BACKGROUND: NUT (nuclear protein in testis) carcinoma is a rare but aggressive cancer, which was first described in 1991. Given the rarity of this enigmatic entity, risk factors are unknown, treatment options are limited, and the prognosis is dismal. Nuclear protein in testis (NUT) carcinoma is a rare, highly aggressive malignancy defined by NUTM1 rearrangement, most commonly involving BRD4. Nonspecific presentation and poorly differentiated histology lead to frequent misdiagnosis. Patients often present with large mediastinal masses, making swift diagnosis both important and challenging. A 29-year-old woman presented with two months of progressive dyspnea, cough, and fatigue accompanied by new-onset hemoptysis. CT demonstrated a large right hilar and mediastinal mass compressing the trachea, pulmonary artery, and superior vena cava (SVC).

Nuclear protein in testis (NUT) carcinoma is a rare neoplasm arising mainly from midline structures. It is an aggressive type of carcinoma associated with poor survival despite the use of multiple treatment modalities. Here, we present a case of a 17-year-old paediatric patient with NUT carcinoma of larynx, which is even rarer among all reported cases. The patient underwent surgery followed by radiotherapy and systemic treatment and he died 15 months after the diagnosis. The management of this rare disease requires further investigation.

We report the case of a three-year-old girl with anaphylactic shock who presented with generalized erythema two hours after ingesting macadamia nuts. Areca nut use is culturally accepted, often initiated early in adolescence, and said to lead to later tobacco use.
