The Effect of Time During the Academic Year or Resident Training Level on Complication Rates After Lower-Extremity Orthopaedic Trauma Procedures.

BACKGROUND: Few studies have evaluated the effect of resident participation on morbidity and mortality after orthopaedic trauma surgery. The goal of this study was to evaluate whether complications after orthopaedic trauma procedures involving residents correlate with the level of resident training and the timing in the academic year. METHODS: The American College of Surgeons National Surgical Quality Improvement Program database was queried for all patients who underwent operative fixation of proximal femoral fractures, femoral shaft fractures, and tibial shaft fractures from 2005 to 2012. Refixation techniques and approaches for distal biceps tendon ruptures: a systematic review of clinical studies.

BACKGROUND: Surgical fixation is the preferred method of treatment for the ruptured distal biceps tendon in active patients. To date, no fixation technique has been proven superior in a clinical setting. The purpose of the study was to systematically review the available literature on approach and fixation methods for distal biceps tendon repair in a clinical setting and to determine the optimal fixation methods of the distal biceps tendon on the radial tuberosity. Our hypothesis was that the outcomes would not be significantly different among the various fixation techniques and approaches. M

Cerebral palsy: the first three years.

The orthopedic surgeion should be an integral part of a medical team for evaluation and treatment of young children with cerebral palsy. Surgical procedures in this first three years of life are usually limited to the adductor releases about the hip. Stretching and plastic splints about the ankle and knee followed by ankle-foot orthoses are frequently effective in correction of deformity.

The therapeutic attitude in distal radial Salter and Harris type I and II fractures in children.

INTRODUCTION: Salter Harris Fractures type, especially type I and II are treated by orthopedic reduction in the emergency room or operating room, under general anesthesia, followed by plaster immobilization. Neglected or incorrectly treated fractures, leading to malunion and radiocarpal subluxations which require surgical procedure. PURPOSE: This paper proposes to evaluate the correctly applied orthopedic treatment and the expose of an original surgical technique in case of neglected and incorrectly treated fractures, leading to mal-unions and impediments in the radiocarpal mobility and aesth

Giant cell tumor arising in Paget's disease of bone. Recurrences after 36 years.

A 96-year-old woman had both Paget's disease and a giant cell tumor of the same bone, the left scapula. Despite five surgical procedures, a full course of radiation therapy, and treatment with salmon calcitonin, human calcitonin, and finally disodium etidronate, the tumor has recurred over a 36-year period. The characteristic viral-like nuclear and cytoplasmic inclusions found in the osteoclasts of Paget's disease were found in the tumor giant cells, in specimens excised over an eight-year period, and in specimens obtained during orthopedic procedures on her other pagetic bone lesions over 12 …

Laminoplasty for cervical myelopathy.

This article reviews cervical laminoplasty. The origin of cervical laminoplasty dates back to cervical laminectomy performed in Japan ~50 years ago. To overcome poor surgical outcomes of cervical laminectomy, many Japanese orthopedic spine surgeons devoted their lives to developing better posterior decompression procedures for the cervical spine. Thanks to the development of a high-speed surgical burr, posterior decompression procedures for the cervical spine showed vast improvement from the 1970s to the 1980s, and the original form of cervical laminoplasty was determined. Since around 2000, s

Development and validation of a specific learning system for minimally invasive ultrasound-guided techniques in Orthopedic Surgery and Traumatology.

INTRODUCTION: Ultrasound-guided minimally invasive techniques have experienced remarkable growth in Orthopedic Surgery and Traumatology. However, their learning process is complex, leading to the development of specific simulation models as educational tools. OBJECTIVE: To evaluate the effectiveness of a personalized phantom-based training program for acquiring skills in ultrasound-guided minimally invasive techniques. METHODS: A prospective comparative study was conducted with 20 orthopedic surgery residents, assigned to an intervention group (n=10; trained with phantoms) or a control group

The significance of infection related to orthopedic devices and issues of antibiotic resistance.

Over the last 15 years, with the advent of modern standards in the control of sterility within the operating room environment and adequate protocols of peri-operative antibiotic prophylaxis, the incidence of infections associated to orthopedic implants has become very low. Nevertheless, the event of infection still represents one of the most serious and devastating complications which may involve prosthetic devices. It leads to complex revision procedures and, often, to the failure of the implant and the need for its complete removal. In orthopedics, for the enormous number of surgical procedu

Paper #8: Cumulative Anesthesia Exposure in Patients Treated for Early Onset Scoliosis.

Anesthesia time was collected for 72 consecutive patients with early onset scoliosis (EOS). Patients averaged 26.8 hours of anesthesia, of which 64% came from orthopedic procedures. 42% of patients received greater than 3 hours of cumulative anesthesia prior to age 3, exceeding the FDA's recommendation. Non-idiopathic diagnosis and treatment other than bracing were statistically significant risk factors for greater cumulative anesthesia exposure.

Anesthesia management in a patient of Apert syndrome.

Apert syndrome is autosomal dominant disease associated with multiple craniofacial and limb deformities. These children usually face many orthopedic, orthodental operative procedures. As anesthetist, we face difficulties in airway management due to mid-facial hypoplasia, craniosynostosis. We report a case of Apert syndrome which was referred to us for syndactyly release, focusing on the difficulties and complications related to it.

Maintaining the balance: the critical role of plasmin activity in orthopedic surgery injury response.

The musculoskeletal system plays vital roles in the body, facilitating movement, protecting vital structures, and regulating hematopoiesis and mineral metabolism. Injuries to this system are common and can cause chronic pain, loss of range of motion, and disability. The acute phase response (APR) is a complex process necessary for surviving and repairing injured musculoskeletal tissue. To conceptualize the APR, it is useful to divide it into 2 distinct phases, survival and repair. During the survival-APR, a "damage matrix" primarily composed of fibrin, via thrombin activity, is produced to con

Single-Incision Endoscopic Trigger Finger Release.

Trigger finger surgery is a commonly performed open procedure with known potential complications of infection, stiffness, pain, nerve injury, bowstringing, and incomplete release of the A1 pulley. We present a novel single-incision endoscopic trigger finger release technique that moves the incision from the palm to the palmar-digital crease, leading to less pain, scarring, and stiffness. We believe that this technique is technically simple, fast, and may decrease the risk of complications commonly encountered with open trigger finger release. Level of Evidence: Therapeutic IV.

Complications Associated with the Surgical Management of Hallux Rigidus.

Hallux rigidus is the most common arthritic pathology of the foot. This review article discusses the pathophysiology and common clinical presentation of hallux rigidus. Furthermore, we discuss multiple classification systems that categorize the arthritic process and guide management. Surgical interventions include cheilectomy, Moberg osteotomy, synthetic cartilage implants, interpositional arthroplasty, and arthrodesis. The common complications are reviewed, and the rates of these complications highlighted. Surgical options for hallux rigidus globally have successful outcomes with low rates of
