OBJECTIVES: To investigate the effect of forefoot varus on single-limb stance postural stability (PS).
BACKGROUND: Impaired PS has been implicated as a potential risk factor for sustaining acute foot and ankle injuries. The identification of variables that deleteriously affect PS may be important in the prevention of future injuries.

The purpose of this study was to measure and describe postural aberrations in chronic and acute low back pain in search of predictors of low back pain. The sample included 59 subjects recruited to the following three groups: chronic, acute, or no low back pain. Diagnoses included disc disease, mechanical back pain, and osteoarthritis. In the present paper a series of experiments will be described in which the postural consequences of long (up to 1.5 h) duration 3G-centrifugation have been subject to investigation. A vestibular model is presented in order to explain for the postural imbalance found in specific conditions after the centrifuge run, and for the often concomitant feelings of motion sickness (50% of the subjects suffer from Sickness Induced by long duration Centrifugation). Finally, the possible relation between SIC and the Space Adaptation Syndrome is discussed.

Each time he stood up a 60-year old man experienced myoclonic jerks, quickly followed by a syncope due to major postural hypotension. Progressive autonomic failure was associated with pyramidal, extrapyramidal and cerebellar features in a pattern characteristic of Shy-Drager disease. Myoclonic jerks suggested seizures, but no electroencephalographic epileptiform activity was found. This case together with data from the literature suggest, that such involuntary movements may result from cerebral anoxia.

The relationship between upright static and dynamic equilibrium function and idiopathic scoliosis was investigated using a seven-part postural equilibrium test battery. Twenty-three normal subjects and 31 subjects with differing degrees of idiopathic scoliosis participated in the study. No significant difference was found among any of the subtest scores of the normal and scoliotic groups. Subtest scores were not significantly correlated with curve measurements. In a normative study of amplitude, frequency, and variability of postural tremor of the hand in 97 normal subjects, 15 to 80 years old, tremor was transduced using accelerometers and characterized by power spectral analysis. Six successive cases of congenital nephrotic syndrome are described. Each one showed flexion deformities of the knees and hips, widely open anterior and posterior fontanelles, and wide separation of the skull sutures. These abnormalities were present not only in cases in which the renal histology was of the microcystic Finnish type of congenital nephrotic syndrome, but also in those in which the histological picture was one of the variants associated with congenital nephrotic syndrome. It is suggested that such abnormalities are postural deformities, possibly produced by the large placenta.

Tests of postural rotational laterality were carried out in 63 patients suffering from torticollis (53) or torsion dystonia with torticollis (10) and in 33 normal subjects. There was a correlation between the predominant direction of postural functions and the direction of the rotatory component of torticollis. Torticollis and torsion dystonia may be due to the release of a postural rotational laterality in pre-disposed subjects and not to a presumed asymmetric lesion of the brain-stem.

The clinical response to therapy with dihydroergotamine (DHE) has been evaluated in one patient with idiopathic orthostatic hypotension. The results of tests performed on the autonomic nervous system are recorded, and the evidence for a central defect in postural blood pressure control is presented. Tilt table testing over a two-month period confirmed the beneficial effect of the medication in preventing profound falls of blood pressure. The dose required (20 mg/day) was higher than previously reported.

A study was conducted to evaluate the effect of indomethacin on orthostatic hypotension in Parkinsonism. Twelve elderly patients participated and the drug was given in two-ways--as an intravenous infusion of 50 mg over 30 minutes and by mouth 50 mg thrice daily for six days. Results were assessed by measuring the degree of hypotension on standing, response to the cold pressor test, and forearm blood flow (by strain-gauge plethysmography).
