BACKGROUND: Orbital polymyositis associated with giant cell myocarditis rarely has been reported in the literature. The authors report the clinical, neuroradiographic, and histopathologic features of the only patient to survive this usually fatal syndrome after cardiac transplantation.

We describe a 42-year-old Chinese patient with polymyositis and nasopharyngeal carcinoma. The polymyositis was complicated by respiratory failure needing mechanical ventilation despite conventional therapy. Treatment of the carcinoma was associated with recovery of muscle strength. To our knowledge this is the first reported association of polymyositis with nasopharyngeal carcinoma.

An association between polymyositis-dermatomyositis (PM/DM) and malignancy has been reported. However, controversy exists regarding a direct causal relationship between the two diseases. We describe a patient who had had PM and subsequently developed non-Hodgkin's lymphoma, and discuss the possibility of their causal relation.

We describe 2 patients with dermatitis herpetiformis who developed polymyositis/dermatomyositis. On HLA typing, both patients were found to be HLA-B8, DR3 positive. The concurrence of these two relatively rare diseases, both associated with immunologic abnormalities, further supports the role of autoimmunity in their pathogenesis and indicates a possible common genetic basis. It also suggests that myositis may be more common in patients with dermatitis herpetiformis than in the general population.

Dermatomyositis-polymyositis was associated with gynecologic carcinoma in ten patients treated at the Mayo Clinic between 1952 and 1982. Only two patients did not manifest the typical skin changes of acute dermatomyositis. In all ten cases, the onset of dermatomyositis-polymyositis preceded evidence of carcinoma. A 42-year-old woman with a sicca syndrome associating xerostomia and xerophthalmia developed proximal motor weakness in all four limbs due to a polymyositis. Regression of the motor deficiency followed the administration of corticoid therapy. Seven (9.2%) of 76 patients with adult-onset polymyositis/dermatomyositis (PM/DM) seen over an 11-year period had cutaneous vasculitis. This was manifest by dermal and/or subcutaneous nodules in 4, periungual infarcts in 3 and digital ulceration in 2. A 39-year-old woman presented with palpitations, presyncope and muscular weakness. Electrocardiographic monitoring and electrophysiological studies confirmed the tachycardia-bradycardia form of the sick sinus syndrome and subsequent investigations including an endomyocardial biopsy demonstrated polymyositis as the cause.
