Posterior cervical fusion with rigid internal fixation.

Rigid fixation of the posterior cervical spine offers definite advantages over conventional wiring techniques. These advantages are emphasized in certain high-risk groups of patients, including those with multiple-level traumatic instability, cervical deformities, and unresectable anterior tumor involvement. Early fusion with minimal external fixation is another important advantage. The focus of this article is the concept of rigid internal fixation of the posterior cervical spine and the application of specific techniques used to achieve this goal.

Measuring Orthopedic Plate Strain to Track Bone Healing Using a Fluidic Sensor Read via Plain Radiography.

OBJECTIVE: We describe a fluidic X-ray visualized strain indicator under applied load (X-VISUAL) to quantify orthopedic plate strain and inform rehabilitative care. METHODS: The sensor comprises a polymeric device with a fluidic reservoir filled with a radio-dense fluid (cesium acetate) and an adjoining capillary wherein the liquid level is measured. A stainless-steel lever attaches to the plate and presses upon the acrylic bulb with a displacement proportional to plate bending strain. The sensor was attached to a plate in a Sawbones composite tibia mimic and a human cadaveric tibia. An osteo

Charcot Spinal Arthropathy Secondary to Spinal Cord Injury - A Case Report.

INTRODUCTION: Pressure ulcers are common complications in immobilized patients, typically occurring over bony prominences. However, in rare instances, particularly in high-risk individuals with hematologic disorders such as sickle cell disease, ulcers may develop at unusual anatomical sites due to external medical devices. This case highlights a rare presentation of a vulvar pressure ulcer caused by prolonged application of a Thomas splint in a patient with homozygous sickle cell disease (HbSS). CASE REPORT: A 24-year-old woman with HbSS was admitted with a closed distal femoral shaft fractur

[Low back pain and sciatica].

In order to see the patients with low back pain and/or sciatica, it is most important to consider the pathophysiology of symptoms. We should know that the image findings such as X-ray and MRI do not always show the cause of symptom. Because degenerative changes in lumbar spine are common findings in asymptomatic group compared to symptomatic patients with low back pain and sciatica. According to pathophysiology (nerve root syndrome, cauda equina syndrome, facet syndrome, discogenic pain syndrome and intermittent claudication for low back pain), the treatment for low back pain and sciatica were

Repair of distal biceps tendon rupture: a new technique using the Endobutton.

The authors describe a technique with a single anterior incision and fixation with an internal button, the Endobutton. The procedure is performed through a 5-cm transverse skin incision, and the tendon is sutured to the Endobutton with 2 number 5 Ethibond sutures. Surgical repair in the depths of the muscular forearm is not required, because the tendon is simply sutured external to the wound. The Endobutton delivers and locks the tendon into a hole in the radial tuberosity. The Endobutton technique was used in 12 patients who were allowed early active mobilization. All were satisfied, returned

Temporary Internal Fixation Using C1 Lateral Mass Screw and C2 Pedicle Screw (Goel-Harms Technique) without Bone Grafting for Chronic Atlantoaxial Rotatory Fixation.

BACKGROUND: The primary treatment strategy for chronic atlantoaxial rotatory fixation (chro-AARF) is traction followed by bracing or application of a halo device. However, to complete these conservative therapies, patient cooperation is mandatory. If conservative therapy fails, surgery is required for reduction and prevention of recurrence. It has been considered that surgery for atlantoaxial rotatory fixation necessitates solid bony fusion. However, once bony fusion is achieved, loss of range of motion is problematic. Here, we report a patient with chro-AARF who was successfully treated with

Arthrodesis for septic arthritis of the ankle: risk factors and complications.

INTRODUCTION: Septic ankle joint arthrodesis is a good therapeutic option in cases of infection after trauma or orthopedic surgical procedures. Many different procedures have been described, but external fixation seems to be standard. Aim of this study is to identify risk factors for complications in septic ankle joint arthrodesis with the external AO frame fixator. MATERIALS AND METHODS: Patients who received septic ankle joint arthrodesis between January 2008 and December 2012 were included in this study. Patients were evaluated clinically and with radiographs or CT scans. RESULTS: Follow-

A review on manufacturing processes of cobalt-chromium alloy implants and its impact on corrosion resistance and biocompatibility.

Cobalt-Chromium (CoCr) alloys are currently used for various cardiovascular, orthopedic, fracture fixation, and dental implants. A variety of processes such as casting, forging, wrought processing, hot isostatic pressing, metal injection molding, milling, selective laser melting, and electron beam melting are used in the manufacture of CoCr alloy implants. The microstructure and precipitates (carbides, nitrides, carbonitrides, and intermetallic compounds) formed within the alloy are primarily determined by the type of manufacturing process employed. Although the effects of microstructure and p

Wartime Soft Tissue Coverage Techniques for the Deployed Surgeon.

BACKGROUND: Since the start of the conflict in the Middle East in 2001, military orthopedic surgeons have faced complex orthopedic injuries including high-energy soft tissue wounds, traumatic amputations, and open fractures. Although orthopedic surgeons are well trained in the management of osseous injuries, the treatment of soft tissue injuries can be technically challenging and unfamiliar. Early washout, debridement of devitalized tissue, external fixation of bony injuries, and antibiotic therapy remain the foundation of early wound management. However, these unique extremity injuries have n

[Surgical treatment for disorders of the cervicothoracic junction region].

PURPOSE OF THE STUDY: The complex anatomy of the cervicothoracic junction region makes a reliable assessment of plain radiographs in lateral projection difficult or even impossible, which may result in failure to detect fracture or other pathology in this region of the spine. The aim of this study was to evaluate the patients with spinal disorders in the region of the seventh cervical to the third thoracic vertebrae treated at our department. MATERIAL: During the period from November 2001 to June 2004, 34 patients with disorders of the C7-T3 region were treated surgically at the Department of

Kluger's "fixateur interne" for spinal instability.

Kluger's "Fixateur Interne" proved to be an excellent tool not only in spinal trauma for repositioning of impacted fractures and transpedicular stabilization of the dorsal column but also in other forms of thoracic or lumbar instability. After spinal tumor excision from a dorsal approach and vertebral replacement with methylmethacrylate additional stability through dorsal fixation was achieved with this device. Spondylodiscitis, symptomatic spondylolisthesis, spinal instability from degenerative disc disease as well as "non-union" following previous surgery could be cured using Kluger's intern

A high-performance, biodegradable Zn-3Cu-0.8Sr alloy with superior strength, biotribology, osteogenesis, and antibacterial activity for orthopedic fixation through deep-cryogenic rolling.

Biodegradable Zn alloys have attracted considerable attention as candidates for load-bearing bone-fixation implants, yet simultaneously optimizing mechanical strength, corrosion-wear resistance, and multifunctional biofunctionalities remains challenging. Herein, a Zn-3Cu-0.8Sr (ZCS) alloy was successfully fabricated by a synergistic processing route that integrated hot rolling (HR) with deep-cryogenic rolling (DCR). The HR+DCR processing effectively refined coarse and brittle SrZn13 and primary ε-CuZn5 phases into uniformly dispersed, well-bonded fine reinforcements, while simultaneously promo
