OBJECTIVE: Our purpose was to identify what anesthetic method is safer for women with a placenta previa.
STUDY DESIGN: We retrospectively reviewed all women with placenta previa who underwent cesarean delivery during the period January 1, 1976-December 31, 1997 at Northwestern Memorial Hospital.

Neuroblastoma, a tumor of primitive neuroectodermal cells, is one of the most common solid malignancies of neonates. Placental metastases have been described in rare cases of congenital neuroblastoma, usually in association with hydrops fetalis, placentomegaly, and widespread disease in the neonate. We report a case in which the unexpected diagnosis of neuroblastoma was made based on the postnatal finding of tumor emboli confined to fetal vessels of the placenta. Radiation and chemotherapy were given based on this diagnosis, obviating the need for an invasive diagnostic procedure.

We present the case of a 38-years-old tertipara at full term with a diagnosis by ultrasound of placenta previa who underwent elective cesarean under spinal anesthesia. After a difficult birth, anomalous adhesion of the placenta prevented its removal and caused massive hemorrhage of the placental bed and hypovolemic shock. The placenta, normally confined to the decidual lining of the uterine cavity, can in some circumstances invade the muscular wall of the uterus, a condition known as placenta accreta. Ultrasound scan showed some degree of placenta previa in 503 patients. Of this group, 5.6% (28) had clinically significant bleeding or documented placenta previa at delivery. A scoring system based on placental localization was not successful in predicting who was at high risk for bleeding. However, the likelihood of clinically excessive bleeding did significantly increase if the placenta previa was noted after 30 weeks' gestation. The study did not find a greater incidence of small-for-gestational-age babies in women with low-lying placentas.
