclampsia (p < .01), vaginal bleeding (p < .05), preterm labor (p < .05), premature rupture of membrane

Lessons from improved neonatal survival at Chandigarh.

We observed a significant fall in neonatal mortality in babies weighing less than 2 kg during 1986 as compared to 1973 (7.94% vs 12.88%; p less than 0.005), and in preterm babies the mortality fell from 26.88 to 11.5% (p less than 0.001) during 1986. This was achieved despite 2-3 fold increase in the high risk babies and without any increase in the number of neonatal special care beds or nurses. Effective utilisation of the facilities was made possible through: (a) reduction in admissions to neonatal special care unit of babies with birth weight more than 1500 g; (b) early discharge of babies
