[Orbital tumors].

Orbital tumours include a variety of orbital diseases of different origins. In the case of malignant orbital tumours, early detection is important so that treatment can be initiated promptly. Neuroradiological imaging, in particular magnetic resonance imaging (MRI), plays an important role in the diagnostic of orbital tumours. In adults, lymphoproliferative diseases, inflammations and secondary orbital tumours are most frequently found, whereas in children mostly dermoid cysts, optic gliomas and capillary haemangiomas are found. Optic glioma is a pilocytic astrocytoma and accounts for two thir…

[Orbital infections].

Orbital cellulitis is a rare occurrence in ophthalmology practice. It is heterogeneous in terms of etiology, anatomical form and severity. Most often, it occurs adjacent to a sinus infection (frequently ethmoidal). Management must be systematized, adapted to the clinical examination and ancillary testing, and above all, proactive, since in certain situations, the functional and/or vital prognosis may be at stake. A clinical examination and urgent CT scan are essential for making the correct diagnosis, enabling us to assess the severity of the condition and determine whether surgery is required…

[Orbital Trauma].

Orbital trauma is a common medical emergency with potentially sight-threatening complications. Due to the confined orbital conditions, any direct injury or space-consuming lesion may cause serious tissue damage. Possible complications are orbital fractures, foreign body injuries, development of orbital compartment syndrome and traumatic optic neuropathy. Clinical signs include periorbital hematoma or emphysema, subconjunctival hemorrhage, proptosis, decreased ocular mobility, decreased vision, increased intraocular pressure and relative afferent pupillary defect. Ophthalmic status and low-dose…

Orbital Fractures.

Anatomic, rather than volumetric, reconstruction leads to improved outcomes in orbital reconstruction. Endoscopic visualization improves lighting and magnification of the surgical site and allows the entire operative team to understand and participate in the procedure. Mirror-image overlay display with navigation-guided surgery allows in situ fine adjustment of the implant contours to match the contralateral uninjured orbit. Precise exophthalmometry is important before, during, and after surgery to provide optimal surgical results.

[Orbital complications].

A disease or symptom of disease spreading from the vicinity of the orbit to the internal structures of the orbit is referred to as an orbital complication. Orbital complications can have a traumatic, inflammatory, allergic, or autoimmunologic cause. They are more frequent in children than adults. The present review aims to provide a description of orbital complications, their etiology, pathogenesis, and treatment. Recent literature in the field is acknowledged and discussed, and results from the authors' own patient groups are analyzed. Particular attention is paid to orbital complications due…

Orbital melanoma.

Primary or secondary orbital melanomas are extremely rare tumors; they represent less than 1% of primary orbital neoplasms. Over 90% of primary orbital melanomas arise from melanocytes (congenital ocular melanosis, oculodermal melanosis). In the Department of Ophthalmology, Comenius University, Bratislava, in period 2001-2007, a total number of 79 patients with uveal melanoma (dg. C69) were treated with radical surgery, combined techniques or isolated stereotactic radiosurgery, 45 (57%) patients with ciliary body and choroidal melanomas were treated with primary enucleation, 34 (43%) patients …

Orbital myiasis.

PURPOSE: To present a case of massive orbital myiasis. CASE REPORT: An 87-year-old debilitated woman suffering from left ocular pain of four days' duration presented with a severely necrotized left orbit and several attached live larvae. The upper and lower eyelids and the eyeball were completely destroyed. She had history of eyelid surgery in the same eye due to a skin lesion, apparently some type of skin cancer, 15 years before. The larvae were identified as Chrysomya bezziana (Diptera: Calliphoridae) or old world screwworm fly. CONCLUSION: Infestation of ocular and orbital tissues by fly …

Orbital pseudotumor.

Orbital pseudotumor is a benign condition that accounts for approximately 10% of all orbital mass lesions. Any part of the orbit may be involved. The etiology is unknown. The presentation may be either acute or subacute. Patients may present with a palpable mass, a swollen eyelid, congestion, pain, diminished ocular motility, and/or decreased visual acuity. Approximately, 25% of patients present with bilateral disease. A modest proportion of patients experience resolution of their symptoms without treatment. Biopsy is indicated for those who do not respond to, or relapse after, first-line ther…

Orbital metastases.

BACKGROUND: Orbital metastasis, although uncommon, is a condition optometrists should consider in a patient presenting with proptosis, ptosis, diplopia, or a lid mass with a history of cancer. However, in as many as 19% of cases, patients have no prior or concurrent history of systemic cancer when presenting with ophthalmic symptoms. If suspecting an orbital metastasis, neuroimaging is important, as well as a referral to the patient's primary care provider, oncologist, and ophthalmologist. CASE REPORTS: Three patients with orbital metastasis are discussed. The first was a 55-year-old white ma…

[Orbital metastasis].

Orbital metastases are uncommon, accounting for 4% of all adult orbital tumors. The mean age at time of presentation is 60 years. The primary tumor is most often a carcinoma involving the breast (40%), lung (11%), or prostate (8%). Proptosis is the main clinical sign at diagnosis and is often associated with visual impairment, pain or ptosis. Imaging features are not specific; nevertheless, an enhanced extraconal tumor with osteolysis suggests the diagnosis. From a pathological point of view, there are no differences between the metastasis and the primary tumor. Regarding the average survival,…

Orbital syndromes.

The orbit is a complex anatomical structure with unique properties not observed in other regions of the body. Composed of seven bones, the orbit is filled by the eye, optic nerve, lacrimal gland, extraocular muscles, peripheral motor and sensory nerves, fat, arteries, and veins. All these structures are intimately related to one another within an intricate framework of connective tissue. A variety of traumatic, vascular, inflammatory, infectious, and neoplastic processes can affect the orbit and its structures. Aside from the many primary orbital diseases, systemic disorders and pathological p…

Orbital inflammation.

Orbital inflammation is a common problem in adults and children, accounting for the majority of all orbital processes. The presentation may be acute, subacute, or insidious. When the onset is acute, the process can be mistaken for orbital cellulitis. In insidious cases, such as the sclerosing subtype of inflammation, the chronic painless course may prompt concerns about a neoplastic infiltration such as lymphoma. Orbital inflammation can be divided into nonspecific, idiopathic, and other specific diagnoses. The differential diagnosis includes allergic, infectious (fungal, mycobacterial, and pa…
