[Hepato-biliary complications in enteral nutrition].

Hepatobiliary complications of enteral nutrition have been studied little. The incidence of increase in hepatic enzymes during enteral nutrition is minimal, less than 5% and no clinical hepatic disorder has been observed during enteral nutrition. The gall bladder is contractile and echo-less. Unlike total parenteral nutrition, enteral nutrition does not cause any hepatobiliary complications.

Nutritional care for patients with Crohn's disease.

Protein calorie malnutrition, in addition to deficits of other nutrients, occurs frequently among patients with Crohn's disease. In most instances the mechanisms by which these deficits occur are multifactorial. The most appropriate method to assess nutritional status includes a nutritionally oriented medical history and physical examination combined with a measurement of serum albumin. If the gut can be used safely, enteral nutrition is the preferred feeding method for Crohn's disease patients needing nutritional support. The advantages of enteral nutrition are stimulatory effects on gastroin

[Total parenteral nutrition (TPN) in oncology (author's transl)].

The role of TPN in oncology is, in spite of favourable reports, not yet settled. Selected undernourished cancer patients are certainly helped by parenteral nutrition during their course of therapy. Their nutritional status becomes better, their endurance to antineoplastic therapy is enhanced with less complications and more important, their "quality of life" is improved. However, the survival time of the patient is not prolonged. Well designed prospective randomised clinical studies will provide a better insight into this problem. The anorexia of cancer patients might be caused by tumor-induce

Detection of eating disorders in patients: validity and reliability of the French version of the SCOFF questionnaire.

BACKGROUND & AIMS: Although eating disorders prevalence is increasing, they are often under diagnosed in cases of unspecific signs of malnutrition. Screening scales may allow earlier diagnosis and nutritional intervention. This study aimed to evaluate the validity of the French version (SCOFF-F) of the SCOFF questionnaire for the detection of eating disorders among a female patient population referred to a clinical nutrition unit. METHODS: After answering the 5 dichotomous questions of the paper version of SCOFF-F, patients were evaluated by one eating disorders specialist blinded to question

Nutrition in acute renal failure patients.

The care of patients with acute renal failure remains a challenge for the nephrologist, with unacceptably high mortality rates. Among many other contributing factors, protein-calorie malnutrition has been suggested as an important predictor of outcome. Protein-calorie malnutrition is highly prevalent in acute renal failure (ARF) patients, and extensive catabolism is a hallmark of these patients. Hypercatabolism can be related to abnormal protein metabolism as well as be a consequence of renal replacement therapy. Inadequate nutritional supplementation also predisposes acute renal failure patie

Nutritional considerations for a new era: A CF foundation position paper.

OBJECTIVE: To provide interim advice and considerations to the CF Community around CF nutrition in the current era. METHODS: The Cystic Fibrosis (CF) Foundation organized a multidisciplinary committee to develop a Nutrition Position Paper based on the rapidly changing nutrition landscape in CF, due in part to widespread use of cystic fibrosis transmembrane regulator highly effective modulator therapy (HEMT). Four workgroups were formed: Weight Management, Eating Behavior/Food Insecurity, Salt Homeostasis and Pancreatic Enzyme use. Each workgroup conducted their own focused review of the liter

A disconnection between nutritional status (in terms of body mass index and phase angle) and psychopathology in anorexia nervosa.

The aim of our study was to investigate the relationship between nutritional status (body mass index and phase angle) and psychological symptoms at admission and discharge in a residential population of anorexic patients. We also aimed to determine the evolution of the above psychological symptoms and nutrition rehabilitation from admission to discharge. Thirty-six consecutive patients were included. The evaluation was performed using the following measures at admission and discharge: body mass index, phase angle, Eating Disorders Inventory-3, Multiphasic Personality Inventory-2 and Body Uneas

Coma query cause.

A 54-year-old woman with coeliac disease was admitted to hospital electively for supplemental nutrition. Shortly after feeding started she deteriorated into a hyperammonemic coma with refeeding syndrome, requiring an extensive intensive care admission. Urea cycle disorders were investigated and a biochemical diagnosis of ornithine transcarbamylase deficiency was made. This is a rare diagnosis in the adult population. This case report summarises protein metabolism, urea cycle disorders and the challenges of management.

Nutritional Considerations for Bladder Storage Conditions in Adult Females.

BACKGROUND: Clinical guidelines developed by urologic, urogynecologic, and gynecologic associations around the globe include recommendations on nutrition-related lifestyle and behavioral change for bladder storage conditions. This study identified and compared clinical guidelines on three urological conditions (interstitial cystitis/bladder pain syndrome (IC/BPS), overactive bladder, and stress urinary incontinence) affecting adult women. METHODS: A three-step process was employed to identify the guidelines. Next, a quality assessment of the guidelines was conducted employing the Appraisal of

[Severe small bowel involvement and chronic intestinal pseudo-obstruction in systemic sclerosis (scleroderma): Pathophysiological, diagnostic and therapeutic basis, including parenteral nutrition].

Gastrointestinal involvement in systemic sclerosis can be severe, reaching the critical point of chronic intestinal pseudo-obstruction, secondary to major disorders of small bowel motility. It is associated with some clinical and biological characteristics, in particular the positivity of anti-fibrillarin/U3RNP antibodies. Chronic intestinal pseudo-obstruction (CIPO) is complicated by a small intestinal bacterial overgrowth that requires cyclic antibiotic therapy. CIPO leads to a reduction of the food intake, due to painful symptoms, nausea and vomiting caused by meals, and ultimately to sever

[Microvillous inclusion disease as a cause of severe congenital diarrhea. Case report].

UNLABELLED: Congenital diarrheas correspond to a severe and low frequency digestive disease, with a high mortality. They start a few days or months after birth, leading to intestinal insufficiency and dependence on parenteral nutrition. It must be highly suspected in newborns or infants with diarrhea and severe electrolyte disorders. The diagnosis is based on clinical, endoscopic, histologic and eventually genetic findings. Treatment is supportive with intensive correction of electrolyte imbalances as well as parenteral nutrition. OBJECTIVE: To present a case report of congenital diarrhea ide

Adolescent nutrition: self-perceived deficiencies and needs of practitioners working with youth.

Sixty-three percent of a random sample of 866 members within three practice groups of The American Dietetic Association responded to a survey designed to assess (a) perceived competency of nutrition management in 20 major areas of adolescent health, (b) desire to increase skill level in each area, and (c) preferred approaches for continuing education activities. Of the 549 respondents, 92% were registered dietitians (R.D.s), 5% were registration-eligible, and more than half (51%) had advanced degrees. Twenty-five percent or more of all practitioners reported deficiencies in 17 of the 20 catego

[Social phobia in anorexia nervosa: evolution during the care].

CONTEXT: The links between anorexia nervosa (AN) and anxiety disorders, and particularly social phobia, are little known. However, social phobia occurs frequently in AN. Some studies have shown reduction in anxious and depressive symptomatology in AN with re-nutrition. But, to our knowledge, no work has examined the evolution of social phobia symptoms during re-nutrition in AN. OBJECTIVES: To specify the links between AN, nutritional state, and social phobia. METHOD: The population consisted of 2 samples and the analysis was conducted using the SPSS11.5. Sample 1 (N=24 AN) was evaluated on a

Body mass index and diet-related inflammation as predictors of sleep disorders: A cross-sectional study.

This study examines diet as a key risk factor for sleep disorders and integrates physiological indicators to develop a machine learning (ML)-based model for targeted public health interventions. Data from 5158 2011 to 2014 National Health and Nutrition Examination Survey (NHANES) participants were analyzed. Dietary, lifestyle, and physiological variables used to build sleep disorder prediction models with random forest, extreme gradient boosting, light gradient boosting machine, and logistic regression. Model interpretability was assessed using Shapley additive explanations (SHAP). Key predict

Nutritional frailty and the incidence of depression and anxiety among middle-aged and older adults: A prospective cohort study.

BACKGROUND & AIMS: Frailty has been linked to an increased risk of mental disorders. However, little is known about the impact of nutritional frailty, a crucial phenotype of frailty, on depression and anxiety. We aimed to examine the prospective associations between nutritional frailty and the incidence and progression of depression and anxiety among middle-aged and older adults. METHODS: Data were obtained from a large prospective cohort study with over 500,000 participants. Nutritional frailty was defined as the copresence of physical frailty (assessed by weight loss, exhaustion, low physic

Does prolonged breast-feeding impair child growth? A critical review.

Recent epidemiologic evidence suggests that children in developing countries who terminate breast-feeding before their first birthday are less likely to be malnourished than those who are breast-fed for longer durations. This finding calls into question the advice women are given to breast-feed as long as possible. This review examines several studies that found a relationship between prolonged breast-feeding and malnutrition. Many studies have shown a negative association between prolonged breast-feeding and growth, but there is little reason to expect the association to be causal. Problems i
