Lack of pyloric interstitial cells of Cajal explains distinct peristaltic motor patterns in stomach and small intestine.

The frequency and propagation velocity of distension-induced peristaltic contractions in the antrum and duodenum are distinctly different an … ave reached the pylorus, the duodenum distended after receiving gastric content and aboral duodenal peristalsis was initiated, often disrupting other motor patterns. The absence of ICC-AP and slow wave activity in the pyloric region allows the antrum and duodenum to have distinct uncoordinated motor activities. Coordination of aborally propagating peristaltic …

[Experimental use of duplex Doppler ultrasonography in the evaluation of intestinal motility in occlusive syndromes].

Intestinal motility is currently evaluated by means of clinical, radiologic and electromanometric methods. Of late, US has allowed to visualize the intestinal loops and their movements directly assessing wall patterns and endoluminal contents, as well as the degree of distension of the ileal loops; its diagnostic value is in fact well established in the study of such specific diseases of the GI tract as inflammatory conditions and neoplasms. On the contrary, the use of duplex-Doppler US to investigate bowel motility is quite new. We used duplex-Doppler US to evaluate intestinal peristalsis in …

Effect of aging and bolus variables on pharyngeal and upper esophageal sphincter motor function.

Effect of aging, bolus volume, temperature, and consistency on the pharyngeal peristalsis, as well as the effect of aging on the upper esophageal sphincter (UES) resting pressure and its response to esophageal distension by air and balloon, were studied in 14 young and 12 healthy elderly volunteers. In both age groups there was no significant volume or temperature effect on amplitude, duration, or velocity of the pharyngeal peristalsis. Compared with water swallows, mashed potato swallows resulted in a significant increase in the amplitude and duration of the hypopharyngeal peristaltic pressur…

Monitoring and evaluating the physiological changes in the horse with acute abdominal disease.

Initial examination and therapy, and the avoidance of maltreatment are emphasized. Gastric decompression is of prime importance, after which no compound should be administered via stomach tube. Where large amounts of high starch grains are fed, primary acute gastric dilatation must be differentiated from that secondary to small bowel dilatation, by immediate gastric intubation and irrigation of the cardia with lidocaine. If cessation of pain and improvement of peristalsis and general attitude follow, the former state may be assumed. If pain persists and peristalsis does not improve markedly, o…

Weight loss and decubitus duodenal ulcer in Parkinson's disease treated with levodopa-carbidopa intestinal gel infusion.

Apparently, unexplained weight loss is a common symptom experienced by patients affected by Parkinson's disease, especially in those treated by levodopa-carbidopa infusion gel (LCIG) with a poor control of dyskinesias. Weight loss is considered part of gastrointestinal dysfunction seen in patients affected by Parkinson's disease, along with gastroparesis and reduced bowel peristalsis. In patients treated with LCIG, weight loss needs to be accurately evaluated, because of possible underlying life-threatening adverse events, like duodenum decubitus ulcer.

Megacolon with infectious etiology that is infrequent in our country: Chagas disease.

We present the case of a 37-year-old male with constipation refractory to medical treatment, in the context of megacolon due to Chagas disease. The entire gastrointestinal tract may be affected but the digestive form is characterized by megaesophagus and megacolon. There is altered peristalsis due to the destruction of the neurons of the enteric nervous system caused by the parasite. Although the mortality rate is low, the disease can have a considerable impact upon quality of life. Chagas disease is poorly recognized and undertreated by healthcare providers in non-endemic regions.

Distal Esophageal Spasm: A Review.

Distal esophageal spasm is a rare motility disorder presenting principally with nonobstructive dysphagia and noncardiac chest pain. In symptomatic patients, the manometric diagnosis is made when >10% of the wet swallows have simultaneous and/or premature contractions intermixed with normal peristalsis. We characterize manometry and barium as complementary diagnostic approaches, and given the intermittent nature of the disorder, one should be always aware that it is almost impossible to rule out spasm. Treatment is difficult; we propose an approach beginning with the least invasive intervention…

Infantile hypertrophic pyloric stenosis with unusual presentations in Sagamu, Nigeria: a case report and review of the literature.

A 24-day old female Nigerian neonate presented with protracted vomiting, fever and dehydration but without palpable abdominal tumour or visible gastric peristalsis. There was no derangement of serum electrolytes. The initial working diagnosis was Late-Onset Sepsis but abdominal ultrasonography showed features consistent with the diagnosis of IHPS. This case report highlights the atypical presentation of this surgical condition and the need to investigate cases of protracted vomiting in the newborn with at least, ultrasonography to minimize complications and reduce the risk of mortality in a re…

MRI of bowel obstruction using the HASTE sequence.

High quality MR images of the gastrointestinal tract are now obtainable using a T2-weighted fast SE MR sequence with half-Fourier acquisition of data (the HASTE sequence). This fast sequence can acquire a set of images in a single breath hold and is insensitive to peristalsis-related artifact. It can be used with other imaging modalities in defining bowel obstruction and is accurate in determining the presence of bowel dilation and level of obstruction and in differentiating obstructing tumor from other causes of obstruction.

[Observation of the ultrastructure of smooth-muscle cells in the narrow segment of the ureter in children with congenital hydronephrosis].

The smooth-muscle cells of narrow segment in the ureter with congenital hydronephrosis in 5 cases were studied by transmission electron microscope. The result showed that the smooth-muscle cells of the narrow segment lack of myofilaments, dense body and dense patch in cells membrane. The above mentioned structures are functional deficient basis of the smooth-muscle cells in the ureter. Its deficiency results in the poor peristalsis function of the narrow segment of the ureter in congenital hydronephrosis. The cause and treatment principle in congenital hydronephrosis were discussed.

Applications and techniques of gastrointestinal intubation.

Gastrointestinal intubation has found increasing applications over the past several years. Although usually an uncomplicated procedure, passage of an intestinal tube may be impeded by ineffective peristalsis, obstructing lesions or surgical alteration of the normal anatomy. A variety of techniques are described for passing tubes into the stomach, duodenum and small bowel. These techniques are an outgrowth of those initially developed for manipulating tubes through the vascular, biliary and genitourinary systems. The characteristics of the various decompression and alimentation tubes as well as…

Surgical treatment of esophageal stricture in patients with scleroderma.

Two patients with dense distal esophageal strictures associated with progressive systemic sclerosis (scleroderma) are discussed. Both were treated with a Thal-Nissen procedure. Both patients now can eat without difficulty and have experienced no dysphagia. Although the peristalsis in the esophagus is markedly depressed or absent, food still can pass through the Nissen wrap into the stomach. The Thal-Nissen patch and fundoplication is an effective and well-tolerated operative procedure for the unusual patient with distal esophageal stricture and scleroderma.

Effect of Carbonated Water on Esophageal Motility in Different Body Positions: A Study Based on High-Resolution Manometry.

BACKGROUND: Carbonated water has long been a popular beverage, but its impact on esophageal motility remains unclear. This study aimed to evaluate the effect of carbonated water using high-resolution esophageal manometry (HRM). METHODS: A standard HRM protocol, incorporating measurements in both supine and upright positions per the updated Chicago Classification v4.0, along with additional carbonated water swallows, was performed in our motility laboratory. Ineffective esophageal motility (IEM) was diagnosed based on > 70% ineffective swallows or ≥ 50% failed peristalsis in the supine positio…

Dysmotility in the ileum of CFTR null swine.

BACKGROUND: Gastrointestinal (GI) complications are a common source of morbidity for people with cystic fibrosis (pwCF). The pathobiology of these clinical presentations is not fully understood, but there is evidence that gut dysmotility may be a primary contributor. METHODS: We studied gut motility in ileum samples from CF (CFTR-/-) and wild-type (WT) swine at birth (P0) and one week of post-natal life (P7) using organ bath assays. RESULTS: Ileal samples from both WT and CF swine displayed spontaneous peristalsis. CF swine presented with reduced basal amplitude of the peristaltic waves comp…

Mechanosensitive modulation of peristaltic contractions in the mouse renal pelvis.

The renal pelvis develops spontaneous phasic contractions (SPCs) that underlie pyeloureteric peristalsis. Increased urine flow into the renal pelvis mechanically stimulates the contractile machinery within the renal pelvis to facilitate the propagation of peristalsis. Here, the effects of mechanostimulation of the pelvicalyceal junction (PCJ), where SPCs originate from, on the properties of SPCs were investigated. Using the wire myograph, isometric tension changes in tubular preparations of mouse renal pelvis with calyces were circumferentially measured, while mechanostimuli were applied to th…

Primary Gastrointestinal Amyloidosis: An Unusual Cause of Acute Intestinal Pseudo-Obstruction.

Amyloidosis of the gastrointestinal tract is an uncommon disorder characterized by the extracellular deposition of an abnormal fibrillar protein. It is rarely proven by biopsy. Amyloid deposition interferes with organ structure and its function. We report a case of a 64-year-old male who presented with severe colicky pain, unable to pass feces, and progressive abdominal distension for 2 days. Physical examination revealed marked abdominal distension, visible peristalsis, high-pitched hyperactive bowel sounds, and generalized tenderness. Plain abdominal radiograph showed markedly diffuse dispro…

Response of colonic motility to dopaminergic stimulation is subverted in rats with nigrostriatal lesion: relevance to gastrointestinal dysfunctions in Parkinson's disease.

BACKGROUND: Constipation is extremely common in patients with Parkinson's disease (PD) and has been described in PD animal models. In this study, we investigated whether a PD-like degeneration of dopaminergic neurons of the substantia nigra can influence peristalsis in colonic segments of rats by impacting on enteric dopaminergic transmission. METHODS: Male, Sprague-Dawley rats received a unilateral injection of neurotoxin 6-hydroxydopamine (6-OHDA), or saline, into the medial-forebrain-bundle. Peristaltic activity was recorded in isolated colonic segments, in baseline conditions and followin…
