Pharyngitis and epiglottitis.

Acute pharyngitis is one of the most common illnesses for which patients visit primary care physicians. Most cases are of viral origin, and with few exceptions these illnesses are both benign and self-limited. The most important bacterial cause is the beta-hemolytic group A streptococcus. There are other uncommon or rare types of pharyngitis. For some of these treatment is required or available, and some may be life threatening. Among those discussed in this article are diphtheria, gonorrhea, HIV infection, peritonsillar abscess, and epiglottitis.

[Pustular diseases of the newborn].

In the newborn, there exists a wide spectrum of pustular skin diseases. These range from transitory, benign adaptation disorders up to systemic, life threatening illnesses. In 30-60% of newborns pustules are observed in association with the relatively harmless Erythema toxicum, the origin of which is still unknown today. It is necessary to differentiate this from the pustular diseases which may be of infectious or non-infectious nature and which require therapy. Typical pathogens include Malassezia furfur, Staphylococci, Streptococci, Candida spp. and the herpes virus group.

The National Lesbian Family Study: 2. Interviews with mothers of toddlers.

In this second report from a longitudinal study of lesbian families in which the children were conceived by donor insemination, interviews yielded the following data: Most couples shared parenting co-equally; the majority felt closer to their family of origin; adoptive co-mothers felt greater legitimacy as parents; biology and nurture received the same ratings for mother-child bonding; and political and legal action had increased among many participants. The impact of these findings and that of homophobia on lesbian family life are discussed.

Vertebral arteria lusoria: a life-threatening condition for oesophageal surgery.

A rare anatomic variant of the right vertebral artery is reported as an incidental finding in a 78-year-old female patient. The vessel originated from the aortic arch distal to the origin of the left subclavian artery and presented an unusual retroesophageal course, thus was named "vertebral arteria lusoria". As multi-detector row computed tomography examination is now widely used, this anatomical variant should be considered in any patient undergoing esophageal surgery. Misdiagnosis of such an aberrant vertebral artery may cause life-threatening issues.

[Physical and neurological state of the newborn after perinatal asphyxia].

Asphyxia neonatorum is the most prevalent birth trauma. Usually it leads to the birth of weak and nonvigorous child. Perinatal asphyxia is the reason of not only postasphyxic hypoxic-ischemic encephalopathy, but also multiple organ dysfunctions. Neurological sequelae and consequences of multisystem dysfunction affect on newborn status in first life months and later on by the muscle tone disturbances, physical and mental retardations. In this paper it is presented modern conceptions about asphyxia origin, risk factors, neurological and systemic complications for child nervous system and organis

Gastric volvulus: acute and chronic presentation.

Diaphragmatic hernia may be congenital or traumatic in origin. Traumatic hernia may menifest immediately or several months/years after the incident. Congenital hernia usually manifests in the early years of life. Diaphragmatic hernia may be complicated by gastric volvulus. Acute gastric volvulus is surgical emergency where as chronic gastric volvulus presents with nonspecific abdominal symptoms. Diagnosis of gastric volvulus is difficult and is based on imaging studies. We describe four cases of diaphragmatic hernia complicated by gastric volvulus, diagnosed on imaging and managed surgically.

Color Doppler examination for the diagnosis of subacute puerperal uterine inversion.

PURPOSE: Obstetric uterine inversion is a rare and life-threatening complication. Diagnosis is often difficult to establish, particularly in recurrent or chronic cases. METHOD: We performed color Doppler examination in addition to B-mode sonography in a case of subacute recurrent uterine inversion. RESULTS: Identification of the vessels providing uterine blood supply helped to clarify the distorted anatomy; furthermore, information about tissue viability was gained. CONCLUSION: We propose to perform color Doppler examination in all cases with suspected uterine inversion or vaginal masses of

Spontaneous Vertebral Artery Dissection in a Healthy 26 Year Old Female Patient: A Case Study.

Spontaneous vertebral artery dissection (SVAD) is an extremely rare, yet life-threatening, event that can potentially result in ischemic stroke or subarachnoid hemorrhage, depending on the origin and extension of the dissection. Vertebral artery dissection is more commonly associated with traumatic injury to the neck, resulting in compromised structural integrity of the vertebral artery wall. This case study discusses the clinical presentation, physical examination, diagnosis, clinical course, and outcome for a young, otherwise healthy, female patient who presented to the emergency department

[Small intestine atresia and abnormal insertion of the umbilicus in a child with fetal alcohol syndrome].

BACKGROUND: Fetal alcohol syndrome is quite common in our region (incidence: 1/700 live births). It usually associates facial dysmorphism, intra-uterine growth retardation and mental delay. CASE REPORT: Jonathan, born to an alcoholic mother, presented a typical dysmorphy of fetal alcohol syndrome and a low inserted umbilicus. A small bowel atresia was discovered at the third day of life and operated on. CONCLUSION: Although never been previously described, this association suggests a common embryological origin between fetal alcohol syndrome, small bowel atresia, and umbilical abnormality.

Elements of dynamics VI: the dynamic unconscious and unconscious dynamics.

The dynamic unconscious remains in contention in cognitive psychology. Academic settings may not provide the compelling evidence of unconscious dynamics regularly experienced by dynamic psychiatrists, psychoanalysts, and other physicians, including neurologists treating psychogenic motor disorders in the movement disorder clinic. An origin of unconscious motor expression is postulated in the intermodal connectivity of neuromental agencies, extending an idea that originated in Freud's (1950/1966) "Project for a Scientific Psychology." Motoric alexithymia is contrasted with synesthesia. Other ex

Review: fetal programming of polycystic ovary syndrome by androgen excess: evidence from experimental, clinical, and genetic association studies.

CONTEXT: Polycystic ovary syndrome (PCOS) is a common endocrine disorder of premenopausal women, characterized by hyperandrogenism, polycystic ovaries, and chronic anovulation along with insulin resistance and abdominal obesity as frequent metabolic traits. Although PCOS manifests clinically during adolescence, emerging data suggest that the natural history of PCOS may originate in intrauterine life. EVIDENCE ACQUISITION: Evidence from experimental, clinical, and genetic research supporting the hypothesis for the fetal origins of PCOS has been analyzed. EVIDENCE SYNTHESIS: Female primates, e

The ethical obligation of the dead donor rule.

The dead donor rule (DDR) originally stated that organ donors must not be killed by and for organ donation. Scholars later added the requirement that vital organs should not be procured before death. Some now argue that the DDR is breached in donation after circulatory determination of death (DCDD) programs. DCDD programs do not breach the original version of the DDR because vital organs are procured only after circulation has ceased permanently as a consequence of withdrawal of life-sustaining therapy. We hold that the original rendition of the DDR banning killing by and for organ donation is

The arterial switch procedure for transposition of the great arteries.

Transposition of the great arteries is the most common congenital heart defect among the birth defects that present with cyanosis during the early neonatal period. Infants with this cardiac birth defect, in which the aorta originates from the right ventricle and the pulmonary artery originates from the left ventricle, usually do not survive without surgical intervention in the first few days of life. The arterial switch procedure, performed via a median sternotomy incision during cardiopulmonary bypass, restores the aorta and pulmonary artery to their normal anatomic positions.

Separate origins of all three major coronary arteries from the right sinus of Valsalva: a rare coronary artery anomaly.

The origination of all three major coronary arteries from three separate ostia in the right sinus of Valsalva is an exceedingly rare coronary anomaly. Few radiographic and clinical details of this anomaly are available in the literature. We describe this anomaly in a patient with acute myocardial infarction who remained asymptomatic until the 8th decade of her life. Atherosclerotic narrowing of the normally originating right coronary artery caused the acute myocardial infarction. Anomalous left anterior descending and left circumflex arteries were free of any significant obstruction.

Hypoplastic left heart syndrome with a single coronary artery originating from the pulmonary artery.

A newborn male infant with hypoplastic left heart syndrome due to mitral and aortic atresia died on the third day of life. Autopsy revealed a single coronary artery originating from the pulmonary artery, reversed coarctation of the aorta, a coronary sinus type atrial septal defect and stenosis of the left subclavian artery. To our knowledge, hypoplastic left heart syndrome associated with a single coronary artery originating from the pulmonary artery has never been reported in the English or Japanese literature. This may be the first such case.

Maternal Supplementation of Probiotics, Prebiotics or Postbiotics to Prevent Offspring Metabolic Syndrome: The Gap between Preclinical Results and Clinical Translation.

Metabolic syndrome (MetS) is an extremely prevalent complex trait and it can originate in early life. This concept is now being termed the developmental origins of health and disease (DOHaD). Increasing evidence supports that disturbance of gut microbiota influences various risk factors of MetS. The DOHaD theory provides an innovative strategy to prevent MetS through early intervention (i.e., reprogramming). In this review, we summarize the existing literature that supports how environmental cues induced MetS of developmental origins and the interplay between gut microbiota and other fundament

Why is this journal called Protoplasma? A history of protoplasm theory and the divisions in cell biology before 1926.

When the journal Protoplasma was founded 100 years ago in 1926, scientists used two different concepts to describe what we would now consider one and the same object: the cell for the membrane-bound structural unit of life, and protoplasm for the living fluid mass or body of the cell. The flourishing of both concepts together dates back to the 1850s, when biologists revised the original cell theory of 1838/39 while also unifying the definition of the cell across plant and animal kingdoms. However, at the beginning of the 20th century a methodological debate over fixation and staining artifacts

Genetic and environmental variation in antibody and t-cell mediated responses in the great tit.

Host parasite coevolution assumes pathogen specific genetic variation in host immune defense. Also, if immune function plays a role in the evolution of life history, allocation to immune function should be heritable. We conducted a cross-fostering experiment to test the relative importance of genetic and environmental sources of variation in T-cell mediated inflammatory response and antigen specific antibody responses in the great tits Parus major. Cell mediated response was measured during the nestling period and antibody response against two novel antigens was measured in two-month-old juven

Embryonal malignancies of unknown primary origin in children.

BACKGROUND: Little is known about embryonal malignancies of unknown primary origin in children. All such cases referred to a pediatric cancer center over a 30-year period were reviewed to delineate their clinical and prognostic features and to develop recommendations for evaluation and therapy. METHODS: Seventeen patients with embryonal malignancies of unknown primary origin were identified by reviewing the institutional solid tumor database. The medical records, imaging studies, laboratory records, and pathology reports for each patient were reviewed and tabulated. RESULTS: Pathology review
