Assessing the Provision of Person-Centered Contraceptive Care at Publicly Supported Clinics Providing Contraceptive Services in the United States.

INTRODUCTION: We sought to understand the extent to which the contraceptive care provided at publicly supported family planning clinics in the United States aligns with aspects of person-centered care. MATERIALS AND METHODS: We conducted a descriptive study with a national sample of U.S. family planning clinics between November 2022 and December 2023. We measured person-centeredness by the scope of services offered, including the extent to which providers solicit and prioritize patients' contraceptive intentions, preferences, and goals; the dispensing protocols for various contraceptive metho

An environmental risk assessment of contamination of lakes and rivers in Northern Ireland with pharmaceuticals, personal care products and drugs of abuse.

In this work, we present the most spatially comprehensive environmental risk assessment of 114 pharmaceuticals, personal care products and illicit drug residues in the surface waters of Northern Ireland. We sampled at 50 sites spread across five lakes and rivers. We detected 21 different substances in 137 water samples and quantified between 0.7-2967 ng L-1 (n = 834 quantifiable measurements). Within these, the top five most frequently detected compounds were the opioid painkiller tramadol (91%), the cocaine metabolite benzoylecgonine (91%, with cocaine itself at 55% frequency), caffeine (80%)

A Personalized Muscle-Tendon Assessment and Exercise Prescription Concept Reduces Muscle-Tendon Imbalances in Female Adolescent Athletes.

BACKGROUND: Imbalances between muscle strength and tendon stiffness influence the mechanical demand on the tendon (i.e., tendon strain) and may increase tendon injury risk. The purpose of this study was to identify muscle-tendon imbalances and to promote a more balanced musculotendinous adaptation through a personalized assessment and exercise prescription concept in female adolescent handball athletes (13-16 years). METHODS: At four measurement time points during a competitive season, we used dynamometry and ultrasonography to assess knee extensor muscle strength, patellar tendon stiffness a

Identification and assessment of Electronic Aids for Daily Living considered essential by persons with high level tetraplegia: a case series.

Although assistive technology (AT) is recognized as a basic human right, access to AT, and particularly electronic aids to daily living (EADL), is limited. We aimed to understand how persons with high level spinal cord injury (SCI) prioritize EADL needs and assess satisfaction and efficacy of self-identified EADL. Thus, in this case series, we recruited three participants with C4, C5 or C6 SCI receiving in-patient SCI rehabilitation. Each received dedicated occupational therapy-based assistance in identifying EADL items within an unrestricted envelope of support ($5000 CDN) for use in maximizi

Inter-rater reliability of Centor score assessments between telemedicine and in-person examinations in patients with an acute sore throat.

BACKGROUND: It is uncertain whether the Centor criteria can be reliably assessed during telemedicine encounters with patients seeking care for a sore throat. Acquiring this knowledge is important as sore throat is a common reason for telemedicine consultations. OBJECTIVES: PRIMARY OBJECTIVE: To compare the inter-rater reliability of Centor score assessments via telemedicine versus in-person examinations. Secondary objectives: To investigate whether the interrater reliability varies when assessing patients who are children versus adults, and whether the telemedicine physician considered condit

Assessment for Antibodies to Rifapentine and Isoniazid in Persons Developing Flu-Like Reactions During Treatment of Latent Tuberculosis Infection.

BACKGROUND: Flu-like reactions can occur after exposure to rifampin, rifapentine, or isoniazid. Prior studies have reported the presence of antibodies to rifampin, but associations with underlying pathogenesis are unclear. METHODS: We evaluated PREVENT TB study participants who received weekly isoniazid plus rifapentine for 3 months (3HP) or daily isoniazid for 9 months (9H) as treatment for Mycobacterium tuberculosis infection. Flu-like reaction was defined as a grade ≥2 of any of flu-like symptoms. Controls (3HP or 9H) did not report flu-like reactions. We developed a competitive enzyme-lin

Patient Derived Xenografts (PDX) Models as an Avatar to Assess Personalized Therapy Options in Uveal Melanoma: A Feasibility Study.

Uveal melanoma is the most common primary intraocular malignancy in adults. Up to 50% of UM patients develop metastatic disease, usually in the liver. When metastatic, the prognosis is poor, and few treatment options exist. Here, we investigated the feasibility of establishing patient-derived xenografts (PDXs) from a patient's tumor in order to screen for therapies that the patient could benefit from. Samples obtained from 29 primary tumors and liver metastases of uveal melanoma were grafted into SCID mice. PDX models were successfully established for 35% of primary patient tumors and 67% of l

Assessing posttraumatic stress disorder symptom clusters with the Minnesota Multiphasic Personality Inventory-3 in a forensic disability sample.

OBJECTIVE: Previous evidence indicates that scales from the Minnesota Multiphasic Personality Inventory (MMPI) family of instruments can measure self-reported posttraumatic stress disorder (PTSD) symptomology and differentiate symptom clusters, including in forensic disability assessments. However, limited research has examined assessment of PTSD symptoms with the MMPI-3, the most recent MMPI instrument. The goal of the current study was to identify the strongest MMPI-3 scale predictors of individual PTSD symptom clusters, measured via self-report. METHODS: Using a sample of 716 disability cl

Using Neuroscientific and Clinical Context to Assess and Manage Changes in Core Personal Traits Caused by Deep Brain Stimulation.

In order to address recent debates on the extent to which deep brain stimulation (DBS) causes changes to core personal traits, Zuk and colleagues (2022) interviewed researchers about their experience with changes in personality, mood, and behavior while using DBS to treat a variety of neurological and psychiatric disorders. Here, we explore how considering these issues with respect to key neuroscientific and clinical details such as conditions and brain regions can aid interpretations of changes in core personal traits. We describe how clinicians treating Parkinson’s disease or obsessive-compu

Stakeholder views on cognitive communication assessment and intervention for a person living independently in the community with severe traumatic brain injury.

BACKGROUND: Cognitive communication disorder (CCD) following traumatic brain injury (TBI) is well documented and these communication problems impede successful re-integration into community living. While there is growing evidence for intervention to both detect and treat the impact of these deficits across the rehabilitation continuum, there are barriers to accessing services. Cognitive communication impairments may be missed because the person can talk, and this may mask the subtle but debilitating impact of a CCD. Referral to a speech and language therapist (SLT) may be overlooked or not tim

Assessing the contributions of modifiable risk factors to serious falls and fragility fractures among older persons living with HIV.

BACKGROUND: Although 50 years represents middle age among uninfected individuals, studies have shown that persons living with HIV (PWH) begin to demonstrate elevated risk for serious falls and fragility fractures in the sixth decade; the proportions of these outcomes attributable to modifiable factors are unknown. METHODS: We analyzed 21,041 older PWH on antiretroviral therapy (ART) from the Veterans Aging Cohort Study from 01/01/2010 through 09/30/2015. Serious falls were identified by Ecodes and a machine-learning algorithm applied to radiology reports. Fragility fractures (hip, vertebral,

Assessing the efficacy of a stepped-care group treatment programme for borderline personality disorder: study protocol for a pragmatic trial.

BACKGROUND: Borderline personality disorder (BPD) is a high prevalence and serious mental health disorder that has historically challenged the finite resources of health services. Despite empirical evidence supporting structured psychological therapy as the first line of treatment, there remains significant barriers in providing timely access to evidence-based treatment for this population. The primary aim of this study is to evaluate the effectiveness of providing a stepped-care structured psychological group treatment to individuals with BPD within local mental health services. The secondary

Cohort study: Apparent redundancy of fibrosis assessment in young persons with HCV; development of realistic approaches to break the paradigm.

INTRODUCTION AND OBJECTIVES: Hepatitis C Virus (HCV) is a blood-borne, hepatotropic RNA virus causing both acute and chronic infection. Chronic HCV infection predisposes individuals to liver fibrosis, cirrhosis and hepatocellular carcinoma. Staging of fibrosis prior to treatment to determine either treatment choice or required follow up, is standard practice. However, this often acts as a barrier to treatment initiation. We sought to validate the hypothesis that those individuals; mono-infected with HCV, ≤35 years of age; with no additional hepatic insult were unlikely to have significant fibr

Assessing recall of personal sun exposure by integrating UV dosimeter and self-reported data with a network flow framework.

BACKGROUND: Melanoma survivors often do not engage in adequate sun protection, leading to sunburn and increasing their risk of future melanomas. Melanoma survivors do not accurately recall the extent of sun exposure they have received, thus, they may be unaware of their personal UV exposure, and this lack of awareness may contribute towards failure to change behavior. As a means of determining behavioral accuracy of recall of sun exposure, this study compared subjective self-reports of time outdoors to an objective wearable sensor. Analysis of the meaningful discrepancies between the self-repo
