Clients' characteristics as mediating variables in perception of counselors' social influence.

48 clients in a rehabilitation center viewed two 12-min, videotapes each, one depicting a counselor high in social influence and the other one of low influence. They then rated the counselors on the Counselor Rating Form and the Empathy subscale of the Barrett-Lennard Relationship Inventory. Black subjects and subjects at lower educational levels rated the counselor of low social influence significantly higher than did whites and better educated subjects on all measures. White subjects and subjects at higher educational levels rated the counselor of high social influence significantly higher o

Reliability of quantitative strength measurements in children.

In normal children, sex, age, height and weight account for approximately 50% to 70% of variance in the strength scores obtained by quantitative measurements. Although quantitative techniques accurately measure the forces generated by muscle contraction, technical and psychological variables may cause some inconsistency of scores and contribute to the variance observed. To explore the role of these factors, reliability of quantitative strength measurements was examined in children withe normal intelligence and with mild mental retardation using the isokinetic method. Results showed mean score

Treatment failure in cognitive-behavioural therapy: therapeutic alliance as a precondition for an adherent and competent implementation of techniques.

OBJECTIVES: Treatment failure is a common phenomenon, but little is known about the reasons. Therapeutic alliance, therapist adherence, and therapist competence are considered important aspects of treatment success and formed the focus of the current investigation. DESIGN: Three randomized controlled trials for the treatment of depression, social phobia, and hypochondriasis were the basis of the current study. METHODS: The role of therapeutic alliance, as well as therapist adherence and competence, were investigated in 61 patients, which were classified either as treatment failure or as trea

The three critical components in the conservative treatment of juvenile osteochondritis dissecans (JOCD). Physician, parent, and child.

There has been explosive growth of intensive, year-round sports for children of both sexes, with a resulting high degree of family involvement and commitment. Consequently, a diagnosis of juvenile osteochondritis dissecans (JOCD) impacts parent and child on many different levels. Compliance is the key element when a conservative treatment plan for JOCD is appropriate. An informed physician understands the dynamics between the "compliance triad" of physician, parent, and child, and recognizes that compliance is a process with foreseeable stages and characteristic behaviors.

Community pharmacist's responsibilities with regards to traditional medicine/complementary medicine products: A systematic literature review.

BACKGROUND: The use of Traditional Medicine/Complementary Medicine (TM/CM) products has gained popularity in many countries. There is a growing body of evidence to support that concomitant use of TM/CM products with certain pharmaceutical medicines may adversely affect treatment outcomes. There is a general consensus that pharmacists have a role to play in the safe and appropriate use of these products. However, the extent of their involvement and responsibilities are not yet defined. Clear guidelines that inform their duty of care are essential for pharmacists to establish their role in the m

The impact of opioid stigma on patient-provider interaction and decision-making in cancer pain management: a qualitative meta-synthesis.

CONTEXT: Clinical guidelines recommend opioids for moderate-to-severe cancer pain, yet undertreatment remains common. In the context of the opioid crisis and increasingly restrictive regulatory policies, communication and decision-making around cancer pain management have become more challenging. Opioid stigma may be an overlooked influence on how patients and healthcare providers communicate and make treatment decisions. OBJECTIVES: To explore how opioid stigma influences patient-provider interaction and decision-making through a qualitative meta-synthesis. METHODS: This qualitative meta-sy

Recognition and Resistance in Early Psychotherapeutic Encounters: Therapist Response Style, Narcissistic Admiration and Rivalry, and Public Mental Health Engagement.

Early engagement with psychotherapy is a public mental health issue because potential patients' first appraisals of psychological care may shape treatment expectations, willingness to continue, and openness to receiving effective support. In first-contact therapeutic encounters, people respond not only to the content of a therapist's intervention but also to the interpersonal meaning conveyed by the therapist's response style. Guided by a recognition-resistance framework and models of narcissistic self-regulation, we examined how therapist response style and trait narcissistic admiration and r

The development of the therapeutic alliance during the first five hypnotherapy sessions.

The therapeutic alliance is a principal element that allows the dynamics and effects of psychotherapy to be analyzed. In the past half-century, many studies have explored various psychotherapeutic approaches, including psychoanalytic, cognitive-behavioral and systemic psychotherapy, but hypnotherapy has not been addressed. This article presents the first analysis using current methods of verifying and understanding the dynamics of change in hypnotherapy, regarding to the therapeutic alliance. Luborsky et al.'s (1996) revised Helping Alliance Questionnaire (HAq-II) was administered to 59 patien

Therapist's positive evaluation of children with hearing impairment: A conversation analysis perspective.

In speech-language therapy, the evaluation of children with atypical speech embodies the practical expertise of the therapist and the therapeutic goal of facilitating rehabilitation by guiding these children toward more typical speech behaviors. This study aims to provide an interactional explanation of how positive evaluations given by speech therapists are sequentially formatted, constructed, and oriented in therapy practice and what interactional consequences they have on the interpretation and learning of children with hearing impairment. Adopting conversation analysis as a methodology, th

Cancer patients' experiences of moments of relational depth during psychological therapy: a qualitative study.

OBJECTIVE: Research into psychological interventions for cancer patients has focused on the effectiveness of different treatment methods. Investigating common factors across treatments, such as qualities within the therapeutic relationship, has been neglected. This study explores cancer patients' experiences of moments of profound contact and engagement with their therapist, including any perceived impact. METHODS: Semi-structured interviews were conducted with ten cancer patients. Eight participants reported having experienced moments of relational depth. Their transcripts were analyzed usin

Exploring the processes of connection and disconnection in imagery work in a patient with depression and dependent personality disorder.

Imagery work is a useful therapeutic tool in the treatment of depression. It is central in different therapeutic approaches, such as cognitive behavioral therapy and compassion-focused therapy. The clinical case of Cynthia is presented. Cynthia started therapy for severe depression associated with social anxiety and dependent personality disorder. At different key moments in the therapy, the proposed change strategies led to ruptures in the therapeutic alliance. For example, difficulties in accepting depression, fears of being judged by the therapist, guided imagery of compassion and work on t

Patients Describe their Lived Experiences of Battling to Live with Complex Regional Pain Syndrome.

Complex regional pain syndrome (CRPS) has never comprehensively been examined from a lived experience perspective. Patients generally have a poorer quality of life than people with other chronic pain conditions. This study aimed to understand the essence of living with CRPS. Data were collected from 17 patients via in-depth interviews. Hermeneutic discussions with four health professionals generated deeper insights. Internet blogs and a book containing patient stories were included for theme verification and triangulation. CRPS is seen as a war-like experience and five themes were identified w

The Phenomenology and Dynamics of Wealth Shame: Between Moral Responsibility and Moral Masochism.

In an age of striking inequality in wealth, a related phenomenon, wealth shame, has developed. A multidisciplinary exploration of such shame examines its intrapsychic, intersubjective, transgenerational, and sociopolitical roots in the U.S., as well as its multiple functions: as an ethical response to economic disparity (moral responsibility), as a manifestation of a pervasive shame pattern (moral masochism), and as a defense against pleasure, feelings of superiority, and the fear of being envied. Several clinical vignettes illustrate these themes and are followed by reflections on their clini

Utilizing an ecological framework to integrate social identities and sociopolitical analysis in psychotherapy.

This article explores how the political events from the past year have influenced psychotherapeutic sessions with a female adolescent who is Peruvian, undocumented, and initially presented with symptoms of anxiety and a history of trauma. Focusing on this case example and utilizing an ecological framework, this article explores how the social identities of the therapist and their relation to power and privilege impact the therapeutic interventions that were chosen and not chosen. Additionally, there is a review of the benefits of consultation with a colleague of differing social identities and
