[Pars plana vitrectomy with extraction of intraocular foreign body in patients with siderosis of the eye: report of two cases].

INTRODUCTION: Penetrating injury is characterized by the existence of entry wound only, and it can be with or without an intraocular foreign body (IOFB). IOFB can lead to a mechanical injury of the eye and to cause infection or to manifest other toxic effects on intraocular structures. Iron and copper can dissolve and cause siderosis, i.e. chalcosis of the eye. Ocular siderosis is diagnosed by clinical and electroretinogram (ERG) findings. OUTLINE OF CASES: The first patient was a 37-year-old male who was injured by a metal foreign body. He presented at the Clinic two years after the injury.

Occult Spontaneous Ocular Perforation Presenting as Conjunctival Chemosis in a Patient with Marfan's Syndrome.

We report a case of occult spontaneous ocular perforation presenting as conjunctival chemosis in a patient with Marfan's syndrome (MFS). A 38-year-old female with MFS presented with bilateral conjunctival chemosis since 6 months. Best-corrected visual acuity was 20/20 in both eyes. On slit-lamp examination, a diffuse conjunctival chemosis was observed in both eyes without any signs of ocular hypotony (decreased visual acuity, low intraocular pressure, shallow anterior chamber, pupil distortion, hypotony maculopathy, and chorioretinal folds). Anterior-segment optical coherence tomography reveal

Early visual-evoked potential acuity and future behavioral acuity in cortical visual impairment.

PURPOSE: Cortical Visual Impairment (CVI) is bilateral visual impairment caused by damage to the posterior visual pathway. Both preferential looking and sweep visual-evoked potential (VEP) can be used to measure visual acuity. The purpose of this study was to determine if an early VEP measure of acuity is related to a young patient's future behavioral acuity. METHODS: The visual acuity of 33 patients with CVI was assessed using the sweep VEP and a behavioral measure on two occasions. The median age of the patients at the initial visit was 4.8 years (range: 1.3 to 19.2 years), and they were fo

Visual outcome in high hypermetropia.

INTRODUCTION: We wished to determine whether final visual acuity is dependent on age at optical correction or presence of esotropia in children with bilateral high hypermetropia. METHODS: We reviewed the charts of all patients at Childrens Hospital Los Angeles Division of Ophthalmology with bilateral hypermetropia of greater than or equal to 5D who were able to provide objective visual acuity outcomes with Snellen letters or linear E. RESULTS: One hundred thirteen patients met entry criteria. The age at first optical correction ranged from 8 months to 141 months (average 45 months). Initial

Comparison of the Storz bifocal zonal and the 3M diffractive multifocal intraocular lenses.

Visual acuity and contrast sensitivity after cataract surgery were evaluated in best-case eyes that received a Storz bifocal zonal intraocular lens (IOL) or a 3M diffractive multifocal IOL. A group of eyes with a monofocal IOL was used as a control. Postoperatively, all eyes had a best corrected visual acuity of 20/40 or better. At near, wearing the best distance correction, 86.0% of the Storz group and 94.5% of the 3M group had a visual acuity of 20/40 or better. Both the Storz and 3M groups had a significantly greater loss in visual acuity in dim light at the lowest contrast levels than the

Aberrometric outcome of customized toric IOL in cataract with very high corneal astigmatism due to pellucid marginal degeneration.

A case of pellucid marginal degeneration (PMD) with cataract where a customized high-power toric IOL was implanted is reported. The patient had preoperative corneal astigmatism of 13.96 diopters (D), and a customized toric IOL of 19 D toricity was implanted; postoperative visual acuity was 6/9 on Snellen chart. The visual quality and aberrometric outcome were assessed with a ray-tracing aberrometer. The toric IOL gave not only a better uncorrected visual acuity but also better quality of vision in this patient with corneal astigmatism due to PMD.

Photovoltaic Restoration of Central Vision in Atrophic Age-Related Macular Degeneration.

PURPOSE: Loss of photoreceptors in atrophic age-related macular degeneration results in severe visual impairment, although some peripheral vision is retained. To restore central vision without compromising the residual peripheral field, we developed a wireless photovoltaic retinal implant (PRIMA; Pixium Vision, Paris, France) in which pixels convert images projected from video glasses using near-infrared light into electric current to stimulate the nearby inner retinal neurons. DESIGN: We carried out a first-in-human clinical trial to test the safety and efficacy of the prosthesis in patients

Traumatic graft dehiscence after anterior lamellar keratoplasty.

A 64-year-old man underwent anterior lamellar keratoplasty using the Melles technique for corneal scar secondary to healed bacterial keratitis in the left eye. Two months postoperatively, the patient presented with blurry vision in the left eye after a fall in the bathroom. The uncorrected visual acuity was 20/400 in the left eye, and the external examination showed periorbital swelling with ecchymosis. Slit-lamp examination revealed an absent graft, a circular crater in the center, and a fragment of suture on the temporal edge of the crater. The Seidel test was negative, and there was no sign

Subperiosteal hematoma and orbital compression syndrome following minor frontal trauma in sickle cell anemia: case report.

We report the case of an 11-year-old girl with sickle cell disease who presented to the emergency room after being hit by a mud pie in the left frontal region. Examination evidenced left eye proptosis, eyelid swelling, reduced visual acuity and afferent pupillary defect, without any inflammatory signs such as fever, hyperemia or tenderness. Computed tomography of the orbits showed a large superomedial subperiosteal hematoma in the left orbit. The patient was treated with canthotomy, cantholysis and surgical draining of the hematoma. Two days after drainage she persisted with a subperiosteal he

Meta-analysis of accommodating intraocular lenses.

Accommodating intraocular lenses (IOLs) based on the concept of optic shift were introduced to restore accommodation after cataract surgery. Currently, 3 types of accommodating IOLs are commercially available: 1CU (HumanOptics), BioComFold (Morcher), and AT-45 Crystalens (eyeonics, Inc.). We present a meta-analysis of the peer-reviewed data from studies of these IOLs that use optic-shift measurements and visual acuity as the main outcome measures. In the 6 randomized controlled studies, 5 of which studied the 1CU IOL, the visual acuity results showed moderate to no improvement in near visual a

Treatment of subfoveal choroidal neovascularization secondary to choroidal nevus using photodynamic therapy.

A 52-year-old man sought treatment for decreased vision in his right eye for 3 weeks. Visual acuity was 20/120. Examination revealed an elevated choroidal nevus at the posterior pole. Fluorescein and indocyanine green angiographies disclosed fibrovascular pigment epithelial detachment extending under the fovea. The patient was treated by two sessions of photodynamic therapy with verteporfin. Visual acuity improved to 20/40. The fibrovascular pigment epithelial detachment area diminished and the fovea is clear of lesion. Seven months after the last treatment, visual acuity remains stable. Photo

Ocular injuries associated with periorbital fractures.

The incidence of serious ocular injuries found in association with periorbital fractures, 10%, was determined through a retrospective review of 230 patient records. Eye injury was most frequently in conjunction with supraorbital fractures. A brief survey of the diagnosis and treatment of the injuries identified, loss of visual acuity, global laceration or rupture, retinal contusion, intraocular hemorrhage, glaucoma, and optic nerve damage, is presented, with special emphasis on the importance of the acute physical examination of the eye and determination of the visual acuity.
