Some recent trends and further promising directions in functional neurosurgery.

The field of functional neurosurgery has developed a number of recent innovative neuromodulatory approaches to treat disease that remains resistant to the best medical therapy. These include novel surgical techniques to intervene in motor and cognitive sequelae of refractory epilepsy, neurodegenerative disease, and certain psychiatric conditions. To a large extent, much of the innovation in our field continues to be driven by a systems-level understanding of the impact of disease on the brain. For example, several groups have exploited findings from neuroimaging work to identify a number of ne

Conducting clinical trials in a constantly changing health care environment.

Continuous change and cost containment characterize the current health care system, making conduct of clinical trials and other clinical research difficult. Identification, accrual, and follow-up of patients who move between health care environments such as hospitals, nursing homes, schools and the home is particularly challenging. This article describes a circuit rider approach to patient identification and follow-up that was established by the Communication Sciences and Disorders Clinical Trials Research Group. It also gives suggestions for design of clinical trials in a constantly changing

Role of menopause in the development of cardiovascular disease in women.

Cardiovascular disease (CVD) is the most common cause of death in women. Early identification of CVD risk factors in women is often overlooked by physicians leading to delays in diagnosis and treatment of CVD with subsequent morbidity and mortality. There may be a correlation between declining hormone levels during menopause and the prevalence of CVD during midlife. Alterations in the CVD risk profile of women throughout the menopause transition (MT) are multifactorial and include changes to arterial stiffness, endothelial function, lipid metabolism, insulin resistance and adipose fat distribu

Postoperative radiotherapy delineation after mandibular bone flap reconstruction in oral cavity and oropharyngeal cancers.

BACKGROUND: Postoperative radiotherapy is a cornerstone of management for locally advanced oral cavity and oropharyngeal cancers. Increasing use of vascularised mandibular bone flaps and patient-specific fixation systems has substantially modified postoperative anatomy, thereby complicating target volume delineation. While consensus guidelines exist for soft tissue flaps, no practical framework specifically addresses mandibular bone flap characterisation and delineation in poRT planning. We aimed to provide multidisciplinary, imaging-based recommendations for bone flap delineation on planning

Personalized neoantigen mRNA vaccines for pancreatic cancer: the role of lipid nanoparticle delivery systems.

BACKGROUND: Pancreatic ductal adenocarcinoma (PDAC) is an aggressive malignancy characterized by late diagnosis, frequent recurrence, limited treatment options, and an immunosuppressive tumor microenvironment. These features have encouraged the development of personalized approaches targeting tumor-specific neoantigens. OBJECTIVE: This review discusses the development and therapeutic potential of personalized neoantigen mRNA vaccines for PDAC, with particular emphasis on lipid nanoparticle (LNP) delivery systems and their role in shaping antitumor immunity. METHODS: Published literature on P

Inhibition of glycogen synthase kinase 3 by lithium, a mechanism in search of specificity.

Inhibition of Glycogen synthase kinase 3 (GSK3) is a popular explanation for the effects of lithium ions on mood regulation in bipolar disorder and other mental illnesses, including major depression, cyclothymia, and schizophrenia. Contribution of GSK3 is supported by evidence obtained from animal and patient derived model systems. However, the two GSK3 enzymes, GSK3α and GSK3β, have more than 100 validated substrates. They are thus central hubs for major biological functions, such as dopamine-glutamate neurotransmission, synaptic plasticity (Hebbian and homeostatic), inflammation, circadian r

The Partnerships for Care Project in Massachusetts: Developing Partnerships and Data Systems to Increase Linkage and Engagement in Care for Individuals Living With HIV.

BACKGROUND: Before implementation of the Massachusetts Partnerships for Care (MA P4C) project, Massachusetts did not routinely provide HIV partner services to newly diagnosed individuals. Identification of individuals disengaged from HIV care and assistance for re-engagement relied on community- and clinic-based HIV medical case management services. Processes to identify individuals out of HIV care used either surveillance or clinic data, but did not combine both. METHODS: The Massachusetts Department of Public Health collaborated with 6 community health centers to implement HIV partner servi

Streptococcus agalactiae sepsis after transfusion of a plateletpheresis concentrate: benefit of donor evaluation.

BACKGROUND: Bacterial contamination of platelet (PLT) components is an important cause of transfusion reactions. Recent efforts have focused on heightened surveillance to detect contamination before transfusion to limit recipient morbidity and mortality. Although identifying the cause of contamination is most often viewed in the context of recipient safety, this case illustrates the importance of a thorough evaluation on donor safety. CASE REPORT: A 68-year-old woman experienced a severe febrile reaction after a plateletpheresis transfusion. Blood cultures from the patient and from the platel

Paracoccidioides brasiliensis PS2: First Autochthonous Paracoccidioidomycosis Case Report in Rio de Janeiro, Brazil, and Literature Review.

Paracoccidioidomycosis (PCM) is a systemic mycosis caused by pathogenic dimorphic fungi of the Paracoccidioides brasiliensis complex. It is the most important systemic mycosis in Latin America, mainly in Brazil. Despite its severity and high mortality rates, it is considered a neglected disease. Species within the genus Paracoccidioides present genetics and morphological variations with probable clinical, diagnostic and therapeutic consequences. In fact, there are a very small number of detailed case reports with molecular identification of these fungal agents. Here, it is reported a case of P

Case report: Francisella philomiragia bacteremia in a patient with acute lymphoblastic leukemia.

Francisella philomiragia is a Gram-negative coccobacillus, which is a very rare human opportunistic pathogen causing pneumonia and systemic infection. It is difficult to identify this bacterium through conventional Gram-staining and biochemical methods due to an amorphous Gram stain appearance after 24 h culture and its relatively fastidious and slow growth giving weak and/or delayed reactions in biochemical tests. It is often misidentified as other bacteria including Haemophilus spp., Pseudomonas aeruginosa, or Sphingomonas paucimobilis. False identification may delay the therapy of the patie

Establishing the protocols for the South Australian Emergency Department Admission Blood Psychoactive Testing (EDABPT) programme for drug surveillance.

OBJECTIVE: ED presentations because of illicit use of psychotropic drugs and pharmaceuticals result in significant medical harm and resource consumption. Patient assessment is complicated by the regular emergence of new psychoactive substances, difficulties associated with their identification and a lack of information about their effects. Here we report the protocol for the Emergency Department Admission Blood Psychoactive Testing (EDABPT) programme, an observational study utilising clinical data capture and definitive drug identification to assess the medical impact and patterns of illicit d

Whipple's disease with isolated central nervous system symptomatology diagnosed by molecular identification of Tropheryma whippelii in peripheral blood.

We report a new case of Whipple's disease (WD) confined to the central nervous system. The patient presented with ataxia, ophthalmoplegia, hypersomnia, hemiparesis and generalized myorhythmia. The diagnosis was confirmed by identification of specific sequences of the causal agent of WD, the actinobacteria Tropheryma whippelii (TW), by PCR of DNA extracted from peripheral blood. An epidemiological survey of TW in patients with dementia suggests that WD is an uncommon cause of dementia in our population. Molecular methods may allow rapid identification of TW in peripheral fluids, and non-invasiv

The utility of a shortened palliative care screening tool to predict death within 12 months - a prospective observational study in two south African hospitals with a high HIV burden.

BACKGROUND: Timely identification of people who are at risk of dying is an important first component of end-of-life care. Clinicians often fail to identify such patients, thus trigger tools have been developed to assist in this process. We aimed to evaluate the performance of a identification tool (based on the Gold Standards Framework Prognostic Indicator Guidance) to predict death at 12 months in a population of hospitalised patients in South Africa. METHODS: Patients admitted to the acute medical services in two public hospitals in Cape Town, South Africa were enrolled in a prospective obs

Morphological analysis of three-dimensionally reconstructed frontal sinuses from Chinese Han population using computed tomography.

The uniqueness and reliability of frontal sinuses for personal identification have gained wide recognition in forensics. However, few studies have assessed the usefulness of a three-dimensional (3D) model of the frontal sinus for human identification. This study aimed to develop standardized techniques to classify the frontal sinus according to its 3D morphological metrics and discover the usefulness of the 3D frontal sinus model in identification of Chinese Han population. One hundred and ninety-six computed tomography (CT) scans of patients older than 20 years (84 males and 112 females) were

Artificial intelligence-enabled cross-scale integration of traditional Chinese medicine and biomedicine for sepsis: from mechanisms to delivery.

Sepsis is an organ dysfunction caused by a dysregulated host immune response to infection. Its pronounced heterogeneity and cross-scale pathological disturbances have resulted in poorly defined core therapeutic targets and inefficient drug delivery. Artificial intelligence, with its capacity for high-dimensional data integration, can serve as a data-integrative and hypothesis-generating tool, offering new avenues for exploring potential solutions to the aforementioned bottlenecks. This review summarizes recent advances in AI-driven multi-omics-based mechanistic dissection, the use of nanodeliv

Artificial intelligence-assisted endoscopic diagnosis of esophageal squamous cell carcinoma.

Esophageal squamous cell carcinoma (ESCC) remains a major cause of cancer-related mortality, and prognosis depends strongly on detection at a curable stage. Endoscopy is central to screening and diagnosis, but subtle flat lesions, operator dependence, cognitive fatigue, lesion-location blind spots, and variability in interpretation contribute to missed or delayed diagnosis. Artificial intelligence (AI), particularly deep learning applied to white-light imaging, narrow-band imaging, blue-light imaging, magnifying endoscopy, Lugol chromoendoscopy, video endoscopy, and endocytoscopy, has shown cl

A multicenter prospective observational multi-omics study protocol to identify biomarkers for severe dengue: COMBAT study clinical protocol.

BACKGROUND: Dengue virus (DENV) infection poses a major global public health burden, particularly in endemic regions where repeated exposure increases the risk of severe disease. Despite revisions to the World Health Organization (WHO) dengue classification, early prediction of progression to severe dengue remains challenging due to overlapping clinical and laboratory features. Current management strategies rely primarily on supportive care and reactive monitoring, underscoring the need for predictive biomarkers that enable early risk stratification and timely intervention. METHODS: COMBAT is

Periodic and Aperiodic Components of Subthalamic Nucleus Activity Reflect Different Aspects of Motor Impairment in Parkinson's Disease.

Excessive beta oscillations in the subthalamic nucleus are established as a primary electrophysiological biomarker for motor impairment in Parkinson's disease and are currently used as feedback signals in adaptive deep brain stimulation systems. However, there is still a need for optimization of stimulation parameters and the identification of optimal biomarkers that can accommodate varying patient conditions, such as ON and OFF levodopa medication. The precise boundaries of 'pathological' oscillatory ranges, associated with different aspects of motor impairment, are still not fully clarified.
