Incontinentia pigmenti achromians (Ito).

A case of incontinentia pigmenti achromians syndrome with associated abnormalities occurring in a 21-year-old woman is reported. Since birth she has had whorling macular, hypopigmented areas on the skin of the lateral, anterior, and posterior aspects of the trunk. Histopathological picture: the melanocytes in the depigmented areas are slightly reduced in number. Some of the associated abnormalities that she exhibits, such as coarse curly hair on her scalp, and pigmented spots in her left iris, have never before been described. Other of her symptoms--thick lips and myopia--have been reported ea

Zebrafish Syndromic Albinism Models as Tools for Understanding and Treating Pigment Cell Disease in Humans.

Melanin is the pigment that protects DNA from ultraviolet (UV) damage by absorbing excess energy. Melanin is produced in a process called melanogenesis. When melanogenesis is altered, diseases such as albinism result. Albinism can result in an increased skin cancer risk. Conversely, black pigment cell (melanocyte) development pathways can be misregulated, causing excessive melanocyte growth that leads to melanoma (cancer of melanocytes). Zebrafish is an emerging model organism used to study pigment disorders due to their high fecundity, visible melanin development in melanophores (melanocytes

7-HYDROXYCHLORPROMAZINE: POTENTIAL TOXIC DRUG METABOLITE IN PSYCHIATRIC PATIENTS.

7-Hydroxychlorpromazine was isolated from liver and urine of psychiatric patients treated with high dosages of chlorpromazine for long periods. These patients developed a purple pigmentation limited to areas of skin exposed to sunlight, as well as opacities of the cornea and lens. Large amounts of melanin were found only in the pigmented skin. It seems likely that a toxic compound is produced by the action of ultraviolet light on a 7-hydroxylated chlorpromazine derivative deposited in skin, cornea, and lens, and that this in turn leads to excessive melanin production and eye changes.

Effects of jogging exercise on patients with the pigmentary dispersion syndrome and pigmentary glaucoma.

BACKGROUND: Exercise-induced anterior chamber pigment dispersion with intraocular pressure (IOP) elevation has been reported in patients with the pigmentary dispersion syndrome. Marked pigment dispersion with or without elevation of IOP could predispose these patients to visual field loss. The authors designed this study to evaluate the effects of jogging exercise on anterior chamber pigment and IOP in a group of patients with the pigmentary dispersion syndrome or pigmentary glaucoma. METHODS: Fourteen subjects with the pigmentary dispersion syndrome, 10 subjects with pigmentary glaucoma, and

[Hereditary pigmentary disorders].

Pigmentation in human skin differs individually and is regulated by more than 100 genes. The discovery of an increasing number of these genes has shed light on the molecular basis and pathogenesis of genetic pigmentary disorders. They are very rare and can be caused by changes in melanocyte number or melanin synthesis as well as development, transport and transfer of melanosomes. Pigmentary disorders can be divided into hyper- and hypopigmentation, of which the distribution can be diffuse or localized. Localized hypopigmentation can be found in piebaldism, Waardenburg syndrome and Tietz syndro

Genetic immunodeficiency disorders.

In this review, selected immunodeficiency disorders are presented in which the cutaneous signs are distinctive and contribute to the diagnosis of the condition. Among these cutaneous abnormalities are alopecia, cutaneous granulomas, cutaneous infections, atopic-like or seborrheic-like dermatitis, petechiae or purpura, silvery pigmentation, poor wound healing, and telangiectasias. Immunodeficiency should be considered in children with a history of infections that are recurrent, respond poorly to antibiotics, are of increased duration and severity, and/or result from unusual organisms. Asymmetrical Macular Thinning on Optical Coherence Tomography (OCT) in Pigmented Paravenous Retinochoroidal Atrophy.

Pigmented paravenous retinochoroidal atrophy (PPRCA) is a rare, typically non-progressive retinal disorder characterized by perivenous atrophy of the retinal pigment epithelium and choriocapillaris, with macular involvement being uncommon. This report aims to highlight a novel manifestation of asymmetrical macular thinning in PPRCA. A 40-year-old man presented with bilateral, gradual visual decline. Fundus examination revealed perivenous retinochoroidal atrophy with pigmentation along the arcades in both eyes and notable macular changes. Spectral-domain optical coherence tomography (OCT) demon

Subretinal autofluorescent deposits: A review and proposal for clinical classification.

Subretinal autofluorescent deposits (SADs) may be found in the posterior pole, associated with very various conditions. These disorders usually present a typical pattern of autofluorescent lesions seen on short-wavelength fundus autofluorescence. We describe SADs according to their putative pathophysiological origin and also according to their clinical pattern, i.e., number, shape, and usual location. Five main putative pathophysiological origins of SADs were identified in disorders associated with an intrinsic impairment of phagocytosis and protein transportation, with excess of retinal pigme

The story of melanocyte: a long way from bench to bedside.

Skin is composed of major layers, namely a superficial epidermis and a deeper dermis. The color of skin is influenced by a number of pigments, including melanin, which is produced by cells called melanocytes. Most skin disorders are relatively benign, but a few, including melanomas, can be fatal. A number of more noticeable disorders, namely albinism and vitiligo, affect the appearance of the skin and its accessory organs. Vitiligo is associated with significant psycho-social morbidity and a major effect on quality of life. Topical steroids, calcineurin inhibitors, phototherapy and surgery are

Electrophysiological responses related to the pigment epithelium and its interaction with the receptor layer.

The standing potential (SP) of the eye is generated mainly at the apical surface of the pigment epithelium, where tight junctions between adjacent cells create a high resistance membrane. It can be recorded indirectly by means of the electrooculogram (EOG) or directly by recording between electrodes connected to the cornea and the forehead. Changes in illumination provoke slow SP changes in the form of damped oscillations with a frequency of about 2/hour. These oscillations are abolished or substantially reduced in pigment epithelial diseases, such as retinitis pigmentosa and vitelline macular

The use of nonhuman primates in the study of bilirubin metabolism and bile secretion.

Close similarities exist in the chemical structure of fetal and adult bile pigments and mechanisms for hepatic bilirubin conjugation and transport in human and nonhuman primates. Newborn monkeys, unlike other animals, develop physiologic hyperbilirubinemia and are ideal animal models in which to investigate this developmental human disorder. Aberrations in bile pigment metabolism and compartmentalization which occur in hemolytic, hepatocellular, and obstructive diseases, closely resemble those reported in counterpart syndromes in man. Similarities also exist in the mechanisms responsible for t

Established treatments of skin hypermelanoses.

Cutaneous hypermelanoses are frequently encountered conditions that can have severe adverse psychosocial and emotional effects on affected patients. Melasma, postinflammatory hyperpigmentation, drug-induced pigmentation, and erythema dischromicum perstans are among the most common cutaneous disorders leading to acquired skin hyperpigmentation. The treatment of these disorders is often challenging and requires a great deal of patience from the patient and a wealth of experience and knowledge from the dermatologist. Current treatments include depigmenting agents, chemical peels, and lasers. The

[Pigment dispersion in a family with albinism (author's transl)].

A report is given on a family wherein two males showed albinism; of the female members, who all had been light blonde in childhood, two were carriers (one of them with pigment changes in the iris and the fundus), a third had no ocular changes, while the fourth showed the typical phenomenon of bilateral idiopathic pigment dispersion without glaucoma. The occurrence of pigment dispersion in a family with albinism has not yet been described. It is regarded as accidental combination.

Enhancer of garnet/deltaAP-3 is a cryptic allele of the white gene and identifies the intracellular transport system for the white protein.

The white gene encodes an ABC-type transmembrane transporter that has a role in normal eye pigment deposition. In addition, overexpression in Drosophila leads to homosexual male courtship. Its human homologue has been implicated in cholesterol transport in macrophages and in mood disorders in human males. The garnet gene is a member of a group of other Drosophila eye colour genes that have been shown, or proposed, to function in intracellular protein transport. Recent molecular analysis indicates that it encodes the delta subunit of the AP-3 adaptin complex involved in vesicle transport from t

Pigmented purpuric dermatosis and hepatitis profile: a report on 10 patients.

BACKGROUND: Pigmented purpuric dermatosis comprises a group of vascular disorders of unknown etiology. Histologically, it is characterized by lymphocytic capillaritis in the papillary dermis. Although leukocytoclastic vasculitis confined to the skin is frequently reported with hepatitis C, lymphocytic vasculitis is rarely reported. METHODS: Ten patients with pigmented purpuric dermatosis were studied clinically and histopathologically. Hepatitis profile was carried out in all of the patients to evaluate the possible relation. RESULTS: Of the 10 patients, five tested positive for hepatitis C

Progressive pigmented purpuric dermatitis and alopecia areata as unusual skin manifestations in recognizing hereditary hemochromatosis.

Hereditary hemochromatosis (HHC) is a common genetic disorder of iron overload, caused by mutations in the HFE gene. If untreated, abnormal accumulation of iron may lead to organ damage and premature death. Significant changes in the symptomatology of HHC have been observed in recent years, and its full clinical expression is rarely seen. The disorder presents a large phenotypic heterogeneity. We report a case of newly identified HHC in a 56-year-old man presenting as pigmented purpuric dermatitis and alopecia areata affecting the beard, accompanied with elevated liver enzymes and elevated ser

A novel approach to gene therapy of albino hair in histoculture with a retroviral streptomyces tyrosinase gene.

In order to induce melanin production in mammalian cells with pigment disorders such as albino hair, a recombinant retrovirus containing the mel locus of Streptomyces antibioticus was constructed. The S. antibioticus mel locus, which consists of the open reading frame (ORF)-438 and the tyrosinase gene, was specifically derived by polymerase chain reaction (PCR) from Streptomyces plasmid pIJ702. The ORF-438 is required for the transfer of copper to apotyrosinase, which is essential for tyrosinase enzymatic activity. The tyrosinase gene was inserted into the XhoI/BamHI cloning site of the pLXSN
