Management of invasive cervical resorption: observations from three private practices and a report of three cases.

Invasive cervical resorption (ICR) is a type of external resorption that is not well understood or well known in the dental community. It is often misdiagnosed, leading to improper treatment or unnecessary loss of the tooth. Treatment may involve the periodontium as well as the tooth and pulp, and management can be complex. Early diagnosis and appropriate treatment are the keys to a successful outcome. This article discusses the decision-making process and management of ICR, with emphasis on the restorative aspects of treatment. Three treatment cases are presented that include nonsurgical and …

[Effects of periodontal disease and its treatment on the condition of the pulp].

Since there is a close relationship between the periodontium and the pulp, formed by the apical foramen, dentinal tubules, lateral and accessory canals, it is evident that both compartments influence one other. In this article, the influence of periodontal disease and its treatment on the dental pulp is described. The review shows that periodontal disease and its treatment, very seldom compromise the vital functions of the pulp, but can cause local pulpal response. A more common reaction is dentinal hypersensitivity. Although there's no clear treatment for dentinal hypersensitivity, this artic…

Comparative evaluation of cathepsin K levels in gingival crevicular fluid among smoking and nonsmoking patients with chronic periodontitis.

BACKGROUND: The aim of the study is to comparatively evaluate the levels of cathepsin K (CSTK) in gingival crevicular fluid (GCF) among smoking and nonsmoking patients with chronic periodontitis (CP). MATERIALS AND METHODS: A total of 160 systemically healthy male patients were included in the study. Based on probing pocket depth, clinical attachment level, plaque index, and modified sulcular bleeding index, the patients were allotted into four groups: Group I - with forty subjects who were smokers with healthy periodontium, Group II - with forty nonsmoking subjects with healthy periodontium,…

Distinct Oral Neutrophil Subsets Define Health and Periodontal Disease States.

Neutrophils exit the vasculature and swarm to sites of inflammation and infection. However, these cells are abundant in the healthy, inflammation-free human oral environment, suggesting a unique immune surveillance role within the periodontium. We hypothesize that neutrophils in the healthy oral cavity occur in an intermediary parainflammatory state that allows them to interact with and contain the oral microflora without eliciting a marked inflammatory response. Based on a high-throughput screen of neutrophil CD (cluster of differentiation) marker expression and a thorough literature review, …

Gingival crevicular fluid proteomes in health, gingivitis and chronic periodontitis.

OBJECTIVE: The aim of this study was to compare the proteome composition of gingival crevicular fluid obtained from healthy periodontium, gingivitis and chronic periodontitis affected sites. BACKGROUND: Owing to its site-specific nature, gingival crevicular fluid is ideal for studying biological processes that occur during periodontal health and disease progression. However, few studies have been conducted into the gingival crevicular fluid proteome due to the small volumes obtained. METHODS: Fifteen males were chosen for each of three different groups, healthy periodontium, gingivitis and c…

Wnt/β-catenin pathway regulates cementogenic differentiation of adipose tissue-deprived stem cells in dental follicle cell-conditioned medium.

The formation and attachment of new cementum is crucial for periodontium regeneration. Tissue engineering is currently explored to achieve complete, reliable and reproducible regeneration of the periodontium. The capacity of multipotency and self-renewal makes adipose tissue-deprived stem cells (ADSCs) an excellent cell source for tissue regeneration and repair. After rat ADSCs were cultured in dental follicle cell-conditioned medium (DFC-CM) supplemented with DKK-1, an inhibitor of the Wnt pathway, followed by 7 days of induction, they exhibited several phenotypic characteristics of cementobl…

Periodontal status and bacteremia with oral viridans streptococci and coagulase negative staphylococci in allogeneic hematopoietic stem cell transplantation recipients: a prospective observational study.

AIM: This study was aimed to investigate whether any association could be found between the presence of an inflamed and infected periodontium (e.g., gingivitis and periodontitis) and the development of bacteremia during neutropenia following allogeneic hematopoietic stem cell transplantation (HSCT). METHODS: Eighteen patients underwent a periodontal examination before HSCT. Patients were classified as periodontally healthy [all periodontal pocket depths (PPD) ≤ 4 mm and bleeding on probing (BOP) ≤ 10%) or as having gingivitis/periodontitis (PPD ≥ 4 mm and BOP > 10%]. Oral mucositis (OM) was s…

Pediatric Odontogenic Infections.

Odontogenic infections are a broad group of head and neck conditions that arise from the teeth and surrounding periodontium. These largely preventable infections disproportionately affect members of ethnic and racial minorities and low-income/uninsured groups, and result in significant costs to our health care system. Left untreated, odontogenic infections can spread to deep spaces of the head and neck and can result in life-threatening complications. The mainstay of treatment includes timely treatment of the affected teeth. These infections are a global public health concern that could be dim…

Paediatric Orthodontics Part 3: Masticatory function during development.

The importance of masticatory function during development, the aim of early therapies. Hypoplasia of the upper maxilla is a widespread condition which orthodontists often need to face during early treatment. Mechanical maxillary expansion (both rapid and slow) produce undiagnosed effects on the palatal suture and unwanted side- effects on the periodontium and alveolar process, as well as anchorage-teeth displacement, root resorption, bone fenestration. It is clear that early therapies need a non-mechanical and non-traumatic approach in the respect of the physiology and biology of one of the mo…

Gingival recession--can orthodontics be a cure?

Does orthodontic treatment help or hinder a patient's periodontal status? What factors affect the periodontium? Can those factors be managed in a way that remedies existing periodontal issues? A 35-year-old woman presented with severe gingival recession and a unilateral Class II malocclusion. The treatment plan was to correct the malocclusion in a way that torques the roots more onto bone and to change her dental hygiene methods. With an extensive review of the literature, this case review attempts to make sense of the enigma of gingival recession and demonstrates an excellent treatment soluti…
