OBJECTIVE: To evaluate interindividual anatomical variations of the epitympanum and the usable space for implantation of active middle ear implants (AMEI) as well as the usefulness of a tablet-based software to assess individual anatomy on computed tomography (CT) scans.
PATIENTS: CT scans of 126 patients, scheduled for cochlea implantation (50.8% men; 0.6-90.0 yr) without middle ear malformations or previous middle ear surgery and with slice thickness ≤0.7 mm were analyzed.
MAIN OUTCOME MEASURES: Since no standardized measurements to assess the size of the epitympanum are available, relevan

OBJECTIVE: Surgical treatment with bone conduction hearing implants and epitheses for ear malformations offer the right combination of hearing rehabilitation and cosmetic reconstruction. The surgical procedure is often performed in two-stage surgical steps. This project aimed to gain experience with a procedure in which the hearing implant and the epithesis anchors are inserted simultaneously.
MATERIAL AND METHODS: Four ears of three patients (nf = 1, nm = 2) with severe ear malformations (type III, according to Weerda) received a Bonebridge and an epithesis anchor with three base posts in on

BACKGROUND Here we present a case of Vibrant Soundbridge implantation in a 13-year-old girl with bilateral aural atresia of the external ear canal. In this instance, we attached the device's floating mass transducer (FMT) to a mobilizable complex of the incus and malleus, which functionally connected to the short process of the incus. CASE REPORT The article presents a case study of a patient with a congenital defect of the middle and external ear and conductive hearing loss, who was referred for middle ear implantation. Tonal audiometry revealed bilateral moderate to severe hearing loss with

OBJECTIVE: To describe a new stapedotomy in which is removed only the stapes head not the entire stapes superstructure and its long-term results.
STUDY DESIGN: Prospective study.
PATIENTS: The study was started on January 2015 and ended on September 2020. Seventy patients with otosclerosis were included in the study. They underwent stapedotomy from January 2015 to April 2016. The main follow-up for the study group was (5.1 yrs). The study group was divided into two groups (A and B) according the short hearing results (1 yr) and long hearing results (5.1 yrs). Group A including short hearing

BACKGROUND AND AIMS: As an alternative to fresh temporal bones, Thiel conserved specimens can be used in the study of ear mechanics. Conserved temporal bones do not decay, permit long-term experiments and overcome problems with limited access to fresh (frozen) temporal bones. Air conduction motion of the tympanic membrane (TM), stapes (ST) and round window (RW) in Thiel specimens is similar to that of fresh specimens according to reports in the literature. Our study compares this motion directly before and after conservation for the same specimens.
METHODS: The magnitude of motion of TM, ST a

OBJECTIVE: The objective of this study is to describe the clinical features, managements and outcomes of a rare coexistence of congenital ossicular anomaly and localized cholesteatoma. A literature review on these cases and each congenital disorder is also presented.
METHODS: A retrospective chart review was performed on patients diagnosed with congenital ossicular anomaly with concurrent localized cholesteatoma from 2008 to 2017. Clinical data of these patients were collected.
RESULTS: A total of 10 patients were identified. All patients presented with unilateral hearing loss. Pure-tone aud

Hearing implant technology is evolving at a rapid rate and more than ever patients with hearing loss are benefiting from these emerging hearing devices. Active middle ear implants are alternatives to hearing aids and bone conducting devices, offering patients an expanded range in improving their hearing. This chapter looks at the devices currently available, their indications and the literature regarding their outcomes.

Proteus syndrome (PS) is a rare hamartomatous disorder characterized by mosaic overgrowth of multiple tissues that manifests early in life and is progressive. The presence of unilateral external auditory canal exostoses in a patient who is not a swimmer or surfer is suggestive of PS. However, hearing loss is not a typical feature. Here, we describe exostoses and ossicular discontinuity with conductive hearing loss in a patient with PS. The treatment consisted of canalplasty and ossicular chain reconstruction. A postoperative reduction was demonstrated in the patient's air-bone gap, from 21 dB

HYPOTHESIS: The goal of this study was to compare the attachment of stapes prostheses with differently shaped loops to the long process of the incus.
BACKGROUND: In stapes surgery, the attachment of the prosthesis to the long process of the incus plays an important role concerning the gain in hearing and the development of late complications such as incus erosion and necrosis. Band-shaped and spiral loops have been developed to achieve a broad, firm attachment to the long process of the incus. During stapes surgery, the view at the prosthesis is restricted, making it impossible to evaluate th

BACKGROUND AND PURPOSE: Middle ear prostheses are among the smallest implanted medical devices, with diameters as small as 0.25 mm. Accurate postoperative imaging is critical for assessing prosthesis placement, detecting complications such as dislodgement or extrusion, and informing potential revisions. Conventional energy-integrating detector CT (EID-CT) is traditionally used for imaging these implants; however, with the recent clinical use of photon-counting detector CT (PCD-CT), further analysis on multiple parameters is required to assess future utility. This study compares the spatial res

BACKGROUND: Stapes surgery is performed to treat hearing loss in patients with otosclerosis. Erosion of the long process of incus can occur postoperatively and requires demanding revision surgery to further improve the hearing. The authors describe a novel method of performing revision stapes surgery where there is erosion of the long process of incus.
METHODS: A retrospective case series analysis of a database of revision stapes operations performed by one surgeon between July 2013 and December 2017 at a tertiary center was carried out. 26 cases were identified where there was significant er

CONCLUSIONS: Multislice computed tomography (MSCT) virtual endoscopy was useful in evaluating mainly post-traumatic and postoperative cases. In other pathological conditions axial images and MPR reformations were most useful.
OBJECTIVES: Evaluation of cross-sectional images, multiplanar 2-D reformations, 3-D reconstructions and virtual endoscopy (VE) in assessment of the middle ear in inflammatory diseases, trauma, otosclerosis and tumours. Comparison of each method and correlation with surgical findings.
MATERIALS AND METHODS: Investigations were carried out in 80 patients with middle ear p

The patient was a 52-year-old woman. She had been aware of her bilateral hearing loss since she was 20 years old. The hearing in her left ear started to deteriorate at the age of 49. Pure-tone audiometry showed a bilateral mixed hearing loss. The hearing levels for the right ear and the left ear were 52 dB and 68 dB, respectively. There were no remarkable findings in a computed tomography (CT) scan of the temporal bone. We suspected that she had otosclerosis, and an operation was performed on her left ear. When the incudostapedial joint (I-S joint) was exposed to investigate the movement of th

Existing mechanical and acoustic models of the human middle ear are used to investigate the expected performance of various middle ear reconstructive procedures. Although the results of these analyses generally are consistent with observed surgical results, such an analytic approach enables the formation of some simple rules for maximizing middle ear function after stapedectomy and tympanomastoid surgery. Model analyses predict the following: (1) stapes prostheses of 0.6 to 0.8 mm in diameter produce air-bone gaps of less than 12 dB; smaller diameter prostheses produce larger air-bone gaps; (2

Since 1988 we have routinely used prostheses made from teeth for ossicular reconstruction and have now done more than 400 implants. Short-time results with at least 1 year implantation were studied in a retrospective analysis of 319 cases with a review of 88 patients. Eighty percent of the implants were covered by an intact tympanic membrane, while in 10% the prosthesis was completely uncovered. This apparent perforation was not due to a material-related extrusion, but was caused by persistent ventilation dysfunction of the middle ear. This was associated with a progressive retraction of the t
