Antigranulocyte bone marrow scans in Paget's disease.

Paget's disease of bone is a disorder of bone remodeling in which excessive production of structurally abnormal bone occurs, with the primary and fundamental abnormality residing in the osteoclast. To evaluate further the jeopardized bone marrow in Paget's disease, the authors studied 29 skeletal lesions by means of conventional bone imaging and antigranulocyte antibody bone marrow imaging in eight consecutive patients affected by Paget's disease. Bone scans showed abnormal tracer uptake in the 29 skeletal sites (11 pelves, 10 vertebrae, 5 extremities, 2 skulls, and 1 rib). Antigranulocyte ant

Is the coned lateral lumbosacral junction radiograph necessary for radiological diagnosis?

The lumbar spine radiographs of 400 orthopaedic outpatients were reviewed to determine if the absence of the coned lateral lumbosacral junction radiograph would lead to a loss in diagnostic accuracy. In 84 (21%) patients the antero-posterior and lateral lumbar spine radiographs failed to visualize adequately the lumbosacral junction. In these patients the coned lateral lumbosacral junction radiograph resulted in 17 (4.5%) diagnoses which would otherwise have been missed. The diagnoses were spondylosis (11), spondylolisthesis (3), spondylolysis (1), Paget's disease (1) and infection (1). Absenc

The diphosphonate space: a useful quantitative index of disease activity in patients undergoing hydroxy methylene diphosphonate (HMDP) bone imaging for Paget's disease [corrected].

Comparison between the plasma levels of intravenously injected technetium 99m hydroxy methylene [corrected] diphosphonate (99mTc-HMDP) and chromium 51 ethylene diamine tetra-acetic acid (51Cr-EDTA) reflects the uptake of diphosphonate into bone (the diphosphonate space). This can be used as an index of skeletal function in metabolic bone disease. In a series of 49 patients with Paget's disease the diphosphonate space (DPS) correlated well with other indicators of disease activity such as alkaline phosphatase and urinary hydroxyproline levels. The DPS is a good predictor of the volume of skelet

Sarcoma in Paget disease of bone: clinical, radiologic, and pathologic features in 22 cases.

The clinical, radiologic, and histologic features of 22 cases of Paget sarcoma were reviewed to determine in which patients with Paget disease these tumors are most likely to develop and what radiologic findings suggest the diagnosis. Clinical findings at presentation included pain and/or a mass (11 patients), pathologic fracture (seven), and neurologic symptoms (four). Survival time in 20 patients ranged from 5 days to 2.5 years. Two patients were lost to follow-up: one at 2 years and one at 8 years. There were 16 high-grade osteosarcomas, three chondrosarcomas, two fibrosarcomas, and one mal

Low dose intravenous 3-amino-1-hydroxypropylidene-1,1-bisphosphonate (APD) for the treatment of Paget's disease of bone.

Twenty patients with severe symptomatic Paget's disease were treated with a series of 15 mg intravenous infusions of 3-amino-1-hydroxypropylidene-1,1-bisphosphonate (APD). A regimen of either five consecutive days of treatment (regimen 1) or a course of 12 weekly infusions was administered (regimen 2). In five cases regimen 2 followed regimen 1 after a three month interval. Alkaline phosphatase levels fell in all patients and returned to the normal range in 12. All but one of the patients obtained symptomatic improvement. There was a median fall in alkaline phosphatase activity of 63%. Eight p

Paget's disease of the bone presenting as an apparent oral antral fistula.

Paget's disease of the bone is a chronic, slowly progressive condition commonly evidenced by bowing of the legs and enlargement of the skull. Its lesions are usually multiple and result in thickened but weakened areas within the involved bones. A case of Paget's disease presented as an apparent oral antral fistula. The patient also exhibited other signs and symptoms of the disease.

Optic neuropathy in Paget's disease.

Of 22 patients (18 men and four women ranging in age from 50 to 89 years) with radiographic and clinical evidence of Paget's disease, nine had visual field defects. All nine had arcuate scotomas and five of the nine had generalized constriction. The visual field changes were asymptomatic in six of nine patients and progressive in two patients. Only two patients had radiographic evidence of optic canal constriction by bony impingement. There was no objective improvement in the optic neuropathy in the three patients treated with synthetic salmon calcitonin. Our data suggested that the optic neur

Protrusio acetabuli: radiographic appearance in arthritis and other conditions.

Normal radiographic anatomic relationships, as well as distortion of those relationships in protrusio acetabuli, have been described. The occurrence of this deformity in arthropathies is stressed and other entities are mentioned and illustrated. Standard anteroposterior hip or pelvis radiographs readily permit the detection of protrusio acetabuli.

High-resolution computerized tomography of the petrous bone.

High-resolution computerized tomography (CT) has been used to investigate abnormalities of the ear and petrous bone. Congenital, traumatic, infective and neoplastic lesions have been evaluated in 30 patients with proven pathology. Some bone changes including abnormalities of the auditory ossicles have been better demonstrated than by conventional tomographic technigues, and high-resolution CT has some advantages in demonstrating both soft tissue abnormality and bone changes in the middle ear in primary neoplastic disease and secondary cholesteatoma. The absence of any practical means of taking

[X-ray morphology of osteodystrophia deformans Paget (author's transl)].

Osteodystrophia deformans Paget is a frequent and localised disease of the skeleton in elderly persons. The x-ray morphology of the disease is not uniform. It depends on the site at the skeleton, on the design of the affected bone, and on the stage and course of the disease. Basing on 71 own observations, the various types of manifestation are discussed and both their characteristic and non-typical pattern explained. Attention is drawn to the complications which can result from involvement of the individual parts of the skeleton.

Skull x-ray in the evaluation of endocrine diseases.

The skull x-ray remains invaluable as a first step in the diagnosis of many endocrine disorders. Pituitary tumors commonly cause enlargement or distortion of the sella turcica, which can be seen on skull x-ray. Pinealomas and craniopharyngiomas may also be detected. Hyperparathyroidism can cause granular decalcification in the skull. Hypoparathyroidism produces calcification of the basal ganglia and cerebellum. Lesions due due to metastatic malignancies or eosinophilic granulomas may be noted in cases of diabetes insipidus. Sellar enlargement may be due to the "empty sella" syndrome. Twenty-fi

Some unusual features of Paget's disease of bone.

The usual features of Paget's disease of bone, e.g. large head, deafness due to involvement of the 8th cranial nerve and bowing of the legs are all well described in text books of medicine. However, some features such as platybasia with neurological deficit, involvement of the 2nd, 5th and 7th cranial nerves, hydrocephalus, dementia, dysphasia, corrugation of the skull, bone pain (with or without associated osteo-arthrosis of hips and knees), heart failure, triradiate deformity of the pelvis have either had an inadequate or no description in books. These features are unusual, but it is importa

Intestinal absorption of 47Ca in elderly patients with osteoporosis, Paget's disease and osteomalacia. Effects of calcitonin, oestrogen and vitamin D2.

The intestinal absorption of 47Ca was studied in elderly patients. A standard dose of 10 muCi of 47Ca was given orally. The radioactivity was measured in the plasma, and expressed as percentage of the administered dose per litre plasma. As a control group served 12 patients aged 60-80 years, hospitalized for observation for various reasons, receiving no medical treatment and not suffering from any known metabolic bone diseases or other metabolic pathological conditions. Results of kinetic curves demonstrate in elderly patients a decreased absorption with maximum specific activity in plasma rea

Spinal cord compression in Paget's disease.

Three cases of spinal cord compression secondary to Paget's disease of the spine are reported. In two of the cases a relatively short history with pain as a prominent feature suggested initially a diagnosis of extradural malignancy. The usual clinical features of this rare disorder are discussed and the characteristic radiological findings are emphasized.
